3.Emergency Nurses' Professional Quality of Life: Compassion Satisfaction, Burnout, and Secondary Traumatic Stress.

PURPOSE: Professional quality of life is the quality one feels in relation to their work as a helper. The purpose of this study was to describe professional quality of life among emergency nurses. METHODS: A total of 15 emergency rooms were selected in three cities. Among 263 nurses working at these emergency rooms, 178 nurses consented to participate in this cross-sectional survey. Professional Quality of Life (ProQOL) Scale version 5 was used to measure compassion satisfaction, burnout and secondary traumatic stress. Cluster analysis was used to classify nurses according to professional quality of life. RESULTS: The mean scores (SD) for compassion satisfaction, burnout, and secondary traumatic stress were 32.12 (5.45), 28.27 (4.28), and 28.20 (5.07), respectively. The result of cluster analysis according to standardized score of compassion satisfaction, burnout, and secondary traumatic stress yielded three clusters. Over 50% of participants showed low professional quality of life. Nurses who were included in a cluster representing low professional quality of life were younger, had shorter periods of nursing experience, and perceivedlower social support than other cluster. CONCLUSION: Education or support programs for emergency nurses are needed to enhance their professional quality of life.

4.Diseases and Health Service Utilization Patterns of Geriatric Inpatients.

BACKGROUND: Recently elderly people have increased in Korea and increased demands of medical service. This study was carried out to identify diseases and health service utilization patterns of geriatric inpatients to provide better understanding and basis planning to meet the needs of expanding geriatric population. METHOD: We use data of `Patient Survey in Korea` in 1999 and analyzed characteristics of disease structure and utilization patterns of health care service of 54,013 cases of discharged patients aged 65 years or more. RESULT: The proportion of the elderly in total discharged patients is 14.7%. Male to female ratio is 1:1.3 and most common age group was between 65 and 69 years old. 84.2% of elderly patients were admitted at secondary or tertiary health care facilities. The most common disease in geriatric inpatients is malig- nancy in male and vascular disease in female. The average duration of hospitalization is 14.2 days. They stay longer than the nation wide average hospital length and stay longest when they are admitted by mental and behavioral disorders. CONCLUSION: Geriatric patients are admitted mostly with chronic degenerative diseases such as neoplasm and vascular disease. Elders prefer to be admitted to tertiary hospitals and oriental hospitals. They stay more in hospital.

6.A Study on the Skin Thickness of Koreans by Ultrasound.

The authors have used a B-mode real-time sector scanner and a 10-MHz probe for measurement of the skin thickness at 21 different sites in healthy Koreans. Subjects consisted of 35 adults(17 males, 18 females) who were divided into three age groups .' young(20 39 years, 12 subjects), middle-aged(40 59 years, 13 subjects), and old(over 60 years, 10 subjects). The results were as follows. 1, The measurements of the skin thickness in examined sites varied from 1.99 to 4.59 milimeters . flexor aspect of the upper arm, dorsum of the hand, extensor aspect of the forearm and flexor aspect of the forearm were thinner, and heel, upper back, metatarsus were thicker than other sites. 2, The skin thickness in males was generally thicker than that in females '. cheek, extensor aspect of the thigh were highly significantly thicker (p<0.01), and chest, upper back, lower back, flexor aspect of the upper arm and extensor aspect of the lower leg were significantly thicker(p< 0.05) in males than in females, 3. The skin was thickest in the old age groups.

7.Criterion Development of 4 Activity Levels for Estimated Energy Requirement Calculation for Primary School Students.

