The subjects in this study were the directors in charge of article inspection from 127 academies that are registered with the Korea Academy of Medical Science. After a questionnaire was prepared covering the examination process, judging committee organization, problems with examination and possible reform measures, a survey was conducted by e-mail and facsimile from October 4 through 20, 2001. The responses from 79 academies were gathered, a response rate of 62.2%, and the collected data were analyzed with SAS. The most common examination process was C type, which convenes publishing committees, employed by 59 academies_(74.6%), and these institutes found it satisfactory. Thirty-four academies_(43.0%) had 10 to 19 judges, and these judges were separately in charge of different fields, whereas 79 academies_(77.3%) allocated five or less judges to each field. Thirty_(38.0%) academies had their judges comply with related official directions or guidelines, and 50 academies_(63.3%) conducted investigation just twice. Fifty-four academies_(68.4%) weren't satisfied with their examination process. As for the problems with the examination process, many_(41.8%) pointed out that it's time-consuming work and that a lot of procedures were repeated. Concerning the benefit of the online examination program, 41 academies_(51.9%) thought that it was more efficient and less time-consuming. Forty-one academies_(51.9%) were aware of the online examination program, but only 23 academies_(29.1%) were willing to use it. In order to realize an online paper review, there should be appropriate criteria and guidelines for the construction of an online paper review system that suits various types of paper review process employed by the academies. The usability of online information and technology for more efficient and higher-quality judgment should be widely be publicized, and consideration should be given for the types of paper review system that might be suitable for those who are neither aware of the use of the internet nor willing to utilize.

Journal of Korean Society of Medical Informatics 2003;9(2):153-162
As internet technology rapidly improves, clinical services are increasingly computerized in most hosptials. In order to effectively support patient care and clinical researches, information technologies are widely utilized to integrate clinical databases and disseminate information. However, most hospital information systems in Korea are still independently implemented and not fully integrated due to lack of standardization and security measure. In order to meet increasing demand for better patient care and upgrading clinical research, there is a need for a development and sharing of integrated clinical information system with a common database including clinical data collected from various hospitals. The purpose of this study was to develop a web-based clinical information system for management of Behcet's disease by constructing an integrated database with treatment data, test results, and patient demographic data in collaboration with several hospitals. Specifically, current treatment procedure for Behcet's disease was analyzed first and enhanced treatment procedure as well as a web-based clinical information system for management of Behcet's disease was proposed using a structured systems analysis and literature review on the shared clinical information systems. Expected benefits of the system include an improvement of consistency in patient management and reduction of duplicate prescriptions and test orders. In addition, this system can help improve communication among clinics and tertiary hospitals by sharing common clinical database. In a long run, the share system can also help reducing hospital expenditures by reducing duplicate investment on high cost equipments by sharing them among hospitals.

Journal of Korean Society of Medical Informatics 2003;9(2):143-152
This study investigated the utilization of clinical information on cancer to establish a basic paln for construction of systematic and efficient Cancer Clinical Information System (CCIS) in Korea. A mail survey was conducted among 141 clinical oncologists from 10 hospitals. Their utilization of the information system, capabilities in managing the information, and opinions on establishing a nationalized CCIS were surveyed and analyzed. By descriptive statistical methods, a Chi-square test, a Kruskal-Wallis test, a t-test and ANOVA. The most popular way to obtain the clinical information on cancer was through paper charts. All the respondents stated that the cancer clinical information procured from the current information system was insufficient due to the lack of standardized clinical information, the absence of coded data and applicable information, and the inefficiency of the information system. Over 90% of respondents indicated the need to develop a well organized and specifically planned CCIS. Information on diagnosis, surgery and pathology were three most useful and accessible item in clinical research on cancer. Information on side effects of cancer treatment and survival rates of cancer patient were necessary, but not available in the current CCIS. In conclusion, the existing information system has not been used effectively in extracting the requested information on clinical oncology. In order to develop a database, a higher priority should be placed on standardizing and continuously updating the contents, diversifying the search subjects and improving the security of the database system. The government needs to get involved in planning this CCIS to connect all the health care facilities across the country. Moreover, there must be some administrative support for the construction of a CCIS in order to achieve some technical advancement in hardware and software development and to regulate the clinical information.

Journal of Korean Society of Medical Informatics 2003;9(2):131-141
We aimed to construct web sites providing information on epilepsy and to evaluate its practical use. To estimate the demand for such information, a questionnaire was administrated to 106 patients, their 54 families, and 78 nurses, who consented to be interviewed. After using the data collected from the survey to build a epilepsy information web site, data on the level of satisfaction for the website were collected by e-mails and direct surveys from subjects who used this web site; the collected data were analyzed. The results of this study were as follows: The educational level demanded by nurses was slightly higher than those of patients and their families. Patients, their families, and nurses wanted more information on how to handle patients during seizures, especially, and. families wanted to know about drug therapy. The content of the web site was configured based on the educational level of perceived users. Photoshop 6.0, Flash MX and FrontPage 2000 were used to build the web site. Images available on the internet that are shared served as the source of images. The program's URL is www.multimoon.com/black. Regarding user satisfaction with the web site, users found that the Information on the Prognosis of Epilepsy, the Pharmacological Treatment of Epilepsy, Occupation and Marriage Status in Daily Life, and the How to Manage Epileptic Attacks sections were quite helpful. Generally, the reactions to the website were positive. To construct an effective web site to provide information about epilepsy to patients, their families and nurses, educational data and information that meet the demands of users should be added. Its contents should be continuously updated, and its environment should be kept user-friendly so that the information can be accessed with ease.

Journal of Korean Society of Medical Informatics 2003;9(2):111-120
Asthma is a chronic disease, which requires a successive and long-term management. In this paper, we implemented a web-based caring system for a more effective management of the asthmatics. The service of this asthmatic-caring system targeted on both the doctor and the patient. The patient connects to the asthma management server through the Internet everyday and inputs his or her daily condition and dosage. Then the doctor will give a proper comment or order to the patient according to the data that patient has filled in. By merely connecting to this web-site, doctors can manage asthmatics everyday and the patient can be under the doctor's management without frequently visiting the hospital. The test verifying the user's satisfaction of the implemented system has been taken by both the doctor- and the patient-groups. The doctor-group pointed out the successive management, an easy inspection of the patient condition, and the accumulation of clinical data for future researches as advantages. The patientgroup pointed out being under the daily management without visiting hospital and the active attitude they can have toward their diseases as advantages.

Journal of Korean Society of Medical Informatics 2003;9(2):101-110
The aim of this study was to develop and evaluate a web-based learning program with multimedia ECG monitoring for clinical nurses. For this, we compared the self-directed, web-based learning method with the traditional lecture method. A quasiexperimental, nonequivalent control group pretest-posttest design was used, the data from 32 nurses(17 for experimental group and 15 for control group) were collected from October 1 to November 30, 2001, and analysed by SPSS program with x2 test, ttest, ANCOVA. The results were as follows : 1) There was significant improvement in the score on learning achievement of the experimental group compared with that of the control group(t=-2.739, p=.010). 2) There was no significant difference between the groups in learning motivation(t=-.054, p=.4785). In conclusion, it is suggested that a higher learning achievement was achieved in self-directed, web-based learning than in the traditional classroom style learning. There was also evidence that two educational styles had same effect in creating motivation. We propose to utilize a self-directed, web-based leaning method as a means to continue life-long education for nurses on a rotational duty system.
