1.Factors Influencing Patient Safety Nursing Activities of Nurses in Long-term Care Hospitals

PURPOSE: The purpose of this study was to identify factors influencing patient safety nursing activities of nurses working in long-term care hospitals. METHODS: The participants were 126 nurses working in 8 long-term care hospitals in B city. Data were collected from June 26 to July 10, 2017. SPSS/WIN 23.0 was used for analysis with t-test, ANOVA, Scheffé test, Pearson correlation coefficients, and hierarchical multiple regression. RESULTS: The factors influencing patient safety nursing activities in participants were general characteristics, career in present long-term care hospital, (β=−.23, p=.008), safety control (β=.29, p=.002) and intention to report (β=.19, p=.037); on the personal side, informal communication (β=−.31, p=.005) for the organizational side. These factors contributed 39% of the total variance in patient safety nursing activities. CONCLUSION: In the organizational dimension of long-term care hospital, formal communication channels should be strengthened to officially direct or report patient safety rather than using informal communication. In order to improve the sense of safety control, which is a personal side of long-term care hospital nurses, it is necessary to provide awareness and education about the continuous safety control and positively prepare the reporting atmosphere to increase long-term care hospital nurses' intention to report.

2.Factors Influencing Resilience in Long-term Care Hospital Nurses

PURPOSE: The purpose of this study was to investigate factors affecting the resilience of emotional intelligence, job stress coping, and organizational socialization of nurses working in long-term care hospitals.METHODS: The participants were 153 nurses working in 8 long-term care hospitals in B city. Data were collected from February. 1 to Feb. 15, 2019 SPSS/WIN 23.0 was used for analysis with t-test, ANOVA, Scheffé test, Pearson correlation coefficients, and stepwise regression.RESULTS: Factors influencing resilience in the participants were emotional intelligence (β=.38, p < .001), coping behavior-focusing on the positive (β=.29, p < .001), nurse motivation (β=.16, p=.006), organizational socialization (β=.17, p=.009), coping behavior-tension reduction (β=.14, p=.023). These factors contributed 54% of the total variance in resilience.CONCLUSION: It is necessary to study emotional intelligence and hospital nurses' coping with job stress while carrying out highly emotional activities on the job and to develop a program that can enhance the resilience of the nurses to improve psychological wellbeing and verify the effects.

3.Analysis of Journal of Dental Hygiene Science Research Trends Using Keyword Network Analysis.

This research team extracted keywords from 953 papers published in the Journal of Dental Hygiene Science from 2001 to 2018 for keyword and centrality analyses using the Keyword Network Analysis method. Data were analyzed using Excel 2016 and NetMiner Version 4.4.1. By conducting a deeper analysis between keywords by overall keyword and time frame, we arrived at the following conclusions. For the 17 years considered for this study, the most frequently used words in a dental science paper were “Health,”“Oral,”“Hygiene,” and “Hygienist.” The words that form the center by connecting major words in the Journal of Dental Hygiene through the upper-degree centrality words were “Health,”“Dental,”“Oral,”“Hygiene,” and “Hygienist.” The upper betweenness centrality words were “Dental,”“Health,”“Oral,”“Hygiene,” and “Student.” Analysis results of the degree centrality words per period revealed “Health” (0.227), “Dental” (0.136), and “Hygiene” (0.136) for period 1; “Health” (0.242), “Dental” (0.177), and “Hygiene” (0.113) for period 2; “Health” (0.200), “Dental” (0.176), and “Oral” (0.082) for period 3; and “Dental” (0.235), “Health” (0.206), and “Oral” (0.147) for period 4. Analysis results of the betweenness centrality words per period revealed “Oral” (0.281) and “Health” (0.199) for period 1; “Dental” (0.205) and “Health” (0.169) for period 2, with the weight then dispersing to “Hygiene” (0.112), “Hygienist” (0.054), and “Oral” (0.053); “Health” (0.258) and “Dental” (0.246) for period 3; and “Oral” (0.364), “Health” (0.353), and “Dental” (0.333) for period 4. Based on the above results, we hope that further studies will be conducted in the future with diverse study subjects.

4.The Effects of Social Participation Attitude, Social Support, and Empowerment on Ageism Experience in the Elderly Using Welfare Centers

Purpose:
This study was conducted to investigate the factors affecting the discrimination of the elderly using welfare centers.
Methods:
A cross-sectional study was conducted with 217 elderly people from a welfare center in D city. Data were collected using a self-administered questionnaire survey consisting of ageism experience, social participation attitude, social support, and empowerment. For data analysis, descriptive statistics, t-test, one-way ANOVA, Pearson correlation coefficients, and multiple regression were performed using SPSS/WIN 21.0 program.
Results:
As a result of analyzing the correlations among the variables, the discrimination experience of the elderly showed negative correlations in social participation attitude and child relationship support. The factors affecting the discrimination experience of the elderly include gender, job, smartphone use, and child relationship support had a significant effect. The explanatory power of the discrimination experience of these factors was 15.4%.
Conclusion
This study found that gender, job, social support, and smartphone use were factors influencing the discrimination experience of the elderly. It is hoped that public relations and policy measures should be considered.
5.Deliberate Practice as an Effective Remediation Strategy for Underperforming Medical Students Focused on Clinical Skills: a Prospective Longitudinal Study

