1.The Dietary Intake of Pregnant Women in Daegu Area.

This study was carried out to figure out the dietary intake of pregnant women in Daegu area. A total of 467 pregnant women was recruited at two hospitals in this area to collect the information on dietary intakes, general characteristics, and anthropometric measurements using an interview. Forty-seven women (10.1%) were in the first trimester, 101 women (21.6%) in the second trimester, and 319 women (68.3%) in the third trimester. The mean age, prepregnant weight and height of subjects were not significantly different among the trimester groups. The mean daily nutrient intakes of energy, fat, protein, carbohydrates, phosphate, sodium, vitamin B1, vitamin B2, and vitamin C were significantly different among the trimester groups, but the other nutrients were not different. For the calcium and iron, the percentage of subjects consumed less than 75% of the Korean RDA was over 80%, and for other nutrients such as energy, vitamin B1, and vitamin B2 over 50% of subjects consumed less than 75% of the Korean RDA in the first trimester. The correlations between nutrient intakes and weight gain during pregnancy showed different patterns among the trimester groups. The mean adequacy ratio and dietary variety scores were not significantly different among the trimester groups. The amount of food intake from cereals and cereal products were higher during the second and the third trimester compared to the first trimester, but those of the other food groups were not significantly different. In conclusion, nutrient intakes of the pregnant women for iron, calcium and some vitamins were inadequate and nutritional management programs for these nutrients are necessary.

2.Relationship among Job Satisfaction, Job Characteristics, and Organizational Commitment of Dietitians in Hospital, School, and Industry Foodservices.

Purposes of the study were to : (a) measure the levels of job satisfaction, (b) investigate the degree of job characteristics, (c) determine the levels of organizational commitment, (d) investigate relationships between demographic variables and job satisfaction, and (e) examine the relationship between organizational commitment, job characteristics and job satisfaction. Questionnaire was developed and mailed to 300 dietitians who are currently working in school, hospital and industry foodservices. A total of 177 questionnaires were usuable : resulting in a 59% response rate. Results of this study were summarized follows: 1. Approximately half of the respondents(46.9%) indicated that the were 26 to 30 years old. A total of 112 respondents(63.3%) were unmarried. A bachelor's degree was reported as the most common education level(46.9%) while 6.2% of the participants reported a master's degree. Years of employment in professional practices were ranged from 2 to 5. Approximately 40% of the respondents had earned over 900,000 won per month. 2. Work itself of the JDI was with the highest job satisfaction mean score followed by supervision, co-workers, wage and promotion. 3. In total job satisfaction score, dietitians in school foodservices were a significant lower than dietitians in industry foodservices and hospital foodservices. 4. A significant relationship was found for demographic factors and four job facets in terms of job itself, co-workers, and promotion. 5. A work itself was the most improtant facet th the dietitian followed by wage and promotion. 6. Task identity of the JCI facets was the most prevalent job characteristics followed by variety, dealing with others, feedback and friendship. 7. Three groups of dietitians had no significant difference in total job characteristics score. 8. A group of dietitians with a higher job satisfaction score had a significant higher score in job variety and job autonomy. 9. Organizational commitment mean score with dietitians was 1.88. 10. Job satisfaction score for five facets and total job satisfaction were significantly correlated with organizational commitment score. 11. Job charateristics with job variety, job autonomy, feedback, task identity, friendship were positively correlated with organizational commitment.

3.Motor Unit Numbers Estimation in Abductor Pollicis Brevis Muscle of Normal Adult.

OBJECTIVE: To estimate the motor unit numbers in normal adult hand muscles using manual incremental technique and F-wave technique. METHOD: We estimated the motor unit numbers in abductor pollicis brevis muscles of twenty normal adults. In manual incremental technique, we estimated the motor unit numbers using motor unit count program setted in electrodiagnostic equipment (Dantec, Keypoint). In F-wave technique, we estimated the motor unit numbers dividing maximal M-potential amplitude by mean amplitude of surface motor unit action potential. RESULTS: There was no significant difference of motor unit numbers between right and left abductor pollicis brevis in both manual incremental and F-wave technique. Motor unit numbers using F-wave technique were significantly greater than motor unit numbers using manual incremental technique. CONCLUSION: The difference of motor unit numbers using manual incremental technique and F-wave technique was considered as a difference of functional electrical stimulation. F-wave technique was more physiologic and functional method of motor unit numbers estimation than manual incremental technique.

4.Cyanidin-3-glucoside Inhibits ATP-induced Intracellular Free Ca2+ Concentration, ROS Formation and Mitochondrial Depolarization in PC12 Cells.

