1.Report of the Korean Association of ExternalQuality Assessment Service on AutoimmuneTesting (2018–2019)

Under the Korean Association of External Quality Assessment Service,autoimmune proficiency testings (PT) for six test items were performed in2018–2019 for laboratory quality improvement. We conducted two trials peryear and sent three PT materials for anti-nuclear antibody (ANA) testing andtwo PT materials for anti-mitochondrial antibody (AMA), anti-smooth muscleantibody (SMA), anti-thyroglobulin antibody (anti-Tg), anti-thyroperoxidaseantibody (anti-TPO), and anti-double stranded DNA antibody (anti-dsDNA)testing in each trial. The analysis was conducted based on the informationand results of each test item entered by the laboratory. The report comprisedof a common report that showed the characteristics of all the participatinglaboratories and a laboratory-specific report that showed the assessmentdata of individual laboratories. The intended response rates for ANA, AMA,SMA, and anti-dsDNA qualitative tests were over 97.5%, 88.2%, 85.0%, and90.4%, respectively. The coefficient of variations for anti-Tg and anti-TPO was10.4%–70.1% and 16.6%–21.0%, respectively. The number of participatinglaboratories in 2019 was more than that in 2018. We believe this statisticalreport will be useful to interpret external PT results and set up autoimmuneassays at each laboratory.
2.BioSubroutine: an Open Web Server for Bioinformatics Algorithms and Subroutines.

We present BioSubroutine, an open depository server that automatically categorizes various subroutines frequently used in bioinformatics research. We processed a large bioinformatics subroutine library called Bio.pl that was the first Bioperl subroutine library built in 1995. Over 1000 subroutines were processed automatically and an HTML interface has been created. BioSubroutine can accept new subroutines and algorithms from any such subroutine library, as well as provide interactive user forms. The subroutines are stored in an SQL database for quick searching and accessing. BioSubroutine is an open access project under the BioLicense license scheme.

3.Report on Unexpected Antibodies Detected Exclusively in the Serum

In 2023, an 81-year-old woman requested a preliminary red blood cell transfusion during pancreatic cancer surgery, and antibody screening was performed using automated blood typing equipment. The antibody screening test results were negative when plasma was used as the sample but positive when the serum was used instead. The 4℃ test was positive when using the enzyme method but negative at warm temperatures. An unknown antibody was identified using a commercial antibody identification test (Bio-Rad, CA, USA). A clinically insignificant cold antibody was detected only in the serum samples, and the negative blood was ultimately released following the third stage of cross-matching using anti-human globulin.
4.Two Cases of Super-Giant Coronary Aneurysms after Kawasaki Disease.

Acute giant coronary aneurysm after Kawasaki disease (KD) is a catastrophic complication that can be fatal and very difficult to manage. However, no fixed consensus has been reached for the management of super-giant coronary aneurysms in the acute setting. Here, we report the successful management of young children with super-giant coronary aneurysms after KD. Based on our experience, hemodynamic stabilization to prevent further coronary dilation or rupture and strict anticoagulation to avoid thrombus formation are mandatory in the management of this condition.

5.Safety and Efficacy of the Off-Label Use of Milrinone in Pediatric Patients with Heart Diseases.

BACKGROUND AND OBJECTIVES: Milrinone is often used in children to treat acute heart failure and prevent low cardiac output syndrome after cardiac surgery. Due to the lack of studies on the long-term milrinone use in children, the objective of this study was to assess the safety and efficacy of the current patterns of milrinone use for > or =3 days in infants and children with heart diseases. SUBJECTS AND METHODS: We retrospectively reviewed the medical records of patients aged <13 years who received milrinone for > or =3 days from January 2005 to December 2012. Patients' characteristics including age, sex, height, weight, and body surface area were recorded. The following parameters were analyzed to identify the clinical application of milrinone: initial infusion rate, maintenance continuous infusion rate, total duration of milrinone therapy, and concomitantly infused inotropes. The safety of milrinone was determined based on the occurrence of adverse events such as hypotension, arrhythmia, chest pain, headache, hypokalemia, and thrombocytopenia. RESULTS: We assessed 730 admissions (684 patients) during this period. Ventricular septal defects were the most common diagnosis (42.4%) in these patients. Milrinone was primarily used after cardiac surgery in 715 admissions (97.9%). The duration of milrinone treatment varied from 3 to 64.4 days (> or =7 days in 149 admissions). Ejection fraction and fractional shortening of the left ventricle improved in patients receiving milrinone after cardiac surgery. Dose reduction of milrinone due to hypotension occurred in only 4 admissions (0.5%). Although diverse arrhythmias occurred in 75 admissions (10.3%), modification of milrinone infusion to manage arrhythmia occurred in only 3 admissions (0.4%). Multivariate analysis indicated that the development of arrhythmia was not influenced by the pattern of milrinone use. CONCLUSION: Milrinone was generally administered for > or =3 days in children with heart diseases. The use of milrinone for > or =3 days was effective in preventing low cardiac output after cardiac surgery when combined with other inotropes, suggesting that milrinone could be safely employed in pediatric patients with heart diseases.

6.Dietary Calcium Intake and Colorectal Adenoma in Men and Women with Low Calcium Intake

Background:
s/Aims: Calcium is a preventive factor for colorectal cancer, which develops from colorectal adenoma. This study aimed to investigate the association between dietary calcium intake and prevalence of colorectal adenoma among Korean adults.
Methods:
Overall, 612 participants aged ≥ 20 years who underwent gastrointestinal endoscopy at 8 medical centers from 2021 to 2023 were included. Dietary calcium intake was assessed using a food frequency questionnaire and was energy-adjusted using the residual model. Multivariate logistic regression models were used to calculate for the odds ratios (ORs) and 95% confidence intervals (CIs). The associations between dietary calcium intake and colorectal adenoma prevalence were also assessed according to the anatomic subsites and adenoma status (advanced or nonadvanced).
Results:
Among the 612 participants, 269 were diagnosed with colorectal adenoma (170 men and 99 women). With respect to the gender-specific association, low dietary calcium intake was associated with higher prevalence of colorectal adenoma among men (ORs [95% CIs]: 2.13 [0.50– 9.00] for < 250 mg/day; 3.53 [1.06–11.76], 250 to < 350 mg/day; and 1.84 [0.63–5.35], 350 to < 650 mg/day, compared to ≥ 650 mg/day of dietary calcium [p for trend = 0.07]). Similar association was observed among women, but neither the association nor trend was statistically significant (p for trend = 0.59). These inverse associations remained similar for distal colon/rectal adenoma among women and advanced adenoma among men.
Conclusions
Low dietary calcium intake was associated with high colorectal adenoma prevalence, particularly among men. Given the limited number of studies among Asian populations, our findings should be replicated in other Asian groups.

As fluoroscopy-guided interventional procedures gain popularity, the associated health threats from radiation exposure to interventionalists during these procedures are increasing. Therefore, an understanding of the potential risks of radiation and careful consideration on minimizing exposure to radiation during the procedures are of paramount importance. The Korean Pancreatobiliary Association has developed a clinical practice guideline to minimize radiation exposure during fluoroscopy-guided interventional procedures. This guideline provides recommendations to deal with the risk of radiation exposure to interventionalists who perform fluoroscopy-guided procedures, and emphasizes the importance of proper and practical approaches to avoid unnecessary radiation exposure.
