3.Radiologic Intervention of Esophagobronchial Fistula in Adults: Effeativeness of Oirect Fistula Occlusion and Pre-surgical Wire Insertion.

PURPOSE: To assess the role and efficacy of radiologic intervention in the adult patients with esophagobronchial fistula. MATERIALS AND METHODS: Radiologic intervertion was performed in 4 patients with esophagobronchial fistula. We tried direct occlusion of fistula tract by glue(histoacylate 0.4cc + lipiodol 0.3cc) and coil in 2 patients and performed presurgical wire insertion by using 8F curved catheter via esophageal opening of fistula tract. The latter procedure was carled out to detect the fistula tract easily at operation. RESULTS: Direct occlusions of fistula tract by glue and coil were performed only to fail in 2 patients. Pre-surgical wire incertion by using 8F curred catheter under guidance of fluroscopy in 3 patients were helpful in detecting fistula tract at operation. CONCLUSION: Direct occlusion of the fistulous tract in esophagobronchial fistulas was ineffective. However, presur- gical wire insertion by using cured catheter under fluroscopy guidance is helpful to detect the fistula tract easily at operation.

4.A Case of Eccrine Angiomatous Hamartoma.

Eccrine angiomatous hamartoma is a distinct developmental anomaly showing, histologically, hyperplasia of eccrine sweat apparatus and of vascular elementa in the same lesion, and clinically, the angiomatous lesions with occasional tenderness and profuse local sweating. Recently, we examined an 18-year-old, otherwise healthy girl having, on the anterior aspect of the right leg, an extensive, irregularly marginated and somewhat bluish, tender, firm subcutaneous mass. The mass was studded with several overlying macular, verrucous or nodular angiomatous patches which showed conspicuous hyperhidrosie. Histological examination revealed marked hyperplasia, hypertrophy and cystic dilatations of eccrine sweat apparatus accompanied by proliferation of the capillary hemangiomatous component and ectopic, thick smooth muscle bundles in the deep dermis and subcutaneous tissue.

8.Nationwide Telephone Survey on Leisure Time Physical Activity among Koreans.

Epidemiologic research has demonstrated that varying grade of physical activity has protective effects on several chronic diseases, including coronary heart disease, hypertension, diabetes mellitus, osteporosis, anxiety and depression. Despite the importance of physical activity as an health promotion factor, there have been only a few research data available in Korea. This telephone interview survey on randomly sampled 1,060 adultsaged 15-69 years was carried out to find out the prevalence of self-reported leisure-time physical activity among Korean adults in terms of health promotion practice. The nationwide telephone interview survey was conducted from April through May 1997. Multistage stratified random sampling method was used for sample selection, the respondent was randomly selected from an entire group of adults who lived at the residence connected with the telephone number. A total of 1,060 adults were interviewed and the interviewees' leisure-time physical activity was categorized into four patterns: 'physically inactive', 'irregularly active', 'regularly active, not intensive', and 'regularly active, intensive'. About 53% of the respondents were physically inactive, 15% irregularly active, 14% regularly active, and only 18% were regularly active, intensive. Women were less active than men anc persons of lower socioeconomic status(SES) were less active than higher SES. Physical activity was not significantly associated with smoking status, drinking pattern and overweight, but negatively associated with daily smoking amount and total duration of smoking in smokers. Individuals with higher lever of physical activity were more likely to check blood pressure regularly and have had screening examination for stomach cancer. Conclusively, the proportion of Korean people who perform appropriate level of physical activity in terms of health promotion is still very low, thus an enforced intervention program becomes indispensable to achieve the national health goal by the year 2000.

9.A study on concentration and application time of lithium sulfate-contained polyacrylic acid for adequate crystal growth.

The purpose of this study was to compare shear bonding strengths and debonding patterns of the ceramic brackets attached on the crystal which were grown on the enamel surface of a tooth with different concentrations of lithium sulphate-contained polyacrylic acid in different application times. Four kinds of concentrations of mixed solutions were made and applied to the enamel surface on extracted human premolars. The solutions were made by adding 0.3M or 0.6M of lithium sulfate to 50% or 65% of polyacrylic acid with 0.3M sulfuric acid. The solutions were applied for 30 or 60 seconds. After bonding, a universal testing machine was used to measure the shear bond strength, and then observations were made of debonding patterns through the stereoscope. And the enamel surface was observed through the scanning electron microscope to examine the pattern of crystal growth and debonding. The results were as follows: 1, Shear bond strength in the enamel surface treated with 50% polyacrylic acid was higher than that with 65% polyacrylic acid. 2. There were no statistical differences in shear bond strength according to concentration of lithium sulfate and application time of solutions. 3. Enamel surface was almost free of resin debris after debonding. 4. Enamel surface treated with 50% polyacrylic acid showed higher density of crystal growth than that with 65% polyacrylic acid under scanning electron microscope.

10.Clinical Analysis of Phyllodes Tumor of the Breast .

PURPOSE: Phyllodes tumor is a rare fibroepithelial tumor of the breast, first described by Johannes Muller in 1838. Much has been written about phyllodes tumor, but very few widely accepted conclusions about its clinical behavior, treatment modality, and prognosis have been reached. This study aims to analyze the clinical, radiological, and pathological characteristics of phyllodes tumor of the breast. METHODS: The medical records of 41 patients with phyllodes tumor who had been treated between February 1982 and August 1998 at the Department of Surgery, Seoul National University Hospital, were retrospectively reviewed for clinical, radiological and pathological findings, treatment modalities, and follow-up results. RESULTS: Of these 41 cases, there were 28 cases (68.3%) of benign tumors and 13 cases (31.7%) of malignanat tumors. All patients were females, and the mean ages of onset were 33.2 years for benign tumors and 40.8 years for malignant tumors. Most patients, 100% of benign and 92.3% of malignant, presented with a papable mass in the breast. The median duration of illness was 2 months for malignant tumors and 8 months for benign tumors. The tumor size was greater than 10 cm in diameter in 5 cases (38.5%) of malignant tumors and in 3 cases (10.7%) of benign tumors. Only 4 cases were preoperatively diagnosed as having a phyllodes tumor by using radiological and fine needle aspiration cytology. Out of the 10 malignant cases reviewed, 5 cases were confirmed as malignant, and 5 cases were confirmed as borderline phyllodes tumors. Cellularity was moderate or above in all 5 malignant and 5 borderline cases. Atypism above moderate degree was found in 4 of 5 (80%) malignant tumors, in 3 of 5 (60%) borderline tumors, and in 4 of 24 (16.7%) benign tumors. Mitotic counts in all 5 malignant cases were 5 or more per 10 high power field while those in the 5 borderline tumors were 2-5 mitoses per 10 high power field. The most commonly performed operative procedures were a simple mastectomy (50%) for malignant tumors and a simple excision (64.3%) for benign tumors. Post-operative adjuvant therapy was done for 4 cases; out of these, 1 case had been initially diagnosed as a malignant phyllodes tumor, but the diagnosis was changed to a benign phyllodes tumor upon review. Of the 27 follow-up cases, recurrences developed in 3 cases (16.7%) of benign tumors. CONCLUSION: From the above results, there were no specific clinical features for differentiating benign from malignant phyllodes tumor preoperatively; therefore, we cannot help depending on the pathologic findings. Pathologic reviews showed that among several criteria, atypism, cellularity, and mitotic count were the most definite pathologic characteristics in differentiating benign from malignant phyllodes tumor. But much more experience and long-term follow-up may be needed to define optimal treatments and to analyze the prognosis for phyllodes tumors of the breast.