Flavonoids have an ability to suppress various ion channels. We determined whether one of flavonoids, cyanidin-3-glucoside, affects adenosine 5'-triphosphate (ATP)-induced calcium signaling using digital imaging methods for intracellular free Ca2+ concentration ([Ca2+]i), reactive oxygen species (ROS) and mitochondrial membrane potential in PC12 cells. Treatment with ATP (100microM) for 90 sec induced [Ca2+]i increases in PC12 cells. Pretreatment with cyanidin-3-glucoside (1micro g/ml to 100microg/ml) for 30 min inhibited the ATP-induced [Ca2+]i increases in a concentration-dependent manner (IC50=15.3microg/ml). Pretreatment with cyanidin-3-glucoside (15microg/ml) for 30 min significantly inhibited the ATP-induced [Ca2+]i responses following removal of extracellular Ca2+ or depletion of intracellular [Ca2+]i stores. Cyanidin-3-glucoside also significantly inhibited the relatively specific P2X2 receptor agonist 2-MeSATP-induced [Ca2+]i responses. Cyanidin-3-glucoside significantly inhibited the thapsigargin or ATP-induced store-operated calcium entry. Cyanidin-3-glucoside significantly inhibited the ATP-induced [Ca2+]i responses in the presence of nimodipine and omega-conotoxin. Cyanidin-3-glucoside also significantly inhibited KCl (50 mM)-induced [Ca2+]i increases. Cyanidin-3-glucoside significantly inhibited ATP-induced mitochondrial depolarization. The intracellular Ca2+ chelator BAPTA-AM or the mitochondrial Ca2+ uniporter inhibitor RU360 blocked the ATP-induced mitochondrial depolarization in the presence of cyanidin-3-glucoside. Cyanidin-3-glucoside blocked ATP-induced formation of ROS. BAPTA-AM further decreased the formation of ROS in the presence of cyanidin-3-glucoside. All these results suggest that cyanidin-3-glucoside inhibits ATP-induced calcium signaling in PC12 cells by inhibiting multiple pathways which are the influx of extracellular Ca2+ through the nimodipine and omega-conotoxin-sensitive and -insensitive pathways and the release of Ca2+ from intracellular stores. In addition, cyanidin-3-glucoside inhibits ATP-induced formation of ROS by inhibiting Ca2+-induced mitochondrial depolarization.

5.Effects of Genistein and Daidzein on the Growth of Human Colon Cancer HCT-116 Cells.

BACKGROUND: Genistein and daidzein are two major soybean isoflavones. They have received increasing attention because of their possible roles for cancer prevention. However, their mechanisms of action and molecular targets on the human colon cancer cells are not fully understood. METHODS: Human colon cancer HCT-116 cells were treated with genistein and daidzein to investigate their effects on the cell growth and this was analyzed with MTT assay. TUNEL assay and Hoechst33342 stain were carried out to identify apotosis. RESULTS: Daidzein was able to inhibit cell proliferation and induce apoptosis of the HCT-116 cells, but genistein didn't affect the cell growth. The ER antagonist ICI182780 didn't attenuate the antiproliferative and proapoptotic effects of daidzein: this means the effect of daidzein on the HCT-116 cells may not be dependent on the ER pathway. The other soybean isoflavone, genistein, attenuated the effects of daidzein on the HCT-116 cells and its mechanism should be elucidated. CONCLUSIONS: These data suggest that daidzein may act as a preventive agent on human colon cancer, and its mechanism of action doesn't involve the ER-dependent pathway.

6.Changes in Flow-Mediated Vasodilation after a Single Session of Hemodialysis.

BACKGROUND: The cardiovascular morbidity and mortality are much higher in the patients who undergo hemodialysis than normal population. Atherosclerotic vascular disease is one of the most important causes of cardiovascular disease, and it is thought to be related to endothelial function. It is known that endothelial function in the end-stage renal disease (ESRD) patients is decreased, but it is not clear the hemodialysis can improve the endothelial function or not. Hemodialysis can remove uremic toxins and improve endothelial function, but it also can cause oxidative stress to vascular endothelial cell. The purpose of this study is to determine whether hemodialysis could improve endothelial function compared with simple ultrafiltration in the same patient. METHODS: We prospectively evaluated endothelial function of hemodialysis patients by flow-mediated vasodilation (FMD) method in the patient's right brachial artery. Total thirteen patients were enrolled in this study, and their right brachial artery diameters were measured by 7 MHz echocardiography probe, and calculated by enlarged scan image. FMD were measured before and after hemodialysis, and measured before and after simple ultrafiltration in the same patient, same four hours of each session. RESULTS: The mean brachial artery diameters were 4.18+/-1.11 mm in pre-hemodialysis and 4.25+/-1.15 mm in pre-ultrafiltration respectively, and there was no statistic difference. Average percent change of brachial artery diameter before and after hemodialysis were 2.20+/-1.67 and 1.35+/-1.22% (p<0.05). The same of ultrafiltration were 2.13+/-1.99 and 2.72+/-1.79% (p=0.431). CONCLUSION: In summary, flow mediated vasodilation after a single session of hemodialysis, in contrast with simple ultrafiltration, could be diminished significantly.