The purpose of this study is finding the easy way of 4 categories activity level confirmation for estimated energy requirement calculation. Total of 386, 5th and 6th grade primary school students participated. The time spent on 7 kinds of activity were collected for 1 day by the internet program developed. Judged by the activity coefficient, sedentary were 6.7% and 5.1%, low active 33.2% and 40.4%, active 43.8%, and45.5%, and very active 16.3% and 9.0% for boy and girl, respectively. The highest and significant correlation coefficient between activity coefficient and time spent on activities shown were 0.339 in commute activity for boys, and 0.466 in leisure for girls. The sensitivity of the sedentary conformation by commute hour for boys was 0.79, and that of very active was 0.56. The sensitivity of the sedentary conformation by leisure hour for girls was 0.67, and that of very active was 0.63. The sensitivity of low active and active by 7 different types of activity was quite low, 0.04~0.37. The exact agreement of activity level conformed by easy way developed was 30.8% and 33.7%, for boys and girls, respectively. More accurate way to identify 4 categories activity level needs to be developed, especially sensitive to conformation of low active and active levels.

8.The Analysis of Activity Energy, Total Energy, and Estimated Energy Expenditures in 5th and 6th Grade Primary School Students.

This study was conducted to investigate the time spent, activity coefficient and energy expenditure on 7 different types of activity. Total of 386 5th and 6th grade primary school boys and girls in Seoul participated in this study. The participants spent 8 hrs for rest, 4 hrs for out of school study, 3 hrs for leisure, 1 hr 45 mins. for hygiene, 1 hr for commute, and 15 mins. for house choir activity in the day of 6 class hours. The average activity coefficient of 1 day for boys and girls were 1.69 and 1.64, respectively. The TEEs were 2,382 kcal and 2,050 kcal for boys and girls, respectively. The energy cost for study related activity (in school and study activities) was 40% of the TEE, and for rest 20%, for commute 9%, and for hygiene 10% of the TEE. The energy cost for house choir was only about 2% of TEE. The 91.2% participant's EER was higher than the EER shown in the table of 2010 DRI for Koreans. When the participant's EER was compared with the energy allowance calculated by the method in 2000 RDA for Korean, 81.6% was in the range of +/- 5%, and the correlation coefficients between the 2 values were 0.981 for boys and 0.978 for girls, which means high agreements.

9.Concept Analysis of Tacit Nursing Knowledge.

PURPOSE: The purpose of this qualitative study is to investigate the nature of tacit nursing knowledge practiced in the clinical field in Korea using hybrid models, and to clarify the definitions and attributes of the concept. METHODS: The definition and nature of tacit nursing knowledge, obtained through a review of the extensive literature at the theoretical stage of the research, and the analysis of the in-depth interview data conducted by the career nurses in the fieldwork stage, are compared and analyzed. RESULTS: The tacit nursing knowledge was found in three dimensions as knowledge related to the person, the clinical situation work context, the self, the others, and the task. The tacit nursing knowledge was defined as personal nursing knowledge and artistic skills that show up as unconscious behavior patterns, learned informally, and internalizedthrough repeated clinical practice experience based on professional nursing knowledge. CONCLUSION: Tacit nursing knowledge has been widely used in clinical practice and has been shown to have a great impact, directly or indirectly, on clinical nursing. Therefore, individual and organizational efforts are needed for validation and clarification using the generation, sharing, collection, and peer review of sound implicit nursing knowledge to ensure that it is properly applied.

10.Predicting Factors of Developmental Delay in Infant and Early Children.

PURPOSE: The purpose of this study was to investigate factors associated with suspicious developmental delay in infants and early childhood. METHODS: Participants were 133 infants, aged from birth to 6 years old and their mothers, who were being seen at 16 Public health centers in B city. Korean Denver II was used to test infant development. chi2-test, Fisher's exact test and multiple logistic regression were used with SPSS 19.0 to analyze data. RESULTS: Of participant infants, 7.5% were below the 3rd percentile for the weight percentile, 8.4% is a weight curve that crosses more than 2 percentile lines on the growth charts after previous achievement, and 9.8% had suspicious developmental delay according to Korean Denver II. Further the predictive factors related to suspicious development delay in the children were decrease of weight percentile (Odds Ratio [OR]=6.69, Confidence Interval [CI])=1.22-36.45), low economic state (OR=6.26, CI=1.50-26.00), and developmental delay perceived by their mothers (OR=4.99, CI=1.24-20.06). CONCLUSION: It is necessary to build a government level system to follow management of development of infants and children from the time of birth. Especially, it is necessary to develop a program for children in low income families.
