Thoracic kyphosis was measured on chest lateral radiographs of 885 normal subjects, comprising of 509 males and 346 females. Subjects were accepted as normal if they had no problems of lung, heart of spine such as scoliosis or significant compression fracture radiologically. Kyphotic angle was measured by Cobbs method from T3 to T10. Index of wedging which represents the wedging degree of vertebral body was measured on apical vertebra, and also the location of apical vertebra was studied. The results obtained were as follows: 1. Kyphotic angle through all age groups showed the range 17°–33° in males and 17°–34° in females. 2. Kyphotic angle increased with the increase of age, but not related with sex. Therefore normal range of kyphotic angle should be determined in respective age group accordingly. 3. Index of wedging in apical vertebra through all age groups showed the range of 1.07–1.21 in males and 1.07–1.23 in females. 4. Index of wedging increased with the increase of age, and normal range of index of wedging in respective age group was supposed to be more significant. 5. There was no correlations between the regression of kyphotic angle and index of wedging. 6. The most frequent apical vertebra was located on T6, and then T7 and T5 in order.

Reoperation*

The clinical review was done on 17 cases with the foreign body in the upper gastrointestinal tract who underwent endoscopic removal of foreign body at Kang Nam Sacred Heart Hospital from Dec. 1981 to Dec. 1991. The results were as follows: 1) The age distribution varies from 3 months to 72 years of age, being most prevalent under 5 years of age. 2) The incidence of foreign body in the order of frequency was coin, fish bone, safety pinring, peanut, keyring, food bolus, artificial teeth and medal. Among 17 cases, 9 cases(53.0%) were lodged in the esophagus and 8 cases(47.0%) were lodged in the stomach. 3) Dysphagia or globus was the common symptom in 8 cases(47.2%), followed by absence of symptom in 7 cases(41.2%), upper abdominal discomfort in 1 case(5.8%) and vomiting in 1 case (5.8%). 4) l0 cases(58.8%) of foreign body were removed within 72 hours and 7 cases(41.2%) of foreign body were removed after 72 hours.

Cystic Adenomatoid Malformation of Lung, Congenital* ; Lung*

A very rare case of multifocal eosinophilic granuloma without extraskeletal involvement in 35 year-old ma was experienced. The patient was admitted for the painful swelling of the right clavicular region and was treati with curettage and autograft of iliac bone. Five months later newly developed the pain in the trochanteric lesion left femur without fever, and also was confirmed as the same lesion. Both lesions were treated with curettage and iliac bone graft, and were followed by radiotheraphy. According to the pathological study, those lesions were confirmed as the multifocal eosinophilic granuloma.

In the treatment of elderly hip fracture, early mobilization as a means of reducing early postoperative morbiyt and morbidity is mandatory. Even though Ender nailing have many advantages, most serious problems with use of Ender nailing method are distal migration of nails, subsequent loss of fixation and limited knee joint motion. To prevent these difficult problems, we have used distal wiring technique with Ender nailing since 1988. In operation, we performed Ender nailing as usual manner under image intensifier and for the prevention of the nail migration, we performed circumferential wiring through the each Ender nail eye and above the adducter tubercle of the distal femur. Forty six cases were available with minimum follow-up of six months which were healed radiologically and clinically (Mean follow up: 21 months). The mean age was 74 years (Range: 52 years-88 years)and showed 26 cases of famale and 20 cases of male. The radiological morphometry using Singh index showed poor bone quality: 36 cases were below Grade III and the fracture type was classified by Tronzo criteria. In results, patients who had Ender nailing with distal wiring showed no distal migration, however one case of proximal migration was observed at the beginning stage of this technique. So, in elderly trochanteric fracture, to reduce the complication of the Ender nailing, especially nail migration, the authors recommend the Ender Nailing with distal wiring technique.

Arrhythmogenic right ventricular dysplasia is a rare heart muscle disorder of unknown cause that primarily involves the right ventricle. It is characterized pathologically by fibrofatty replacement of the right ventricular myocardium. Clinical manifestations include structural and functional abnormalities of the right ventricle, electrocardiographic depolarization/repolarization changes, and presentation with sudden death or ventricular arrhythmias of right ventricular origin. It is one of the important causes of the ventricular arrhythmia or sudden death among apparently healthy young people. We report a case of arrhythmogenic right ventricular dysplasia with the review of the literature.

Huntington's disease (HD), an autosomal dominant dominant disorder of mid-life onset, is characterized by progressive involuntary choreiform movement, psychological change and dementia. We present here one case of Huntington's disease, who was admitted at Seoul National University Hospital from 2nd Oct. to 16 Oct. 1985.

Tibial nonunion is often associated with the soft tissue atrophy and the local circulatory disturbance, so it is important to minimize the soft tissue injury and maintain the stability at fracture site for bone union. We reviewed 29 cases of subcortical onlay bone grafts and plate fixations for the nonunion and delayed union of tibial shafts, from Jan. 1988 through Jan. 1994. The results were as follows. 1. Twenty-nine patients who had nonunion of the tibia underwent subcortical cancellous onlay bone graft and plating, and 28 fractures(96.6%) went on to union. 2. Subcortical elevation or shingling of cortical bone fragments of the tibial cortex, is advocated to provide sound biological condition in atrophic soft tissue envelope. 3. Narrow DCP encouraged early motion and reliably promoted obtaining anatomical alignment. So, in treatment of protracted tibial nonunion, especially in an area with deficient soft tissue or unyielding scar tissue, subcortical onlay bone graft and plate fixation is very useful method, result in good union and no skin problem.