7.Pediatric Extracorporeal Membrane Oxygenation in Korea: A Multicenter Retrospective Study on Utilization and Outcomes Spanning Over a Decade

Background:
Over the last decade, extracorporeal membrane oxygenation (ECMO) use in critically ill children has increased and is associated with favorable outcomes. Our study aims to evaluate the current status of pediatric ECMO in Korea, with a specific focus on its volume and changes in survival rates based on diagnostic indications.
Methods:
This multicenter study retrospectively analyzed the indications and outcomes of pediatric ECMO over 10 years in patients at 14 hospitals in Korea from January 2012 to December 2021. Four diagnostic categories (neonatal respiratory, pediatric respiratory, postcardiotomy, and cardiac-medical) and trends were compared between periods 1 (2012–2016) and 2 (2017–2021).
Results:
Overall, 1065 ECMO runs were performed on 1032 patients, with the annual number of cases remaining unchanged over the 10 years. ECMO was most frequently used for post-cardiotomy (42.4%), cardiac-medical (31.8%), pediatric respiratory (17.5%), and neonatal respiratory (8.2%) cases. A 3.7% increase and 6.1% decrease in pediatric respiratory and post-cardiotomy cases, respectively, were noted between periods 1 and 2.Among the four groups, the cardiac-medical group had the highest survival rate (51.2%), followed by the pediatric respiratory (46.4%), post-cardiotomy (36.5%), and neonatal respiratory (29.4%) groups. A consistent improvement was noted in patient survival over the 10 years, with a significant increase between the two periods from 38.2% to 47.1% (P = 0.004). Improvement in survival was evident in post-cardiotomy cases (30–45%, P = 0.002).Significant associations with mortality were observed in neonates, patients requiring dialysis, and those treated with extracorporeal cardiopulmonary resuscitation (P < 0.001). In pediatric respiratory ECMO, immunocompromised patients also showed a significant correlation with mortality (P < 0.001).
Conclusion
Pediatric ECMO demonstrated a steady increase in overall survival in Korea;however, further efforts are needed since the outcomes remain suboptimal compared with global outcomes.
8.A Case of Belsey Mark IV Fundoplication after Failed Nissen Antireflux Surgery.

Antireflux surgery has been indicated in gastroesophageal reflux disease (GERD) that does not respond to medical treatments. Although the most commonly performed operation is Nissen fundoplication, Belsey Mark IV fundoplication is indicated for more complicated cases, such as, in cases of a failed Nissen operation or a long lasting hiatal hernia. Here, we report a case of Belsey Mark IV fundoplication for a failed Nissen fundoplication. The infant developed frequent times of aspiration pneumonia after initial Nissen for a hiatal hernia with GERD during the newborn period. At 15 months of age, a 2nd Nissen operation was attempted, but fundoplication was not available because of excessive mesenteric adherence to the liver and cardia. Therefore, Belsey Mark IV fundoplication was performed via trans-thoracic approach, which can provide full esophageal mobilization and better visualization of the herniated fundus and the surrounding tissues. Subsequently, she has shown an improved general condition without GERD.

9.A Case of Bleeding from a Jejunal Gastrointestinal Stromal Tumor Diagnosed by Double Balloon Enteroscopy.

Gastrointestinal bleeding from a small bowel lesion is uncommon, but it is the most common cause of obscure gastrointestinal bleeding and it can go undiagnosed using traditional upper endoscopy and colonoscopy. Recently, capsule endoscopy and double-balloon enteroscopy (DBE) were developed as new techniques for visualization of the entire small intestine and have improved access to the lesion site in patients with obscure gastrointestinal bleeding. In this report, we describe a 49-year male presenting with melena from a gastrointestinal stromal tumor (GIST) of the proximal jejunum that was diagnosed by DBE and resected by laparoscopic surgery.

10.A Clinical Study of the Bacterial Cultures Obtained from the Epidural Catheters Used in Postoperative Continous Epidural Analgesia.

Epidural anesthesia is a technique of regional anesthesia in which the spinal nerves are blocked as they pass through the epidural space. The current study was conducted to determine the extent of contamination of epidural catheters and to attempt to idntify the factors contributing to contamination. The catheters removed from fifty patients were studied, two cultures were taken from each catheter and the following results were obtained. 1) Catheter tips(20%) in ten cultures and portions of the catheter located in the subcutaneous tissue in nine cultures(18%) were found to be contaminated. 2) Staphylococcus epidermidis was found in eight patients, and staphylococcus aureus and xanthomonas maltophilia were found in two patients each other. 3) In the contaminated populations, no significant difference between the growth rate of organisms and the factors in relation to the duration of catheter implantation, age of the patients and the site of catheter inserition was found.