BACKGROUND: It is critical to develop remedial education for underperforming medical students, but little is known about how to create an effective remediation program. Deliberate practice (DP) is a structured and reflective activity that is designed to optimize performance. Here we applied the concept of DP to create remedial education to improve the clinical practices of medical students. We also analyzed the effectiveness of the remediation program. METHODS: Based on the expert performance approach of DP, we designed a 4-week remedial program for clinical performance that included feedback and reflection. There were 74 student participants in this program from 2014 to 2017. Their clinical performance was re-evaluated after completion, and changes in their clinical performance scores were analyzed. RESULTS: Students who completed the remediation program showed significant improvements in clinical performance scores (P < 0.001). Most students found the program to be instructive and helpful for improving their clinical performance. They reported that role play with peers was the most helpful for improving their skills. CONCLUSION: The DP-based remediation program improved the clinical performance of failing medical students. This remediation program should continue to be offered to underperforming students to ensure that medical school graduates are competent.

8.How to enhance students’ learning in a patient-centered longitudinal integrated clerkship: factors associated with students’ learning experiences

Purpose:
Longitudinal integrated clerkships (LICs) have been introduced in medical schools, as learning relationships with clinical faculty or peers are important components of medical education. The purpose of this study was to investigate the characteristics of student-faculty and student-student interactions in the LIC and to identify other factors related to whether students understood and acquired the program’s main outcomes.
Methods:
The study was conducted among the 149 third-year students who participated in the LIC in 2019. We divided the students into groups of eight. These groups were organized into corresponding discussion classes, during which students had discussions with clinical faculty members and peers and received feedback. Clinical faculty members and students were matched through an e-portfolio, where records were approved and feedback was given. A course evaluation questionnaire was completed and analysed.
Results:
A total of 144 valid questionnaires were returned. Logistic regression analysis showed that relevant feedback in discussion classes (adjusted odds ratio [AOR], 5.071; p<0.001), frequency of e-portfolio feedback (AOR, 1.813; p=0.012), and motivation by e-portfolio feedback (AOR, 1.790; p=0.026) predicted a greater likelihood of understanding the continuity of the patient’s medical experience. Relevant feedback from faculty members in discussion classes (AOR, 3.455; p<0.001) and frequency of e-portfolio feedback (AOR, 2.232; p<0.001) also predicted a greater likelihood of understanding the concept of patient-centered care.
Conclusion
Student-faculty interactions, including relevant feedback in discusstion classes, frequency of e-portfolio feedback, and motivation by e-portfolio feedback were found to be important factors in the LIC program.

Transient receptor potential canonical 4 (TRPC4) channel is a nonselective calcium-permeable cation channels. In intestinal smooth muscle cells, TRPC4 currents contribute more than 80% to muscarinic cationic current (mIcat). With its inward-rectifying current-voltage relationship and high calcium permeability, TRPC4 channels permit calcium influx once the channel is opened by muscarinic receptor stimulation. Polyamines are known to inhibit nonselective cation channels that mediate the generation of mIcat. Moreover, it is reported that TRPC4 channels are blocked by the intracellular spermine through electrostatic interaction with glutamate residues (E728, E729). Here, we investigated the correlation between the magnitude of channel inactivation by spermine and the magnitude of channel conductance. We also found additional spermine binding sites in TRPC4. We evaluated channel activity with electrophysiological recordings and revalidated structural significance based on Cryo-EM structure, which was resolved recently. We found that there is no correlation between magnitude of inhibitory action of spermine and magnitude of maximum current of the channel. In intracellular region, TRPC4 attracts spermine at channel periphery by reducing access resistance, and acidic residues contribute to blocking action of intracellular spermine; channel periphery, E649; cytosolic space, D629, D649, and E687.

10.Establishing a Patient-centered Longitudinal Integrated Clerkship:Early Results from a Single Institution

Background:
Longitudinal integrated clerkships (LICs) have been adopted by medical schools to overcome the limitations of traditional block clerkship rotations and to promote continuity of care. In 2018, Seoul National University College of Medicine introduced a patient-centered LIC program as part of a new curriculum in parallel with traditional block rotation clerkships. The purpose of this study was to present the patient-centered LIC program and to investigate its educational effects.
Methods:
In 2018 and 2019, a total of 298 third-year medical students participated in the LIC program. We divided the students into groups of eight, which were organized into corresponding discussion classes. Throughout the academic year, students followed up patients by interviewing them at the hospital or reviewing their electric medical records.Discussion classes on set topics were held seven times per year with facilitators and clinical faculties. Students completed a course evaluation questionnaire at the end of the academic year. The questionnaire included 22 items measured on a 5-point scale and two open-ended questions asking about the benefits and limitations of the program. The items covered three domains: student experience, satisfaction, and self-assessment. Final reflective essays were collected as both student assessments and data for qualitative analysis.
Results:
During the study period, the overall experience of the students improved. We increased the number of faculty members and patients and decreased the number of students in each discussion class. We also provided additional feedback through an e-portfolio.Students' satisfaction changed positively. Compared to the rotational clerkship, students answered that the LIC provided additional help in learning the two core competencies.During the first 2 years of the program, the percentage of students who answered that the program was more helpful than the rotational clerkship increased from 23.7% to 46.4% for continuity of care (P < 0.001), and from 20.5% to 50.7% for patient-centered care (P < 0.001).
Conclusion
Our patient-centered LIC, in parallel with traditional block rotation clerkships, had a positive effect on students' experience of continuity of care and patient-centered care.
