M llerian anomalies have been infrequently encountered clinical problem for the obstetrician gynecologist, incidence was estimated to occur in 1% to 6% of all women. Recently, the use of vaginal ultrasound, MRI and endoscopic techniques have led to more accurate description and classification of m llerian anomalies. This report of a complete uterine septum with duplication of the cervix and a longitudinal vaginal septum with hemivaginal stricture represents a unique case of septate uterus, which to our knowledge previously has not been reported.

2.Relationship of Shift Work to Gastrointestinal Disorders in Hospital Nurses.

BACKGROUND: This study researched the relationship between shift work and one of its relevant health problems, functional gastrointestinal disorders. The study targeted nurses working at a university hospital, and compared the differences in gastrointestinal disorders of shift workers and non-shift workers and identified causes that significantly influence these symptoms. METHODS: In August 2013, a self-report survey was conducted on nurses working at a university hospital. Among those sent out, 1,201 responded and 851 were used for the final analysis. RESULTS: It was shown that the frequency of gastrointestinal disorders in shift workers was 1.881 times higher than that of non-shift workers. Factors that influenced gastrointestinal disorders included age, marital status, work duration, type of employment, job satisfaction, and perceived health status. Gastrointestinal disorders were seen more often with older age, being married, longer duration of employment, non-regular worker, low job satisfaction, bad perceived health status, and shift works. CONCLUSIONS: To prevent gastrointestinal disorders in nurses who work shifts, a health checkup system should be in place for clinical nurses who are of older age and have longer employment records. To decrease the causes of gastrointestinal disorders, campaigns suggesting regular diet and quitting alcohol use should be held consistently. Also, a category related to gastrointestinal disorders should be added to the employee health checkup for nurses with long work experience.

Tumor necrosis factor-n (TNF - alpha) involved in the pathogenesis of multiple sclerosis and contribute to the degeneration of oligodendrocytes as well as neurons. TNF - alpha is produced by miocroglia and astrocytes, which also produce hormones and cytokines that influence its biological activity. Astrocytes, the major glial cells in the CNS, are capable of producing TNF - alpha at both the mRNA and protein levels in response to interleukine-1 (IL-1) or TNF - alpha. Two immunosuppressive cytokines, transforming growth factor - beta (TGF - beta) and IL-10, have been shown to influence glial cell function. TGF - beta can modulate the activity of glial cells by inhibiting interferon-gamma (IFN - gamma) induced expression of class II major histocompatibility complex (MHC) molecules on astrocytes and microglia. To explore the role of astrocytes in the production of TNF - alpha, astrocytes were pretreated with IL-10 or TGF - beta and then stimulated with IL-1p to determine their effects on TNF - alpha production. The secretion of TNF - alpha by human fetal astrocytes was markedly inhibited by TGF - beta at a low concentration. In contrast IL-10 had no effect on TNF - alpha mRNA level. These results show that TGF - beta may regulate the expression of TNF - alpha in activated human fetal astrocytes.

5.Validation of a Modified Early Warning Score to Predict ICU Transfer for Patients with Severe Sepsis or Septic Shock on General Wards.

PURPOSE: To assess whether the Modified Early Warning Score (MEWS) predicts the need for intensive care unit (ICU) transfer for patients with severe sepsis or septic shock admitted to general wards. METHODS: A retrospective chart review of 100 general ward patients with severe sepsis or septic shock was implemented. Clinical information and MEWS according to point of time between ICU group and general ward group were reviewed. Data were analyzed using multivariate logistic regression and the area under the receiver operating characteristic curves with SPSS/WIN 18.0 program. RESULTS: Thirty-eight ICU patients and sixty-two general ward patients were included. In multivariate logistic regression, MEWS (odds ratio [OR] 2.02, 95% confidence interval [CI] 1.43-2.85), lactic acid (OR 1.83, 95% CI 1.22-2.73) and diastolic blood pressure (OR 0.89, 95% CI 0.80-1.00) were predictive of ICU transfer. The sensitivity and the specificity of MEWS used with cut-off value of six were 89.5% and 67.7% for ICU transfer. CONCLUSION: MEWS is an effective predictor of ICU transfer. A clinical algorithm could be created to respond to high MEWS and intervene with appropriate changes in clinical management.

Deep palmoplantar warts (Myrmecia) are frequently tender papules or plaques caused by HPV-1. Myrmecia are usually misdiagnosed as paronychia or digital mucous cysts. They occur not only on the palms and soles but also on the lateral aspects and tips of the fingers and toes. We report a rare case of deep palmoplantar wart occurring on proximal nail fold, unusual site of right 5th finger in an 11-year-old girl.

