2.Treatment of Herpes Simplex Keratitis with IDU and Cryocautery.

The effect of IDU and cryocautery in the treatment of Herpes Simplex Keratitis was presented. The cryoapplicator was a model of Krwawlcz probe (2 mm in diameter) which was refrigerated in a mixture of dry ice and methyl alcohol. The applicator was appJied on the lesions 7 seconds each and antibiotics and atropine solutions were instilled. A good result was experienced in the treatment of several cases of dendritic kenititis by freezing the corneal lesions by means of cryoappJicator, which had not been responded to IDU therapy speedy. The deep neovascularization of the cornea in a case of stromal keratitis was disappeared after cryocautery for the treatment of the superficial corneal ulcers.

3.A Case of Muscle Transplantation in the Lateral Rectus Muscle Paralysis.

Muscle transplanation surgery is presently widely accepted as a good precedure not only for cosmetic, but also for functional results, a concept to which Marina's observations on the absence of fixed and predetermined functions in the nuclei and associated tracts of the oculomotor system give great support; however, not everyone is in agreement with this concept. This paper report one case in which the total transplantation of the vertical rectus muscles midway between their insertions and the insertion of the lateral rectus muscle were carried out, in addition recession of the medial rectus muscle and resection of the paralyzed lateral rectus muscle. Our case have been followed up during 8 months to rule out late complications, aspecially ocular movement and diplopia. No late complications, however, have been found up to remaining underaction of the right superior and inferior oblique muscles and also medial rectus muscle. The eyes were straight with abduction of the right eye up to 40 prism diopters. Binocular vision was present and there was no diplopia. The cosmetic result was excellent.

6.A Case of Sparganosis in the Calf.

Sparganosis is a tissue-invading disease caused hy plerocercoid of Spirometra mansoni91. Usually, uncooked snakes or flogs and infected water are primary sources of sparganosis. Sparganosis found in the lower extremities, abdominal wall, breast, inguinal region and scrotum. Simple radiographs show linear or elongated calcification. Serodiagnostic tests have also heen used widely. Confirmative diagnosis of sparganosis is made by surgical removal of the worm. We experienced a case of sparganosis in a 48-year old male in the calf. We report a case and review related articles hriefly.

7.Seroprevalence of toxoplasmosis in Korean pregnant women.

This study was performed in order to evaluate the sero-epidemiological status of toxoplasmosis in pregnant Korean women. Among 5, 175 sera and 750 amniotic fluid samples obtained from pregnant women, 41 serum samples (0.79%) and 10 (1.33%) amniotic fluid samples tested positive for IgG antibodies by ELISA. Fifty one cases showing a score more than 0.25 on ELISA were tested for PCR reaction against the SAG1 gene. Only one case of the 51 ELISA positive cases exhibited a positive reaction on all tests. This case had a history of acute nephropyelitis during early pregnancy, but fortunately, had delivered a phenotypically healthy baby. In this study, the seroprevalence of toxoplasmosis in pregnant women was found to be comparatively low, consistent with previous reports from Korea. However our trials, performed with a variety of diagnostic tools, were considered to be useful for the precise diagnosis of congenital toxoplasmosis.

Mediastinal fibrosis is pathologically characterized by chronic inflammation and fibrosis of mediastinal soft tissue. Mediastinal fibrosis is local expression of a family of systemic fibrosing syndroms. This can result in compression of adjacent mediastinal structures. Idiopathic fibrosing syndromes include retroperitoneal fibrosis, sclerosing cholangitis of the orbit and fibrosis of the thyroid gland(Riedel's struma). The cause of these disorders is obscure, in some instance there is an underlying malignancy, infection, history of drug ingestion, or trauma with retoperitoneal bleeding. Treatment of mediastinal fibrosis depends on structures involved by the fibrotic process. The disease is self limited in most case or improved by steroids uses. We experienced a case of idopathic solerosing mediastinitis with orbital fibrous dysplasia of unknown cause, which was confirmed by open lung biopsy, so reported it with a review of literature.

9.A Case of Primary Disseminated Cutaneous Aspergillosis in Chrinic Active hepatitis.

An 41-year-old woman with chronic active hepatitis developed multible asymptomatic erythematous papules, plaques and purpuras on the both lower leg, feet, forearms and hands. Some lesions progressed to ulcers with central black colored necrotic eschars. Biopsies and cultures demorrstrated a fungus, aspergillus fumigatus a the etilogic agent. No evicience of involvement of other organs by aspergillus fumigatus was found. Intravenous amphotericin B therapy was discontinued because of he immediate adverse effects. The patient received treatment with oral itaconazole(200mg/day) and intravenous fluconazole(400mg/day), but skin lesions have been recurring repeatedly.

10.Serum Testostrone Levels in Leprsy Patients.

Testicular involvement in male patients with leprosy is well documented and may be associated with impotence, sterility and gynecomastia. Testicular histology shows atrophy of the seminiferous tubules with hypertropy and clumping of Leydig cells and hyalinization of the small and medium sized vessels. The hormonal functions of the testes have been studied by a number of workers, usually in patients with testicular atrophy and gynecomastia. In these particular patients, androgens are generally diminished while gonadotropins are increased. The pathogenesis of testicular damage is uncertain, though Wall and Wright(1974) found that testicular germinal cell antibodies were present in 75% of lepromatous, and postulated that :autoirnmunity, Erythema Nodosum Leprosum imrnune complex damage and direct invasion by Mycobacterium leprae may all be contributory.
