4.An Immunohistochemical Study for the neu and ras Oncoprotein and Epidermal Growth Factor Receptor in the Breast Carcinoma.

To evaluate correlation between the expression of neu and ras oncoprotein and epidermal growth factor receptor on breast carcinoma and other known prognostic factors, immunohisto-chemical studies were performed. Positive reaction for neu, which appeared as brown granular deposits along cell surface and cytoplasm of the tumor cells, was significantly correlated with the histological grade but not with other prognostic factors such as tumor size, lymph node me-tastasis, local recurrence, and estrogen and progesteron receptor status. Also granular deposits of ras were noted in the carcinoma cells in the cytoplasm, while the epithelaial cells of the normal lobule and duct showed negative reation. But expression of ras was not significantly associated ras with other prognostic factors. The reaction for EGFR was mostly negative on epithelial cells of both the normal lobule and duct, and was not significantly associated with other prognostic factors. The results suggested that expression of the neu oncoprotein is significantly associated with the histological grade of breast carcinoma, while the ras and the EGFR do not show significant prognostic value.

5.Statistical Analysis of Serologic Test for Syphilis in Normal Population (1981 ~ 1984).

Serologic tests for syphilis including VDRL and TPHA tests were carried out in 5,413 VISA applicants for emigration, from January, 1981 to December, l984 The results are summarized as follows: 1. In 5,413 VISA applicants, the reactive rate of VDRL test was 2,0% totally. The reactive rate of VDRL test of male was 2. 6%, and that of female was 1. 6% 2. The reactive rate of 1983 was 1.4% and that was the lowest in annual incidence, but there was no statistically significant differences in comparing the annual incidences. 3. The reactive rate increased with age, and males above the 5 th decade revealed an especially high reactive rate. 4. The biologic false positive rate of VDRL teet was 16. 5%, in gl VDRL reactive persons, using TPHA as standard. 5. The quantitative test of VDRL showed low titer below 1: 4 in 88. 4%, and high titer above 1: 8 in 11. 6% of the applicants.

6.MRI features of right atrial angiosarcoma: a case report.

Primary tumors of the heart are rare, and one-third of them are malignant. The most common malignant intracardiac tumor is angiosarcoma, which usually involves right atrium. MR features in one patients with right atrial angiosarcoma is presented and the literature briefly reviewed. Angiosarcoma demonstrated slightly heterogeneous intermediate signal intensity on T1-weighted images, high signal intensity on T2-weighted images and higher signal intensity on Gd-DTPA enhanced images. MRI provided more information regarding the extent of the tumor than 2D echocardiography or CT.

7.An Ultrastructural Cytochemical Study on the Mechanism in the Development of Fatty Liver Induced by the Orotic Acid.

For the elucidation of a possible pathogenetic mechanism of fatty accumulation by the orotic acid in the liver, male Sprague-Dawley rats were fed on semisynthetic diet containing 1 per cent orotic for a week, and followed by the morphological examination on the rat livers. The experimental animals were sacrificed on the 1st, 2nd, 3rd, and the 7th days of orotic acid feeding and daily sacrifice was performed after conversion to normal diet feeding during the performed after conversion to normal diet feeding during the period of successive 7 days. The control group animals were fed on normal regular diet, or semisynthetic diet without orotic acid. The liver tissues were examined by the light ad electron microscopy, together with electron microscopic cytochemical study for glucose-6-phosphatase and thiamine pyrophosphatase. On the light microscopy, the lipid droplets began to appear in the 3rd day of orotic acid feeding and then occupy through the all hepatic lobules in the 7th day of orotic acid feeding. On the electron microscopy in the 3rd day of orotic acid administraion, the endoplasmic rdticulum were fragmented and vesiculated, and vesicles containing VLDL particles were demonstrated closely around the Golgi complex. In the 7th day, Golgi complex were increased in numbers. The limiting membrances of lipid vacuoles were regarded to originated from rough-surfaced endoplasmic reticulum of account of demonstration of glucose-6-phospatase in them. In the Golgi complex the activity of thyamine pyprophosphatase were markedly decreased in the 1st day of orotic acid feeding, and then the activity of this enzyme is failed to present afterthat. On the above findings it is assumed that orotic acid depresses the function of trans aspect of Golgi stack and the VLDL is interrupted and followed by the retrograde accumulation of VLDL. The cis aspect of Golgi stack and rough-surfaced endoplasmic reticulum were accumulated with VLDL particles, and eventually coalesced vesicles of VLDL with become lipid vacuole. The sequential alteration of endoplasmic reticulum and Golgi complex is supporting the concept of conformational change of membranes in cytocavitary net-work.