7.The Effect of Socioeconomic Status to Depression of Perimenopause Women: Pathway Analysis Using the Reserve Capacity Model

Purpose:
Based on the Reserve Capacity Model, this study investigated the effects of pre- and postmenopausal women’s socioeconomic status (SES) on depression, focusing on the mediating effects of self-esteem, happiness, and family relationship satisfaction with social network relationships.
Methods:
This cross-sectional study involved secondary analysis of national data on 771 perimenopause women gathered from the 16th Korea Welfare Panel Study (KOWEPS) 2021. A path analysis model was constructed to evaluate the relationship between SES, social network satisfaction, self-esteem, perceived health status, and depression. Data were analyzed using ADANCO 2.3.1 and Mplus 8.4.
Results:
Although SES had no direct effect on depression, it did affect depression through self-esteem, happiness, and satisfaction with family relationships.
Conclusion
The findings of this study indicate that perimenopausal women’s personal resources—psychosocial variables such as self-esteem and happiness—had a higher effect on depression than tangible reserves like SES. Therefore, interventions for enhancing self-esteem and happiness may prevent depression in perimenopausal women effectively.
8.Factors Influencing Fall Experiences among the Older Adults in Community: Using the 2021 Community Health Survey

Purpose:
This study aims to identify the factors that influence the experience of falls among older adults living in the community.
Methods:
The study participants were 70,887 65-year-olds who participated in the 2021 Community Health Survey. The study employed the Rao-scott x 2 test to examine the variation in fall experiences based on the characteristics of the older adults. Multiple logistic regression analysis was conducted to investigate these characteristics’ impact on older adults’ fall experiences.
Results:
The proportion of subjects in fall experience was 16.6%. The factors influencing the subject's fall experience were sex (odds ratio [OR]=1.47, 95% confidence interval [CI]=1.37~1.57), age (OR=1.48, 95% CI=1.34~1.65), family structure (OR=1.23, 95% CI=1.15~1.31), body mass index (OR=1.13, 95% CI=1.06~1.20), diabetes (OR=1.12, 95% CI=1.06~1.20), depression experiences (OR=1.56, 95% CI=1.42~1.70), stress (OR=1.12, 95% CI=1.05~1.19), subjective health status (OR=1.77, 95% CI=1.63~1.92), life satisfaction (OR=1.57, 95% CI=1.41~1.76), and chewing discomfort (OR=1.29, 95% CI=1.21~1.38).
Conclusion
Efforts should be made to effectively educate and develop various programs aimed at reducing falls among older adults. It is essential to emphasize the importance of continuous and active attention to falls in the older adult population.
9.Effects of Postpartum Depression and Temperament of Infant on Child-care Stress among Mothers of Newborn Infants.

PURPOSE: This study was designed to identify mother and infant related factors that influence child-care stress among the mother of newborn infants. METHODS: Data were collected through self-administered questionnaires and descriptive statistics, correlation and multiple regression were used to analyze the data. The data survey was conducted with 957 conveniently selected mothers of infants when they visited a public health center in Seoul to have their children immunized. RESULTS: The average item score for the Childcare Stress Inventory was 38.03, for the Edinburgh Postnatal Depression Scale, 9.31 and for the Degree of Bother Inventory, 23.42. The CSI was positively correlated to EPDS (r=.44, p<.001) and DBI (r=.40, p<.001). Also these two variables explained 30.0% of CSI in infants' mothers. CONCLUSION: These findings are expected to expand the understanding about postpartum mothers' child-care stress and can contribute to the development of comprehensive interventions based on community health nursing.

10.CCL5 Inhibits Elevation of Blood Pressure and Expression of Hypertensive Mediators in Developing Hypertension State Spontaneously Hypertensive Rats.

CCL5, a proinflammatory chemokine, has been shown to attenuate angiotensin (Ang) II-induced expression of hypertensive mediators as well as Ang II-induced inhibition of anti-hypertensive mediator expression in vascular smooth muscle cells (VSMCs) of spontaneously hypertensive rats (SHR). In the present study, functional roles of CCL5 on hypertension were examined in developing hypertension state SHR (DHSHR). DHSHR at an age of 8 weeks were injected CCL5 (1.5 microg/kg) subcutaneously twice a day for 3 weeks (SHRi, n=5). Control groups consisted of normal age-matched saline-treated SHR (SHRc, n=5) and normotensive Wistar-Kyoto rats (WKY, n=5). Effect of CCL5 on blood pressure was measured before treatment, weekly during treatment, and 1 day after the final injection. After injecting for 3 weeks, effects of CCL5 on expression of hypertensive mediators were examined in thoracic aorta tissues and VSMCs. Blood pressure in SHRi was maintained without any elevation during the treatment period, whereas blood pressure in SHRc progressively increased with age. Expression of Ang II subtype I receptor was reduced in SHRi thoracic aorta tissues and VSMCs compared to those in SHRc. In addition, expression levels of hypertensive mediators were significantly reduced in SHRi thoracic aorta tissues and VSMCs compared to those in SHRc. In contrast, AMP-activated protein kinase (AMPK) activity and interleukin-10 (IL-10) expression were elevated in SHRi thoracic aorta tissues and VSMCs compared to levels in SHRc. These results suggest that reduction of hypertensive mediators and elevation of anti-hypertensive mediators by CCL5 treatment promotes maintenance of blood pressure in DHSHR.
