1.Conjunctival Bacterial Flora at Birth: Comparisons between Cesarean Section and Vaginal Delivery.

Conjunctival bacterial cultures for 113 neonates who were delivered by cesarean section or vaginal delivery were performed within 30 minutes after birth. These neonates were in the condition of no eye drops applied. The 43 newborns were delivered by cesarean section(CS group) and the 70 newborns were delivered vaginally(VD group). The 10 newborns(P1) (14 eyes(P2)) in the CS group and the 29 newborns(41 eyes) in the VD group showed culture positivity. These differences between two groups were statistically significant(p1, p2<0.05). The CS group had significantly fewer species and colony forming units cultured per subject (0.395 +/- 0.821(p3), 5.116 +/- 27.095(p4) than the VD group(0.971 +/- 1.393, 25.843 +/- 62.285) (p3,p4<0.05). Coagulase negative staphylococci were predominantly isolated in the CS group and coagulase negative staphylococci, gram positive bacilli, E. coli, nonfermenting bacilli, and S. aureus in the VD group. Therefore it was found that the conjunctiva of the newborns delivered by cesarean section bore significantly fewer bacteria and need less prophylactic measures than that of newborns delivered vaginally.

2.Labeling and Biodistribution of Re-188-DTPA ( Diethylenetriaminepentaacetic acid ).

Re-188 is useful candidate for therapeutic radionuclide because it has a physical half life of 17 hours, contains beta ernissions suitable for therapy(maximum energy 2.12MeV) and emits a garnma ray that is suitable for quantitative diagnostic scanning(155keV). To use He-188 as a radionuclide compound of angioplasty balloon radiotherapy, we investigated the labelling method and biodistribution of Re-188- DTPA. We postulated that labeled Re-188-DTPA is preferable because it would be excreted via urinary system more easily than other compounds. To label Re-188 with DTI'A, 1ml of 222MBqI(6mCi) of Re-188 was added to DTPA solution(DTPA 20mg, SnC4 2HsO 10mg, pH 3.5) and boiled at 100C for 120min in water bath. pH was adjuted to 5 with 2.3Fo sodium acetate. I.abeling efficiency was measured using TLC-SG(acetone, saline). We evaluated biodistribution of Re-188-DTPA in sacrificed mice at 10 and 60 minutes after injection. We acquired images of kidneys, and drew tirne-activity r.urves in normal dogs and rats and calculated Trnax and Tl/2 in rats. The labelling efficiency was 95.7Yo on average. Labelling of Re-188-DTPA was stable(90% after 5hours) in vitro at room temperature. According to time-activity curves of dogs and rats, it took 15 to 20 minutes after injection for Re-188-DTPA to be washed out through kidneys. In conclusion, Re-188-DTPA was successfully labeled, Re-188-DTPA was stable in vitro and was excreted early via kidneys in animals. We could recornmend Re-188-DTPA as radionuclide of potential use in angioplasty balloon radiotherapy.

Catastrophic antiphospholipid syndrome is a highly fatal condition characterized by widespread thromboembolism subsequent to a triggering factor (e.g., infection, trauma, and neoplasia) in antiphospholipid antibody-positive patients. This paper reports a case of a 29-year-old male without the underlying disease who developed extensive mesenteric thromboembolism and jejunal necrosis during the treatment for acute enteritis. The patient’s condition was improved with low-molecular-weight heparin and an intravenous Ig treatment with emergency surgery. The serum antiphospholipid (anticardiolipin IgM) and lupus anticoagulant antibody tests showed positive results. Acute infectious enterocolitis is generally considered a mild disease. On the other hand, aggressive evaluation and treatment should be considered if the clinical conditions do not improve and deteriorate rapidly despite appropriate antibiotic treatment because of the possibility of acute immunological complications, such as catastrophic antiphospholipid syndrome.

6.The Results of Modified Brostrom Procedure for the Treatment of Chronic Lateral Ankle Instability.

