1.A Case of Benign Lichenoid Keratosis.

A case of benign lichenoid keratosis(BLK) developed in a 25-year-old female is presented. She had two erythematous plaques with slightly verrucous surfaces on the lips for 4 weeks. A biopsy specirnen taken from the periphery of the lesion revealed a focal parakeratosis, the absence of granular cells, and hydropic degeneration of the basal cells in the epidermis. The upper dermal infiltrate composed chiefly of lymphacytes intermingled with a few histiocytes was also seen. The direct immunofluorescent study showed depositions of a few cytoid bodies and fibrin around the areas of the dermoepidermal junction. These plaques on her lips, demonstrating lichenoid tissue changes, cleared in 8 weeks from the evalution, without any treatment.

2.A Study of Pinprick Test with Food Allergens in Urticaria.

BACKGROUND: Prick test, a kind of skin test, is most frequently used in clinical procedure because of its simplicity and safety of application and better correlation with clinical history history as well as specificity and highty reproducibility. OBJECTIVE: Studies into the result and clinical significance of the pinprick test with relationship to food allergens as the contributing factor of urticaria. METHODS: We have examined the skin test with extracts of 45 food allergens(Bencard Ltd. In England) in three groups. 10 acute urticaria patients. 30 chronic urticaria patients, and normal controls. RESULTS: Of the 40 patients with urticaria, 23(57.5%) revealed positive reaction to one or more food allergens in comparison with 9(45.0%) of the control, ad there was no statistically significant difference(p>0.05). Positive reaction to food allergens in 15 food correlates was 11(73.3%), Which was higher than 11(44.4%) in 25 patients with no relation to food. and there was a statisticals difference (p>0.05). Food allergen groups which showed frequent positive reaction were fishes(35.0%), shell fishes(30.0%), cereals(27.5%), fruit(20.0%), vegetabies(17.5%), egg & milk(10.0%), and meats(7.5%) in that order. CONCLUSION: These results suggest that food allergen test might not be useful in evaluation the contributing factor of urticaria.

3.A case of tuberculosis verrucosa cutis.

We report a case of tuberculosis verrucosa cutis in a 34-year-old male patient who presented with asymptomatic verrucous plaque over the right buttock, right inner thigh and right inguinal area. The Mantoux test revealed a strong positive reaction. The histologic picture consists of hyperkeratosis, acanthosis and papillomatosis overlying an inflammatory infiltrate in the upper dermis. Tuberculoid granulomas without caseation necrosis are seen in the dermis. Tubercle hacilli were not demonstrated. The patient was treated with INAH, Rifampicin and Ethambutol for 6 months. To date, the skin lesions have markedly improved.

6.MR Imaging of the Pigmented Villonodular Synovitis of the Knee.

PURPOSE: To describe the magnetic resonance (MR) findings of pigmented villonodular synovitis(PVNS) of the knee, and to evaluate the clinical value of MR in the diagnosis of PVNS. MATERIALS AND METHODS:MR imagings of seven patients with PVNS were studied. The type of lesion, presence of bony erosion, the signal intensity, and the relationship between contrast enhancement and signal intensity on T2-weighted images were analyzed, retrospectively. RESULTS: The lesion was mainly villous in three patients and nodular in four, and bony erosion was seen in one patient. On T2-weighted image, the signal intensity of the villous form was mixed iso- and hypointense in two, hypointense in one, and that of the nodular form was heterogeneous with hypo-, iso-, and hyperintensities. The hypointense portion on T2-weighted image showed poor contrast enhancement, which may suggest hemosiderin deposition or advanced fibrosis. The iso- or hyperintense portion on T2-weighted image showed strong enhancement, which suggest active cellular proliferation. CONCLUSION:MR imaging could be a very useful modality in the diagnosis as well as prediction of histological findings of the PVNS.

We report herein a case of pityriasis lichenoides et variloiformis acuta in a 48- year-old man, who has wide spread erythematous macule, papule, vesicle, hemorrhagic vesicle and reddish-brown crust on the neck, trunk, axillae and both extremities. He was treated successfully with tetracycline 2. 0 gm a day for 4 weeks.
