Objective:To preparate BCUN-loaded lipid ultrasound contrast agent and detect its physical properties and contrast-enhancing effects.Methods:Mechanical oscillation was used to prepare BCUN-loaded lipid contrast agent.Density was detected and analyzed with microscope.Malvern determined intensity,disposition and Zeta potential.RP-HPLC detected envelopment rate.Cryodesiccated-microb ubbles were sterilized by 60Co irradiation.Following injected it through rabbit ear vein,effects of ultrasounic enhanced imaging in rabbit liver was observed.Results:The density of microbubble was(2.6~3.1)?109/ ml.The mean intensity was(1.05?0.20)?m.The mean Zeta potential was(12.93?1.84)mV.The mean nvelopment rate was(42.6?0.11)%.After injecting this microbubble,good and sustained ultrasounic enhanced images in rabbit liver were seen.Conclusions:BCUN-loaded lipid contrast agents were successfully prepared,which hopefullyprovide a new option oftargetingtreatment for the tumor.

Objective:This paper will retrospectively analyze the long-term results of PORT for Completely Resected Stage Ⅲa(N2) NSCLC.The strategies of treatment are also to be discussed.Methods:92 cases with Completely Resected Stage Ⅲa(N2)NSCLC treated in our hospital from 1987 to 2004 were analyzed.Among them,46 patients received PORT(Group S+R),46 no PORT(Group S).39 of the entire patients。received chemotherapy.Median radiation dose was 56 Gy(40～64 Gy).The survival rates were analyzed and compared by Kaplan-Meier and Log-rank.Results:(1)The 5 and 10-year overall survival(OS) for all patients was 44.5% and 30.4%,respectively(.2)The 5 and 10-year OS of group S+R and S was 49.1% vs 36.5% and 36.3% vs 25.4%,respectively,with no significant difference(?2=0.83,P=0.65)(.3)For the patients with single station involved N2,PORT tends to improve the disease-free survival(DFS) but no reach significant difference.As for patients with multiple stations involved N2,PORT could improve the DFS,the 5 and 10-year DFS of group S+R and S was 40.6% vs.4.5%;21.2% vs.4.1%,respectively,(?2=4.35,P=0.03),meanwhile,it might increase the OS(.4) The survival rate treated with PORT from 1996 to 2004 was higher than that from 1987 to 1995,(?2=4.28,P=0.04)(.5)Recurrence was seen in 63% of patients.The total(both local/regional recurrence and distant metastasis)and local/regional recurrenc rate of group S+R was lower than that of group S,i.e.50.0% vs76.1%(?2=6.72,P=0.001)and 8.7% vs 32.6%(?2=8.03,P=0.001),respectively.Distant metastasis was found to be the main failure cause for both group S+R and S.Multivariate analyses showed that age and number of mediastinal lymph node station involved have influence on the survival of patients with Completely Resected Stage Ⅲa(N2)NSCLC.the DFS.But its value for patients with single station involved N2 was unclear and a further randomized clinical trail is still warrant.(2)Age and the number of mediastinal lymph node station involved are the two significant prognosis factors for patients with Completely Resected Stage Ⅲa(N2)NSCLC(.3)Condition of equipment and radiation technology used could have some influence on the result of PORT(.4)The fact that distant metastasis was the main failure cause for both group S+R and S might imply that patients with Completely Resected Stage Ⅲa(N2)NSCLC might benefit from chemotherapy.It suggests thus that chemotherapy should be considered for these patients.

Objective:To investigate the effects of three different anesthetic techniques on pere-anesthesia circulation,stree response and post-anesthesia recovery in patients undergoing leparoscopic cholecystectomy(LC).Methods:Sixty-six ASA Ⅰ or Ⅱ patients aged 18 to 60 undergoing selected LC surgery were randomly divied into 3 groups(n=22).The patients in group I were induced and maintained with remifentanil and sevoflurane.The patients in group Ⅱ with fentanil,propofol and isoflurane,while the patients in group Ⅲ with remifentanil and propofol TCI.MAP and HR were measured before induction,before intubation,at 5 min after intubation,1 min and 15 min after the establishmen of CO2 pneumoperitoneum and at the end of operation.Blood cortisol(Cor)and blood glucose(Glu)were measured before induction,after induction 5 min and at the end of operation;Blood interleukin-6(IL-6)was measured before induction and at the end of operation.Postoperative recovery time,Alddret ≥9,the rate of PONV and VAS score were measured on the first postoperative day.Results:There were significant decreases of MAP in group Ⅰ and Ⅱ before intubation(P 0.05).Conclusion:The three anesthetic techniques can be used safely on LC operation,anesthesia with remifentanil and sevoflurane,anesthesia with remifentanil and propofol TCI have shorter post-operative recovery time and high quality compared with the anesthesia with fentanil,isoflurane and propofol.LC operation can cause obvious stress response of the body,significant postoperative pain and high rate of PONV,which should be actively dealt with.

Objective:To investigate the prevalence,antibiotic resistance and risk factors of Acinetobacter Baumannii hospital-acquired pneumonia(ABHAP)in Respiratory Intensive Care Unit(RICU)of our hospital for guidance of clinical prevention and treatment of ABHAP.Methods:Clinical information and results of Antibiotic susceptibility test for all of the patients with hospital-acquired pneumonia(HAP)in RICU of our hospital from May 2007 to October 2008 were collected.Antibiotic resistance and risk factors of ABHAP were analyzed retrospectively.Results:159 patients suffered from HAP.Of which,49 patients had ABHAP and accounted for 30.8% of all cases.Acinetobacter Baumannii was the second commonest pathogen of HAP.The resistance rate of Acinetobacter Baumannii strains to the third generation of Cephalosporin and Fluoroquinolone were over 90%.And the resistance rate to Imipenen was 42.7%.Pan-resistant rate was 23.5%.Univariate analysis showed that APACHEII score ≥20,hospita1ization in RICU ≥2 weeks,invasive mechanical ventilation,invasive mechanical ventilation ≥7 days,trachea intubation or tracheotomy,trachea intubation or tracheotomy≥7 days and broad-spectrum antibiotic therapy≥7 days were associated with BAHAP.Multivariate logistic analysis showed that hospita1ization in RICU≥2 weeks,invasive mechanical ventilation≥7 days,APACHEII score≥20 and broad-spectrum antibiotic therapy ≥7 days were independent risk factors of ABHAP.Conclusions:The present study found high incidence rate,severe drug-resistance and therapeutic difficulties of ABHAP in RICU.Shortening duration of hospitalization and invasive mechanical ventilation,rational use of antibiotics are helpful to prevent and treat ABHAP.