PURPOSE: This study was performed to evaluate clinical and radiographical results of treatment of chronic lateral ankle instability with modified Brostrom procedure. MATERIALS AND METHODS: This is a retrospective study of 22 cases in 22 patients treated with modified Brostrom procedure under the diagnosis of chronic lateral ankle instability from May 2000 to August 2004. Average age was 32.3 years. Average follow-up period was 23 months. Preoperative and postoperative radiographs of ankle anteroposterior view, lateral view and varus stress view were analyzed. The clinical evaluation was performed according to the Americal Orthopaedic Foot and Ankle Society scales. RESULTS: After the modified Brostrom procedure, Americal Orthopaedic Foot and Ankle Society scales score was improved by average 28 points. Among the 22 cases, 13 cases were excellent, 3 cases good, 4 cases fair, and 2 cases poor with satisfaction of 73%. Five cases had intermittent ankle pain of whom 3 cases had difficulty during competitive sports activity. Lateral tilting of talus improved by average 5.4 degrees on varus stress view. CONCLUSIONS: The modified Brostrom procedure is one of the most effective methods for treating of chronic lateral ankle instability.

7.Influential Variables on Intention and Action to Quit Smoking between Adolescent Smokers and Adult Smokers-Based on the Transtheoretical Model.

PURPOSE: This study identified and compared influential variables on intention and action to quit smoking between adolescent smokers and adult smokers. METHODS: For the selection of variables, the transtheoretical theory was used. A survey was conducted with 376 adolescent smokers in 4 high schools and 451 adult smokers in community settings in South Korea. Discriminant analysis was used for data analysis RESULTS: The variables of adolescent smokers that predicted an intention to quit smoking were: smoking temptation, self re-evaluation, counter conditioning and stimulus control. The variables that predicted an action to quit smoking were: self-efficacy for smoking abstinence, pros for smoking, self reevaluation, and self liberation. The variables of adult smokers that predicted an intention to quit were: smoking temptation, pros for smoking, cons for smoking, self reevaluation. The variables that self liberation and predicted an action to quit smoking were: self efficacy for smoking abstinence, smoking temptation, and counter conditioning. CONCLUSIONS: Developing stage specific smoking intervention methods based on different ways of how individuals make a decision to quit smoking within their contexts needs to be done.

8.Changes in Smokers' Perception about smoking before and after Participation in a Hospitalized Smoking Cessation Program.

PURPOSE: This study was done to analyze the changes in smokers' subjectivity about smoking before and after participation in a 5-day hospitalized smoking cessation program. Design: Q-methodology, which provides a scientific method on subjectivity, was applied. Findings: Nine types of subjectivity changes about smoking were identified; Type I, Nicotine craving --> Confident of self control for smoking; Type II, Nicotine craving --> Cognition of the link between habituation and smoking, Type III, Nicotine craving --> Nicotine craving, Type IV, smoking as a social behavior --> Cognition of the link between habituation and smoking, Type V, smoking as a social behavior --> Motive to turn away from being addicted to smoking to being healthy, Type VI, Habitual smoking --> Confident of self control for smoking, Type VII, Habitual smoking --> Cognition of the link between habituation and smoking, Type VIII, Habitual smoking --> Motive to turn away from being addicted to smoking to being healthy and Type IX, Denial of the negative effects of smoking --> Confident of self control for smoking CONCLUSION: The types of changes in smokers' subjectivity before and after the smoking cessation program in this study suggest a better tailored intervention for individual smokers. The types of changes on smokers' subjectivity provide insight about which factors of the interventions for smoking cessation are especially valued for a particular group of smokers.

BACKGROUND AND OBJECTIVES: Although CO2 laser turbinate surgery is known to be an effective method in reducing the nasal symptoms of perennial allergic rhinitis, the long term effects have not yet been evaluated. Therefore, the purpose of this study was to assess the subjective and objective long term results of CO2 laser surgery. MATERIALS AND METHODS: The charts and questionnaires of 49 patients who received laser surgery at our department from January 1994 to December 1999 were reviewed. Preoperative and postoperative nasal volume of 20 randomly selected patients were measured by acoustic rhinometry. Nasal mucosa of 10 patients before and after laser surgery yearly for 6 years were examined under light and electron microscopy. RESULTS: Watery nasal discharge, nasal obstruction and sneezing had improved significantly after laser surgery. Postoperative nasal volumes also increased significantly. Histological findings 6 years after laser surgery revealed focal squamous epithelium and regenerating ciliated columnar epithelium. Decreased goblet cells, fibrous proliferation and scarce vessels and glands were also observed. CONCLUSIONS: CO2 laser surgery proves to be an effective method in reducing the symptoms of perennial allergic rhinitis for short and long periods.
