1.Quantitative Changes of Immunoglobun Levels in the Serum of Allergic Dermatoses Patients.

With development of immunochemical methods for anlysis of serum proteins, quantitative determination of serum immunoglobulin levels and purification of them have been made possible in recent years. There are numerous reports about immunoglobulin levels in various diseases. A search of the literature reveals very few reports of quantitative immunoglobulin determinations in allergic dermatoses. The present study was conducted to find quantitative changes of serum immunoglobulins in a few allergic dermatoses. To quantify the IgG, IgA and IgM, one hundred and two sera were analysed form 30 normal control group, 30 contact dermatitis patients, 12 stopic dermatitis patients and 30 urticaria patients by modified Oudin's capillary tube method. The results are as follows. 1.In contact dermatitis the IgG was significantly increased to compare with control group(p<0.01) 2.In atopic dermatitis the IgA was significantly increased to compare with control group(p<0.01) 3.In urticaria the IgG was increased (p<0.05) 4.The IgM has no differences between allergic dermatoses and control group.

3.A Case Report of an Acardic Acephalic Twin.

Acardic twining is a rare congenital anomaly, usually in monozygotic twins. This condition is characterized by the absence or rudimentary development of fetal hear, and associated with various anomaly(especially with anecephalus). The acardiac malformations are uniformly fatal in the affected twin, and mortality in the co-twin is as high as 55%. We recently experienced a case of acardius acephalus associated with a normal term male infant, so present with a brief review of the literature.

5.The Effectiveness for Vagal Stimulation by Ice Water Application to Face during Controlled Respiration and Mechanism of Individual Variability of Responsiveness to Vagal Stimulation (Power Spectral Analysis of Heart Rate Variability).

BACKGROUND: The effectiveness for vagal stimulation by ice water aplication to face during metronome-controlled respiration of 15 breaths/minute was examed. The importance of basal vagal tone and sympathovagal interaction in the individual variability of responsiveness to the vagal stimulation was investigated. METHOD: Fifty three 12~13 year old healthy volunteers were included. Vagal tone and sympathovagal interaction before and after application of ice water to the face were assessed by power spectral analysis of RR interval(heart rate) variability. RESULT: Basal heart rate, high frequency power, and low to high frequency power ratio were 81+/-13(58~110 beats/min), 791+/-1061(56~4161 msec2) and 1.08+/-1.22(0.04~4.85) during controlled respiration. After ice water application twenty three children developed 5 more nodal escape beats due to severe bradycardia. Minimum heart rate, high frequency power and low to high power ratio changed to 42+/-12(19~72/min), 1890+/-1882(221~7258msec2) and 0.64+/-0.43(0.12~1.46). The increased ratio of high frequency power, maximum heart rate decrement and its percent after stimulation were 5.44+/-5.62(0.63~24.26), 39+/-14(10~81/min) and 47+/-15(16~81%) respectively. The increased ratio of high frequency power was correlated with basal logarithmic high frequency power(r=-0.60, p=0.0004). Maximum heart rate decrement was correlated with basal logarithmic high frequency power(r=-0.41, p=0.0018) and low to high frequency power ratio(r=0.27, p=0.04). CONCLUSION: Application of ice water to the face during controlled respiration produces powerful vagal stimulation and bradycardia, however, there is a wide individual variability of responsiveness to it. The absolute basal vagal tone contribute to this individual variability.

6.Four Cases of Vibrio vulnificus Infection.

We recently noticed four cases of Vibrio(V.) vulnificus infection from July, 1985 to September, 1985. The V. vulnificus was isolated by culture from the necrotizirig skin bullae in three cases, blood culture in two cases, and cerebrospinal fluid(CSF) in one case. The clinical characteristics of V. vulnificus infection in our cases was as follows: 1)All patients were men with their age over forties and the outbreak of the disease was during the summer season. 2) It was suspected that all patients had the previous hepatic problems. 3) The skin lesions showing bullae in three patients and subcutaneous nodules in one patient were noticed. 4) Two patients were showed positive in blood culture and one of thern also showed positive in lesional skin, urine and CSF. Lesional skin culture showed positive in three patients. 5) The isolated v. ulnificus was sensitive to chloramphenicol, erythromycin, gentamycin, kanamycin and cefobid. 6) Two patients died due to sepsis within 48 hours after liospitalization and one patient died due to hepatic failure.

7.A Case of Angiosarcoma of the Scalp.

Angiosarcoma is a rare malignant vascular tumor of endothelial cell origin. Most lesions of cutaneous angiosarcoma occur over the face and scalp. We experienced a case of angiosarcoma of the scalp in a 63-year-old man. Multiple pea-sized erythematous papules and nodules developed on the forehead 6 months ago. Thereafter, the lesions have extended to make violaceous, compressible and hemorrhagic plaques. On histologic examination, there were irregular, often anastomosing vascular channels lined by the atypical endothelial cells in the dermis.

8.3 Cases of Intraocular Lens Dislocations and their Management.

Recently we have experienced 3 cases of IOL dislocations. There are 2 cases of PCL and a case of iridocapsular lens. In case 1 and 2, they received ECCE and PCL implantations and had managed well good visual acuity before the dislocation of the IOL had occurred. In case 1, he gained good vision by replacing with ACL but in case 2, he lost his vision due to secondary glaucoma with optic nerve atrophy which did not seem to be related directly with PCL dislocation. In case 3, the patient had received iridocapsular lens implantation 7 years ago and had three times of episodes of subluxation of his IOL. Last September he had another episode of subluxation of IOL loop inferiorly. The medical therapy of the reposition of IOL was failed and IOL was removed and replaced by ACL. Now he lost his vision due to bullous keratopathy as a result of repeated episodes of endothelial touch. The three cases of this report remind us that the patient can restore good vision if he or she visit to the ophthalmologist as immediately as possible when the blurred vision has developed as prodromal symptom of dislocation of IOL.
