1.The Effect of Topical Interferon alpha-2b in the Treatment of Conjunctival Squamous Papilloma.

PURPOSE: To study the effectiveness of Interferon alfa-2b (IFN alpha 2b) for treating squamous papilloma of the conjunctiva. METHODS: One million units/cc of IFN alpha 2b was applied 4 times a day after identifying squamous papilloma by excisional biopsy. The IFN alpha 2b was used continuously 2 to 6 months after the tumor became undetectable with the unaided eye. The dose was then tapered. RESULTS: Three of the 4 patients had a history of recurrences (1-4 times). In one patient, the tumor was difficult to remove surgically due to the location of the tumor (around the upper punctum). The tumor disappeared with no complications or recurrences during the follow-up period. CONCLUSIONS: In patients with conjunctival squamous papilloma with frequent relapses or difficulty in excision, applying IFN alpha 2b as an eye drop appears to be an effective form of therapy and can be used safely without complications.

2.A Case of Single Fetal Death in Twin Pregnancy.

The antepartum death of one fetus in twin pregnancy is a rare obstetric complcation. If one fetus in twin pregnancy died in uterus remote from term and another fetus did not, the dead fetus will be compressed between the uterine wall and the membrane of living fetus, becomes a fetus compressus or fetus papyraceous. The primary maternal threat when the fetuses undelivered is disseminated intravascular coagulation. Concern for the surviving fetus after death of its co-twin is balanced between the risks of preterm birth and those involving exposure to events in utero that may threaten its well-being. Recently we experienced a case of twin pregnancy complicated by the death of one fe- tus, we report this case with concerned literatures.

4.A Case of Chilblain Lupus Erythematosus.

Chilblain lupus erythematosus(CLE) is a chronic unremitting from of LE seen predomin antly in women, The chilblain lesion occurs commonly on the digits, calves and heels. Chronic facial discoid LE usually appears before the chilblain form. A 23-year-old female had relatively well defined, erythematous discoid plaques on her both cheeks and scattered erythema multiforme-like ring lesions on her right hand and right wrist. There were also chilblain lesions showing multiple, purple colored macules on her knees, lower legs and periungual areas of fingers and toes. The skin lesions developed at November, 1982 and then the skin lesions remitted during the next summer. The skin lesions recurred at December, 1983.

5.Changes in Eyelid Height with Time after Levator Resection under Local Anesthesia.

PURPOSE: To evaluate the factors that affect the eyelid height changes during the postoperative period in patients who underwent levator resection under local anesthesia. METHODS: Among the 242 patients that underwent levator resection under local anesthesia by the same surgeon between January on 1995 and December 2003, marginal reflex distance 1 (MRD1) measurements were performed using a caliper in 91 patients who were followed for more than 3 months. RESULTS: There were 36 males and 55 females, aged between 12 and 78 years (average of 33.6 years). The average follow-up period of the patients was 8.7 months (3 months ~ 58 months). During this period, 86 patients (94.5%) experienced satisfactory results. The average change in the MRD1 of the eyelids preoperatively, during the operation, and 1 week, 1 month, 3 months, 6 months, and 1 year postoperatively were 0.8 mm, 3.9 mm, 3.0 mm, 2.7 mm, 2.5 mm, 2.4 mm, and 2.2 mm, respectively. The MRD1 decreased 1.2 mm after 1 month and stabilized. When the levator function was greater than 8 mm, the height of the eyelids stabilized within 1 week. The worse the function of the levator palpebrae, such as in the case of congenital ptosis, the greater the correction was needed. CONCLUSIONS: Levator resection under local anesthesia is a preferable method in adjusting the height of the eyelids. In a patient with poor levator function, a greater amount of correction is needed to achieve a satisfactory eyelid height.

6.Clinical Observation on Displaced Proximal Humeral Fracture

Most proximal humeral fractures respond satisfactorily to conservative treatment. It is only the occasional displaced fracture or fracture-dislocation that demaads special treatment. The purpose of this study is to analysis the results of closed and open reduction of displaced proximal humeral fractures according to Neers classification. Fifty-two cases of these fractures, followed up more than five months, are presented. 1. The average age of patients was 40. I years. 2. The fractures were classified according to Neers method. Nearly half (48.1%) of the cases were one-part fractures. Next, two-part fractures rated 42.3%, while three-part fractures, only one (l.9%). 3. Among the total 52 patients, 42 cases(80.8%) were treated conservatively and 10 were operated. We performed surgical operations in seven cases among 22 two-part fractures, aix were reduced with Kirschner wires and one treated with Kirschner wire and staple. The average age of these seven cases was 26. 6 years and the resulta were good except one. 4. The results of these patients were evaluated by the Neers criteria. Of 52 caaes, 40 had good results, five, fair, and the remaining seven poor results. Twenty-three cases (92%) in 25(100%) one-part fractures were good and 17 cases (77.3%) in 22 (100%) two-part fractures were also good. In three-part and four-part fractures the results were all poor.

7.A Case of Ectopic Orbital Meningioma.

A 37 year-old male was suffered from proptosis without visual loss for 5 months. The orbit CT scan showed an eyeball-sized, extraconal, intraorbital soft tissue mass. The mass was removed by modified Kronlein operation and was diagnosed as the transitional type of meningioma by light microscopic and electron microscopic findings. After, surgery, there is no recurrence after 6 months.

8.The Effects of Hospice Unit Practice Education on Spiritual Well-being, Attitude to Death, and Hospice Awareness of Nursing Students.

PURPOSE: The purpose of this study was to examine the effect of hospice unit practice education on spiritual well-being, attitude to death, and hospice awareness of nursing students. METHODS: This study used a one group pre-posttest research design. The participants were 29 nursing students attending D university located in Y City. All students participated in 2 weeks of hospice unit practice education. The hospice unit practice education program consisted of 90 hours. Data collection was performed from March 2 to April 30, 2015. 2 weeks. The data were analyzed using frequency, and paired t-tests, with the SPSS 21.0 program. RESULTS: The results of this study were as followings; the score of existential well-being, attitude to death, necessary of pain and symptom control were significantly increased after hospice unit practice education. CONCLUSION: Based on these results, hospice unit practice education is effective for improving spiritual well-being, attitude to death, and hospice awareness of nursing students. These results provide basic data to develop future nursing practice curriculum.

9.A Case of Bilateral Central Serous Chorioretinopathy in a Chronic Tibial Osteomyelitis Patient.

Central serous chorioretinopathy (CSC) is characterized by central serous retinal detachment. It occurs usually in young adult male, and bilateral involvement is uncommon. We have experienced bilateral CSC occurred in a 33 year old male patient with chronic tibial osteomyelitis. We treated only the right eye with krypton laser to compare the course of laser treated eye with untreated opposite eye. There was no difference between the course of two eyes in our case.

10.Clinical Aspect of Lens Extraction by Pragmatome(TM) and Ocutome(R): A preliminary report.

Phacofragmentation is one of the method of extracapsular lens extraction using ultrasonic frequency. We have used this method, sometimes inconjunction with anterior vitrectomy since April, 1980 for 10 eyes of 10 cataract or early adult cataract. Our machines for this purpose is one of the Girard type phacofragmetor, Berkeley Model 820 Fragmatome(TM), in conjunction with Berkeley Model 800A Ocutome(R). We have had quite good results after operation and we are sure this method was preferable operation at least in above mentioned cases. This is an our preliminary report and it will be necessary to perform operation to many cataract patients and to determine longterm effects of the operation.
