Occasionally, patients with gastrointestinal Crohn's disease (CD) develop granulomatous skin lesions at sites remote from the gastrointestinal tract, separated from other ulcerations by normal skin, a phenomenon that has been referred to as metastatic cutaneous CD. Although metastatic CD of the vulval region has been often reported in English literature, we could not find such cases with family history. We report a case of vulval CD occurring in siblings.

6.Clinical Analysis of Down Beat Nystagmus in Atypical Positional/ing Vertigo .

BACKGROUND AND OBJECTIVES:Traditionally, down beat nystagmus is regarded as a sign of central nervous system dysfunction. But, several years has passed since Herdman et al reported the down beat nystagmus developed during treatment maneuvers for posterior semicircular canal benign paroxysmal positional vertigo(BPPV). We undertook this study to evaluate the character and clinical analysis of the positional or positioning down beat nystagmus, to discuss the clinical significance of positional or positioning down beat nystagmus as a diagnostic criteria of superior semicircular canal BPPV, and to propose the new treatment method. MATERIALS AND METHOD:From November 1999 to March 2004, we sampled the 103 patients with positional or positioning down beat nystagmus. Of these patients, we selected 16 patients except for the patients with central nervous system dysfunction, nonspecific or artifact result, idiopathic origin. RESULTS:All of 16 patients had no sign and radiologic result of central nervous system disorder. 10 patients was reported or suspected the diagnosis of posterior semicircular canal BPPV. Fatigability was reported in 9 patients and reversibility was reported in 1 patient. Average latency was checked less than 2 seconds. CONCLUSION:Although the diagnostic criteria of superior semicircular canal BPPV that we reported was not controversial, we expect that this criteria is useful in diagnosis for patients with atypical positional or positioning down beat nystagmus. And the new treatment method that we reported will has the better results than previous method.

7.A case of fetal atrial flutter with hydrops fetalis.

Fetal hydrops is often serious and associated with a high perinatal motality rate. Cardiac causes of fetal hydrops include congenital heart diseases and rhythm disturbances. An irregular fetal heart rate may indicate atrial fibrillation and atrial flutter with variable AV conduction. Fetal atrial flutter is characterized by the pressence of flutter waves which are regular sawtooth undulations in the baseline that are larger than p waves. Authors experienced a case of fetal atrial flutter with hydrops fetalis at 30 week's gestation which was confirmed by fetal M-mode echocardiogram and electrocardiography. A new born infant had shown to have atrial flutter in utero and after delivery was successfully converted to normal sinus rhythm with digoxin and quinidene.

8.Gender Role Identity and Quality of Life of Hysterectomy Women Receiving Hormone Replacement Therapy.

PURPOSE: The purpose of this study was to investigate the degree of the gender role identity and quality of life of women applying hormone replacement therapy with hysterectomy and to identify the relation among these variables.The purpose of this study was to investigate the degree of the gender role identity and quality of life of women applying hormone replacement therapy with hysterectomy and to identify the relation among these variables. METHODS: The subjects were 121 outpatients applying hormone replacement therapy with hysterectomy at a university hospital in Busan province. The data were collected using self-reported questionnaire and medical record, from September to October 2008. RESULTS: There were statistically significant differences in the degree of quality of life according to the age, educational level, occupation, family economic status, Body Mass Index (BMI), menopause state, duration to restarting intercourse from hysterectomy, discomfort from intercourse, satisfaction from intercourse and duration, sources, and formulation of hormone replacement therapy (HRT). The femininity identity group occupied the largest proportion (31.4%), and then the masculinity identity group (28.1%), the undifferentiated identity group (26.4%), and finally the androgyny identity group (14.1%). There was a positive correlation between femininity and quality of life (r=.276, p=.002). CONCLUSION: These results indicate that HRT helps the women's sense of loss caused by hysterectomy to be reduced, the postmenopausal symptom after hysterectomy to be relieved and the quality of life to be improved.

9.A Case of Dyschromatosis Symmetrica Hereditaria.

Dyschromatosis symmetrica hereditaria(DSH), also known as reticulated acropigmentation of Dohi, is a rare pigmentary disorder that has autosomal dominant inheritance. Characteristic lesion includes a mixture of hyperpigmented and hypopigmented macules with reticulate pattern, which are distributed on the dorsal aspects of the extremities. We report a 5-year-old boy who showed typical DSH with family history in four generations.

10.Superficial Candidal Infection Occurred on the Site of Epidermal Transplantation.

An epidermal grafting using the top of suction blister has been used successfully for the treatment of depigmented skin, such as vitiligo, achromic areas after dermabrasion, and burns. A 9-year-old girl showed the delayed wound healing of epidermal grafting site for the treatment of vitiligo on popliteal fossa. Direct microscopic examination (KOH) from the wound showed numerous hyphae. Fungus culture and germ tube test confirmed Candida albicans from the wound surface. The wound healed by antifungal treatment, leaving scar.
