1.Prevalence of Diseases of Circulatory System Among Rural Korean Adults.

This report is a part of the health survey, which was carried out in the summer of 1973 on all adults older than 15 years of age, inhabiting in the sampled area. Thirteen villages out of 31 natural villages in Shindong Myon, Chunseong Gun, Kangwon Province, a typical rural area of Korea were randomly sampled. A little over 70 percent of the population sampled, or 1198 persons were examined. The health survey included laboratory screening tests, history taking, physical examination and consultation to specialists for selected cases. Diseases of circulatory system were classified by WHO ICD A-List (150). Hypertensive heart disease was defined by the criteria that was used in U.S. Health Survey, and hypertension by WHO criteria for the purpose of comparison. Results obtained were as followings: 1. The overall prevalence of the diseases of circulatory system per 100 adults examined was 8.6 for male and 9.9 for female. 2. Age specific prevalence of the diseases showed gradually increasing pattern as age advanced for both sexes. For example, the prevalence for 15~19 years old age group was 2.4/100 adults which increased to 24.8 when age reached over 60 years. 3. The most prevalent disease category, 3.6/100 adults, was A-88: other diseases of circulatory system that was consisted mainly of varicose vein and hemorrhoids; the second one was hypetensive heart disease, 2.4. 4. Female had higher prevalence than male in A-84: other form of heart diseases (4 times, arrhythmia, beriberi heart disease, and heart disease secondary to anemia), in chronic rheumatic heart disease (4.5 times), and in hypertensive heart disease (1.4 times); in other hand, cerebrovascular diseases (4.5 times) and diseases of arterioles and capillaries (1.5 times) occurred more frequently among males than females. 5. The proportion of hypertensive heart disease among persons with definite hypertension was 22.0% for male, 25.8% for female, and it increased gradually as age increased. Hypertensive heart disease among persons with borderline hypertension was 2.2%. Only 27% of hypertensive heart disease patients had previous treatment for hypertension. 6. The prevalence of definite hypertension per 100 adults examined (double blood pressure check with interval of 30~40 minutes) was 9.5 for male, 9.4 for female, with increasing pattern by advancing ages; marked increase occurred after age of 45 years for male and 35 years for female. The prevalence of hypertension in this study was much lower than that of the study on employees of Korea Electricity Comparny when compared with the same age groups. 7. Electrocardiographic findings on 105 heart disease suspects revealed left ventricular hyperspecific change in 4 persons. This study was supported by China Medical Board of New York Inc.

3.A Comparative Analysis of Dynamic Strategy and Normal Strategy in Octopus 101 Perimeter.

Dynamic strategy(DS) is a fast threshold algorithm for the Octopus 101 perimeter. To analyze the efficiency of DS, we compared the results of visual field measurements using DS and conventional normal strategy(NS). Subjects comprised 60 eyes of 30 normal individuals in the 3rd decade who has not experienced perimetry before. Program G2 was performed on all subjects at random and enough intervals between the two tests(more than 1 hour). Mean testing time was 1047.63+/-219.38 seconds in NS and 684.98+/-130.95 seconds in DS. Testing time reduction rate was 32.69+/-14.35% on average. Mean defect(MD) was 2.80+/-1.02dB, 2.15+/-0.94dB in the two strategies, respectively. Corrected loss variance(CLV) was 1.31+/-0.50dB2 and 1.03+/-0.61dB2 in each test. There was significant difference in MD and CLV statistically(P<0.01). Mean short term fluctuation(SF) was 1.38+/-0.49dB and 1.45+/-0.50 dB; mean reliability factor(RF) was 2.05+/-2.73% and 1.98+/-2.80% in the two strategies, respectively. There was no significant difference in SF and RF statistically(P>0.01). It took less time with DS than NS in Octopus 101 perimeter, but further study will be needed for clinical application of DS.

7.Ecology of filariasis on Che Ju Island.