9.A Clinical Study on Polyarticular Juvenile Rheumatoid Arthritis (JRA) (III. Polyarticular Type) .

OBJECTIVE: The purpose of this study was to analyse clinical manifestations and laboratory findings in childhood patients with polyarticularonset juvenile rheumatoid arthritis (JRA). METHODS: Eleven cases of polyarticular JRA who were diagnosed and treated in the Department of Pediatrics, Seoul National University Children's Hospital from June 1988 to May 1995 were investigated for clinical manifestations and laboratory findings. RESULTS: 1) There were 6 males and 5 females and their ages of onset were 4 years to 15.1 years(mean 10.9 years). 2) Systemic manifestations were not observed, but low-grade fever was noted in 5 patients. 3) The involvement of joints was symmetric in 9 patients and asymmetric in 2 patients. 4) The most commonly affected joints were knees and ankles, followed by proximal interphalangeal joints of hand, shoulder, elbow, temporomandibular joint, and other joints. 5) Roentgenographic changes of joints were detected in 6 patients and bone scan in 7 patients showed increased uptake in the involved joints. 6) The main laboratory findings observed were microcytic and hypochromic anemia (64%), thrombocytosis (82%), elevated eryhtrocyte sedimentation rate (100%), positive or increased C-reactive protein(100%), positive rheumatoid factor(RF) (18%), positive antinuclear antibody(ANA) (27%). RF was positive in 2 girls with later age of onset and the pattern of immunofluorescent ANA were all homogeneous. 7) Nonsteroid antiinflammatory drugs (NSAIDs) were used most frequently and steroid with or without sulfasalazine was tried in 4 patients unresponsive to NSAIDs. 8) At last follow-up, 6 cases(55%) were classified as functional class I, 4 cases(36%) as class II, and 1 case(9%) as class III. CONCLUSION: These data showed the clinical manifestations and laboratory findings of polyarticularonset juvenile rheumatoid arthritis in Korean children.

10.Treatment of the Difficult Fractures of the Femur using the Ilizarov External Fixator

There are many difficult problems and various methods in the treatment of difficult fractures of the femur. Form May 1991 to August 1994, 9 cases of difficult fractures of the femur were treated with Ilizarov external fixator. We obtained good result for all cases as follow : 1. There were 1 case of severely segmental and comminuted fracture with bone loss, 3 cases of infected nonunion after internal fixation(2 cases combined with bone loss), 2 cases of non-union after internal fixation and 3 cases of severely segmental and comminuted fractures. The traffic accident injury was in 6 cases and the falling down injury was in 3 cases. 2. The bone union was obtained in all cases and the duration of Ilizarov external fixator application was minimum 7.7 months up to 19 months with an average of 1.3 months and full weight bearing was possible in all cases after removal of the Ilizarov external fixator. 3. We experienced 9 complication; there were 6 cases of pin tract infection which were controlled by antibiotic injection at that site and there were three cases of limitation of motion of knee. So we conclude that the Ilizarov external fixation technique is one of the effective methods for the treatment of difficult fracture of the femur(ex. segmental, comminuted fracture, bone defect, osteomyelitis and nonunion) in spite of having experienced only few cases.
