2.The Role of Initial Relative Renal Function in Unilateral Hydronephrosis.

Glassy cell carcinoma is an unusual neoplasm of the uterine cervix with highly aggressive clinical behavior. On cervico-vaginal smear examination, the tumor has well confused of atypical repair cell of the endocervix. Recently, we have experienced two cases of glassy cell carcinoma of the uterine cervix, diagnosed on cervico-vaginal smears and confirmed on following histologic sections. The cervico-vaginal smears revealed abundant clusters with well defined boarders. The cell clusters were composed of large tumor cells. The tumor cells had distinct granular cytoplasm and eosinophilic macronucleoli. Characteristic cytologic features of this tumor were discussed in view of differential diagnosis.

3.A Clinical Study of the Therapeutic Effect of Clocortolone on soem Dermatoses.

Cloeortolone (9-a-chloro-6-a fluoro-11-B 21 digydroxy-16 methyl preganl,4-diene) is a newly synthesized corticonsteroid which has been found to have potent antinflammatory action and good tolerance through animal experiment. Several investigators reported its therapeutic affect in various dermatoses by topical application. The present was undertaken to evaluate the therapeutic effect of 0.1% clocortolone cream (Purantix-Sandoz) in patients with eczematous dermatoses and with psoriasis and to compare its effect with that of I% hydrocortisone. Eleven patients with contact dermatitis, 10 with psoriasis entered this study durtin the period of 6 months from January to june, 1977 at the Department of Dermatology, Seoul National University Hospital. All patients were instructed to apply 0.1% Clocortolone cream on one side of their lesion and 1%hydrocortisone cream on the opposite site for two weeks. After wo weeks' period of observation the status of the lesion was evaluated clinically and the tesults were as follows. 1. Ninety one percent of patiens with contact dermatitis, 70% of atopic dermatitis, 60% of nenrodermatitis, 67% of nummular eczema and 60% of psoriasis responded very effectively or moderately effectively. The therapeutic responses were generally similar to that of 1% hydrocortisone. 2. During the ovservation period, there was no systemic of local side effect of 0.1% Clocortolone cream.

4.A Case of Well Differentiated Liposarcoma of Spermatic Cord.

Liposarcoma of the spermatic cord is very rare and preoperative diagnosis is difficult. Well differentiated liposarcoma is in general less aggressive than other type of liposarcoma and seldom metastasizes. Radical orchiectomy with wide local excision appear to be adequate treatment modality and offer better prognosis. We report a case of well differentiated liposarcoma in 51-year-old male with brief review of the literatures.

6.Treatment of Infected Nonunion of the Femur with Marked Shortening by Compression and Gradual Distraction at the Nonunion Site: A Report of 2 cases.

The authors report two cases of successful reconstruction in infected nonunion of the femur involving marked shortening by compression and gradual distraction at the nonunion site using Ilizarov external fixator. At first, infection was controlled by radical excision & the administration of systemic antibiotics for four weeks and we also started compression at the nonunion site 3 days after operation. Autogenous iliac bone graft was added to strengthen the site of new bone formation lastly. In this report, the role of the compression seems to provide a good environment for distraction osteogenesis by focal necrosis and triggering inflammation. Compression and gradual distraction may be one of the treatment mordalities for nonunion of a long bone with massive bone loss or shortening, even infected.

8.Two Cases of Vesicoureteric Reflux Treated by Endoscopic Injection of Teflon.

Primary vesicoureteric reflux is regarded as a congenital condition resulting from a short submucosal ureteral segment and an absence of adequate detrusor support behind the intravesical ureter. Several antireflux operations have been devised, all of which entail opening the bladder and performing a variety of procedures on the ureter. Polytef paste is a sterile, relatively inert, injectable paste, containing polytef, glycerine and polysorbate and polytef is Teflon that has been pyrolysed. Subureteric injection of Teflon is a good substitute for open operation which eliminate vesicoureteric reflux because it is well tolerated, avoids open operation and shortens Hospital day. Herein we report two case of vesicoureteric reflux treated by endoscopic injection of Teflon and review the literatures.

9.A Case of Coccidioidomycosis.

A case of systemic coccidioidomycosis with cutaneous lesions has been found in Dec. 1974, for the first time in Korea, which is proved through cutaneous biopsy .and mycologic culture from skin lesion. The patient was 57-year-old Korean male. Skin lesions on left extremities showed several skin colored or ligbt brownish papules and surrounding circular erythematous scaly patches with slightly elevated borders. The patient was also found to have moderate diabetes mellitus, hypokalemia and resultant neuropathy. Histopathology revealed granulomatous lesions in the upper dermis, and numerous spherules both inside and outside of giant cells. Mycologic culture from grind skin specimen on Sabourauds glulose agar showed snow-white colony and ather typical arthrospores. Ampbotericin B intravenous injection was started and continued for 16 days. During a,dministration of the drug, side effects consisting of chills, fever, headache, chest pain and dyspnea developed. Therefore, further administration of the drug could not be done. During tbe several months following last discharge from the hospital, the patient has been, with no further specific therapy, in relatively good general condition, and developed no new lesions. The old papules have diminished in size, but did not disappear.

10.Chest Wall Reconstruction with Muscle Flap.

The chest wall plays a role to protect vital organs in the trunk such as heart and lung, and to facilitate a semi-rigid structure for breathing. When a defect occurs in a chest wall for such reasons as trauma, tumor, infection, inflammation, post-radiation necrosis, congenital anomaly, and so forth, not only anatomical, but also functional aspects should be considered in reconstruction process. It is, generally speaking, difficult to reconstruct a chest wall defect, since these patients tend to be old aged and have underlying disease with chronic and heavily contaminated wounds which had experienced irradiation or operation. In chest wall reconstruction, the authors utilized muscle flaps in trunk to secure a semi-rigid structure, because muscle flaps in trunk are easily accessible, and have reliable blood vessels. We have conducted 42 cases of chest wall reconstruction between Feb, 1996 and Jan, 2003. Except one case, we succeeded in gaining satisfactory results in functional and anatomical aspects. Based on these experiences, we would like to clarify the principles of chest wall reconstruction as below, helping to choose a proper method in the operations. The principles are appropriate debridement, skeletal reconstruction when indicated, coverage of soft tissue with muscle flap and removal of dead spaces.
