1.Video Archiving and Communication System (VACS): A Progressive Approach, Design, Implementation, and Benefits for Surgical Videos

Objectives:
As endoscopic, laparoscopic, and robotic surgical procedures become more common, surgical videos are increasingly being treated as records and serving as important data sources for education, research, and developing new solutions with recent advances in artificial intelligence (AI). However, most hospitals do not have a system that can store and manage such videos systematically. This study aimed to develop a system to help doctors manage surgical videos and turn them into content and data.
Methods:
We developed a video archiving and communication system (VACS) to systematically process surgical videos. The VACS consists of a video capture device called SurgBox and a video archiving system called SurgStory. SurgBox automatically transfers surgical videos recorded in the operating room to SurgStory. SurgStory then analyzes the surgical videos and indexes important sections or video frames to provide AI reports. It allows doctors to annotate classified indexing frames, “data-ize” surgical information, create educational content, and communicate with team members.
Results:
The VACS collects surgical and procedural videos, and helps users manage archived videos. The accuracy of a convolutional neural network learning model trained to detect the top five surgical instruments reached 96%.
Conclusions
With the advent of the VACS, the informational value of medical videos has increased. It is possible to improve the efficiency of doctors’ continuing education by making video-based online learning more active and supporting research using data from medical videos. The VACS is expected to promote the development of new AI-based products and services in surgical and procedural fields.

3.Factors Predicting Response to Splenectomy in Patient with Immune Thrombocytopenia Purpura.

OBJECTIVE: Splenectomy has been proposed to be the standard therapy for patients with steroid refractory immune thrombocytopenic purpura (ITP). This study aimed to describe valuable factors predicting the effect of splenectomy in patients with ITP. METHODS: A total of 51 adult patients who underwent splenectomy for steroid refractory ITP were evaluated their medical records retrospectively. The response to the treatment was classified on the basis of the platelet count. RESULTS: The responding group included 35 patients (68.8%), the partial-responding group included 4 patients (7.8%), and non-responding group was 12 patients (23.5%). On univariate analysis, the response of splenectomy correlated with only the intravenous immune globulin (IVIG) response (66.7%, P=0.006), but hemolysis, autoantibody, a presence of accessory spleen, the response of steroid were not significantly associated with the effect of splenectomy. On multivariate analysis, the response of IVIG and the amount of platelet transfusion were independent variables of the response of splenectomy. CONCLUSION: Patients with ITP who have good responses to IVIG are likely to have a good or favorable responses to splenectomy.

Human mesenchymal stem cells (MSCs) have emerged as attractive cellular vehicles to deliver therapeutic genes for ex-vivo therapy of diverse diseases; this is, in part, because they have the capability to migrate into tumor or lesion sites. Previously, we showed that MSCs could be utilized to deliver a bacterial cytosine deaminase (CD) suicide gene to brain tumors. Here we assessed whether transduction with a retroviral vector encoding CD gene altered the stem cell property of MSCs. MSCs were transduced at passage 1 and cultivated up to passage 11. We found that proliferation and differentiation potentials, chromosomal stability and surface antigenicity of MSCs were not altered by retroviral transduction. The results indicate that retroviral vectors can be safely utilized for delivery of suicide genes to MSCs for ex-vivo therapy. We also found that a single retroviral transduction was sufficient for sustainable expression up to passage 10. The persistent expression of the transduced gene indicates that transduced MSCs provide a tractable and manageable approach for potential use in allogeneic transplantation.

5.Hepatoduodenal fistula formation following transcatheter arterial chemoembolization and radiotherapy for hepatocellular carcinoma: treatment with endoscopic Histoacryl injection.

A 71-year-old male patient was readmitted to our hospital 1 month after discharge because of relapse of abdominal pain. He had been diagnosed with hepatocellular carcinoma (HCC) 1 year prior and had undergone repeated transcatheter arterial chemoembolization and radiotherapy. During the last hospitalization, he was diagnosed with a liver abscess complicated by previous treatments for HCC and was treated with intravenous antibiotics and abscess aspiration. Follow-up abdominal computed tomography revealed a liver abscess with a duodenal fistula, which was successfully treated with endoscopic Histoacryl injection into the fistula. Liver abscesses with duodenal fistulas rarely occur, but they are intractable and possibly fatal in patients with HCC. In the literature, they have frequently been managed only with abscess treatment without fistula management. We herein report the first case of a patient with a liver abscess complicated by a fistula between the duodenum and the abscess, which was treated with endoscopic Histoacryl injection.

6.A Retrograde Approach to Coronary Ostial Stenosis after a Bentall Procedure in a Patient with Behcet's Disease.

We describe a case of chronic total occlusion of the right coronary artery ostium 5 months after a repeated Bentall procedure in a patient with Behcet's disease. In this patient, an antegrade approach to delivering the guidewire during percutaneous coronary intervention was not successful. Coronary angiography revealed the existence of collateral blood supply from the left coronary artery. Using a retrograde approach, a guidewire was successfully advanced from the distal left circumflex artery through the collateral vessel and into the posterolateral branch of the right coronary artery. After the guidewire crossed over the occluded right coronary artery ostium and was snared into the ascending aorta, antegrade access for balloon and stent delivery succeeded.