Study of filariasis to determine important factors involved in its ecology was carried out on Che Ju Island for three consecutive years from 1968 to 1970 in seven villages, three coastal villages and four islets remote from the main island. One village which was located in mountainous area far from the coast was surveyed to serve as control area. About 90% of population inhabiting the study area had at least one blood smear during the three-year period; about one third had three blood smears, and a little over one third had two, and the rest only one examination. Animal and mosquito surveys were carried out at the same period. Followings are the results obtained: All human cases but several had microfilariae identical to the description of B. malayi. The several cases who had morphologically different microfilariae from that of B. malayi need further study for definite conclusion. Five persons randomly sampled from Mf positives and bled every two hours demonstrated nocturnal periodicity between 9 p.m. and 3 a.m. Human is considered to be only reservoir host for human filariasis in the area since animal survey and experimental exposure to the infective larvae of human filaria species showed failure to infect animals. Microfilaria rate, microfilaria density, prevalence of elephantiasis varied by area and age with correlation, which indicated cumulative process of the parasite by repeated exposure and development of host immunity to certain extent. Clinical manifestation of filariasis (symptom complex and elephantiasis ) taken from history and inspection was low in its prevalence with range of 0.9% 11.8% of total population. Only 5.2% of 517 Mf positives had the clinical manifestation. 24.8% of 109 persons with clinical manifestation had microfilaria; 42.9% with symptom complex only, 23.1% with both symptoms and elephantiasis, and none with elephantiasis only were microfilaria positive. Ae. togoi was the only species infected with the filaria. Mosquito infection rate by area showed positive correlation to the Mf rate and density of human population; where the Mf rate and density were high, the mosquito infection rate also high.

8.Perception and Knowledge of Hospice Care and Attitude toward Death Held by Medical Professionals from the Same Region in Korea.

PURPOSE: The purpose of this study was to examine medical professionals' perception and knowledge of hospice and palliative care (HPC) and attitude toward death. METHODS: The survey was performed on 84 physicians and 172 nurses in Busan, Korea. Data were collected from April 5, 2012 to April 30, 2012. RESULTS: Regarding the purpose of HPC, the most popular perception was "To alleviate pain and accommodate comfort" among both physicians and nurses. For the need for HPC, "Terminal patients need a separate ward for emotional fluctuation" the answer chosen by both groups the most. Both groups scored low on knowledge of HPC. For pain and symptom management, physicians scored higher than nurses (physicians: 6.97+/-1.82, nurses: 5.68+/-1.93, P<0.001). CONCLUSION: Considering the survey results, we suggest development of a program to improve medical professionals' perception and knowledge of HPC and attitude toward death.

9.Monostotic Eosinophilic Granuloma of the Skull: A Case Report.

The authors report a case of monostotic eosinophilic granuloma which occured in the skull. A 37 year-old woman was presented with headache and soft mass in the occipital region. Plain skull X-ray showed a "punched out" osteolytic lesion in the right paramedian posterior parietal bone. Computed tomography shows a soft tissue mass and irregular bone destruction in corresponding region. On magnetic resonance image, the mass was of high signal intensity, with subgaleal extension and looks like a shirt stud or "collar button" on T2-weighted image. Total excision of the tumor and postoperative radiotherapy(1000cGy) were performed. The woman had no headache complains nor showed any focal neurologic deficit during the six months follow-up period. We report a case of monostotic eosinophilic granuloma of the skull with review of the pertinent literatures.

10.The Effect of Subconjunctival Retention of Perfluoropropane Gas on the Success Rates of Trabeculectomy in Rabbits.

PURPOSE: This study was aimed to evaluate the effect of subconjunctival retention of perfluoropropane gas on the success rates in trabeculectomized rabbits. METHODS: Fifthteen white rabbits were trabeculectomized by fornix-base conjunctival flap, full thickness sclerotomy and iridectomy. The right eye was control and the left eye was injected with 0.2 ml perfluoropropane gas(C3F8) into subconjunctival space through the 30 guage long needle. Each rabbit was examined for bleb morphology under slit-lamp microscope and measured for intraocular pressure by tonopen at preoperative, postoperative 3 days, 1 week, 2 weeks, 4 weeks, and 6 weeks. The results were analyzed by Wilcoxon rank sum test in SAS release 6.12 program. RESULTS: Mean gas retention time was 15+/-2 days. Intraocular pressure measured at postoerpative 2 weeks was statistically significantly lower in gas injected group than control group. At postoperative 2 weeks, fibrovascularization was more prominent in control group than gas injected group. Fibroblast proliferation and collagen tissue formation were more prominent in control group than gas injected group at postoperative 6 weeks. CONCLUSIONS: Subconjunctival perfluoropropane gas(C3F8) bubbles, via "spacer" effect, aid the maintenance of filtering blebs in the early postoperative period. This technique may be useful in augmenting or salvaging blebs at high risk of failure.
