1.Immunological Observation of 46 Cases Poliomyelitis

The Author reports here the statistical observation of poliomyelitis from the 46 cases during a 3 month period (June, August, 1971) at the department of Orthopaedic Surgery, Korea General Hospital under the support of Shrine Club in Korea. The statistical results were as follows: 1. Among 46 cases, 30 were male, age of infection of poliomyelitis ranged from 2 month to age of five. The majority of cases were under the age of one and 95% of total patients were under the age of three. 2. Among 46 cases, 78.2% was not vaccinated fer poliomyelitis. The sequela of paralysis were more prevalent in this group. 3. tibialis anterior muscle and quadriceps femoris muscle were paralized predominently, 78.2% and 60.8% respectively. 4. Seventy eight and two tenth percent (78.2%) of patient seeked Chinese medicine practitionar with herb and acupunture, following paralysis, only 39.1% reported to the hospital and 4.4% did not see any treatment at all.

4.A Clinical Study of Zomepirac

A clinical study was performed to evaluate the effectiveness of Zomepirac in 27 patients who had undergone bone surgery, 13 patients who had soft tissue surgery and 7 non-surgical patients admitted to the department of orthopedic surgery of Hanyang University Hospital from Oct., 1981 to Nov., 1981. The conclusions were as follows: 1. To decrease pain by half, Zomepirac was determined to be much more effective in patients with severe pain than in patients with moderate pain. 2. The initial pain relief was also more marked in patients with severe pain than in patients with moderate pain. 3. The duration of analgesia with Zomepirac was longer in patients with severe pain than in patients with moderate pain. 4. The duration of pain relief after medication with Zomepirac was from 1 to 3 hours in 79.3% of patients. 5. The results of treatment were excellent in 5 patients (10.6%), very good in 5 patients (10.6%), good in 7 patients (14.9%), fair in 4 patients (8.5%), poor in 1 patient (2.2%), when the intensity of pain before treatment was severe; but fair in 13 patients (27.7%), poor in 11 patients (23.3%), when the pain was moderate. Thus the clinical results demonstrated that this medication was remarkably effective in patients with severe pain and much less so in patients with only inoderate pain. 6. Side effects were mild transient drowsiness in 3 patients (6.3%) and gastric colic in 2 patients (4.3%). In patients with drowsiness was continuously administered. The gastric colic subsided when the medication was combined with antacid.

5.A Case of Primary Cutaneous Leiomyosarcoma.

Primary leiomyosarcomas are extremely rare. Its clinical configuration is a round to oval, more or less hemispherical elevation of the skin, but the appearance of these lesion is not distinctive so that the diagnosis is established by the histopathologic findings. Microscopically the cutaneous leiornyosarcomas consist of a poorly delineated proliferation of spindle shaped atypical myomatous cells arranged in interlacing fascicles which merge into collagenous stroma. We experienced a case of primary cutaneous leiornyosarcoma which developed as a hen egg sized tumor on the left shin in a 71-year-old woman and present this case with the review of literatures.

6.Experimental Study on the pathogenesis of Candida albicans in Swiss mice.

In search for animal model of systemic candidiasis, the authors attempted to infect the mice with Candida albicans (CUMD 23 and CUMD 25) isolated from human subjeets and study the mode of infection and pathogenesis. Swiss mice were inoculated with these organisrns either intravenously or intraperitoneally. After deterrnining the lethal doses of the pathogens at 30th day, another group of mice were inoculated with a sublethal dose. They were sacrificed weekly up to 4 weeks and the organs were examined mycologically and histopathologically. The results show that strain CUMD 23 is more virulent than CUMD 25. The most susceptible organ to C. albicans is the kidney followed by the peritoneum, mesentery and the pancreas. In the early lesion, C. atbicans generally show the spore form in the peritoneum, mesentery and the kidney when injected intraperitonea,lly but as time went by it rather took the hyphal form especially in the kidney. In the case of intravenous injection, the hyphal masses were seen in the kidney more quickly. With this result, it is suggested that the dissemination of C. albicans occurs easily in the latter and the spores reaching the kidney transform into hyphae which are not easily subjected to phagocytosis.

7.Correlation between Tumor Angiogenesis (Microvessel Density), Metastasis and Tumor Cell Proliferation in Colorectal Carcinomas.

Tumor angiogenesis has been shown to be associated with metastatic potentials in breast, lung and prostatic carcinomas. The relation between tumor angiogenesis and metastatic potentials in colorectal cancer has not been established to date. We analysed 66 selected patients with colorectal carcinomas (37 with and 29 without nodal metastases) for the microvessel density, tumor proliferation activity, and the clinicopathologic parameters including size, stage, histologic grade, growth pattern, presence of angioinvasion, perineural invasion and lymph node metastasis. For evaluation of microvessel density and tumor proliferative activity, the primary tumors were immunohistochemically stained for CD31 and PCNA. The mean microvessel counts (MVC) per 200X field were 99.27+/-23.28 and 131.35+/-31.48 in node-negative and node-positive patients, respectively. The PCNA index was 39.41+/-5.63% and 56.60+/-7.09% in node-negative and node-positive patients, respectively. MVC and PCNA index were higher in tumors with nodal metastasis (p=0.002, p<0.001), and also correlated each other (sr=0.33, p=0.007). Higher microvessel counts were seen in tumors with advanced stage (p=0.016). Tumor proliferation activity assessed by PCNA immunostaining was significantly higher in tumors with advanced stage, perineural invasion, angioinvasion, poor differentiation and larger size. From these results, MVC and PCNA index in colorectal carcinomas are assumed to be valuable prognostic parameters. Thus assessment of tumor angiogenesis and tumor cell proliferation in colorectal carcinomas may be helpful for the patients in need of aggressive therapy.
