2.Comparison of Clinical Outcomes of Argon Laser Versus Selective Laser Trabeculoplasty in POAG.

PURPOSE: To compare the clinical outcomes between patients undergoing argon laser trabeculoplasty (ALT) and those undergoing selective laser trabeculoplasty (SLT). METHODS: Sixty-one eyes of 61 patients with primary open-angle glaucoma who were treated with more than 1 kind of antiglaucoma medication eye drop were enrolled in this study. Thirty-five eyes of 35 patients were treated with argon laser trabeculoplasty (ALT). Twenty-six eyes of 26 patients were treated with selective laser trabeculoplasty (SLT). The mean IOP at 1, 3, 6, and 12 months and the success rate of treatment at the 12-month follow-up period were evaluated in all patients. RESULTS: At 12 months postoperative, the ALT group had a mean IOP of 16.53+/-2.58 mmHg with a mean IOP reduction of 35.27%, while the SLT group had a mean IOP of 18.40+/-5.61 mmHg with a mean IOP reduction of 17.37%. The success rates of treatment defined as definition 1 were 62.85% in ALT and 62.50% in SLT, and the success rates defined as definition 2 were 51.42% in ALT and 46.15% in SLT. CONCLUSIONS: There were no significant differences in the IOP lowering effect or success rates between the ALT and SLT patients at 12 months postoperatively.

5.A Case of Idiopathic Hypereosinophilic Syndrome.

The idiopathic hypereosinophilic syndrome (HES) represents a heterogenous group of disorder characterized by prolonged eosinophilia of undetectable cause and multiorgan system dysfunction. Bone marrow is the most frequentry involved organ, but the most severe clinicopathologic involvement is heart. The major cause of death in patients with the HES is cardiac dysfunction especially congestive heart failure resulted from endocardial fibrosis and restrictive cardiomyopathy. We have experienced a case of DES with both cardiac and pulmonary involvements. The patient was an 18-month-old infant with poorly controlled lung abscess who complained of fever and productive cough. On the microscopic examination of pleural effusion, eosinophilic infiltration was noted. M-mode echocardiogram showed diffuse thickening of interventricular septum and left ventricular posterior wall. On the peripheral blood smear, prolonged eosinophilia was observed. We confirmed this case with bone marrow aspiration which showed eosinophilic hyperplasia.

7.Length determination of long bone by CT scanogram

The CT digital radiography is a new accurate technique for measuring the long bone length. Authors performed acomparative study on accuracy, time and film consumption and radiation dose between conventional spot scanogramand CT scanogram. We used two femur and two tibia specimen for materials, the Somatom II(Siemens) for CTscanogram, and Rad Check(Voctoreen) for radiation dosimetery. There was no significant difference in accuracybetween direct measurement, conventional scanogram and CT scanogram(lesser than 1% difference). The examinationtime of conventional scanogram was about 35 minutes, CT was 15 minutes, and the film consumption of spot scanogramwas 3 of 14x17'' size, but the CT scanogram need only one 8x10'' film for completion of study. The radiation doseof hip, knee and ankle joint were 220 mRad, 365 mRad, respectively, in spot scanogram, but it was lesser than 5mRad in all joints in CT scanogram. The advantages of the CT scanogram are simple, rapid and reduced radiation.

8.The Effects of Pulsed Lavage on the Penetration of Cement into Metaphyseal Cancellous Bone.

PURPOSE: To compare the effects of pulsed lavage versus bulb syringe irrigation on cement penetration into metaphyseal cancellous bone. MATERIALS AND METHODS: Bone slabs, about 1 cm in thickness, taken from the distal metaphysis of 10 femurs of adult pigs were used. Slaps were sectioned in the sagittal and transverse planes, and divided into two groups. One half of the specimens was irrigated with saline by pulsed lavage, while the other half was irrigated with saline using bulb syringes. Bone cement was pressurized into the cut surface using a cement gun. Trimmed bone blocks were fixed in formaldehyde, dehydrated in ethanol, and embedded in plastic resin (Spurr's low-viscosity medium). Contiguous sectioning was done and the sections surfaces were ground. The depth of cement penetration, and the total area of cement-bone interdigitation were analyzed using image analysis software. RESULTS: The average total area of cement-bone interdigitation was 34.9 mm2 in the pulsed lavage group and 20.2 mm2 in the bulb syringe group. The average penetration depth of cement was 4.4 mm in the pulsed lavage group, and 2.5 mm in the bulb syringe group. CONCLUSION: The penetration of cement during arthroplasty is significantly increased by pulsed lavage, which could reduce late complications and aseptic loosening.

9.Zona Hardening of Mouse Oocytes Undergone Meiotic Resumption In Vivo.

It is well known that the bona pellucidae of mouse oocytes become 'hardened' when they are allowed to mature in vitro in the absence of serum components. To see if oocytes already undergone meiotic resumption in vivo exhibit similar zona hardening, hardening of ZP of cumulus-enclosed oocytes(CEOs) was examined after culture in vitro since their release from follicles various hours after hCG injection. When CEOs matured in vivo for 3h or longer were subjected to culture in vitro for 14h with BSA alone, zona hardening was significantly reduced compared to those cultured in vitro from the begining of maturation. However, when CEOs matured in vivo for 5h were freed from cumulus cells and then cultured in vitro with BSA alone, little reduction of zona hardening was observed. Preincubation of CEOs for 5h with fetuin, one of the well known inhibitor of in vitro zone hardening, did not prevent bona hardening during its subsequent culture of CEOs for 14h without fetuin. However, when CEOs precultured with both fetuin and PMSG for 5h and then further cultured with BSA alone for 14h, zona hardening was dramatically reduced. Under these conditions, the expansion of cumulus cell was observed. In addition, CEOs cultured with both BSA and dbcAMP to prevent their meiotic resumption showed a significant increase of zona hardening. Whether the observed zona hardening was correlated with the conversion of ZP2 to ZP2f was examined. Zona pellucida, isolated from CEOs matured for 5h in vivo and then further cultured with BSA alone was subjected to SDS-PAGE. Most of ZP2 molecules from these CEOs did not undergo conversion from ZP2 to ZP2f. From these results, it is concluded that CEOs undergone meiotic resumption in vivo do not exhibit bona hardening when they were subsequently cultured in vitro without serum components. It appears that cumulus cells play an important role in this phenomenon.

10.Dysgenesis of Corpus Callosum' CT and MR Findings.

PURPOSE: To evaluate the specif!c radiologic findings of the dysgenesis of corpus callosum(CCD) on CT and/or MR images. MATERIALS AND METHODS: The authors reviewed retrospectively CT and/or MR images of 10 patients with CCD. After classifying CCD into partial and total type according to the degree of its development, we investigated structural abnormalities on CT and/or MR images resulting from or related to CCD and then associated anomaly. RESULTS: The most common findings of CCD on axial planes of CT and/or MR images were dilatation of posterior part of the lateral ventricles(8/10) and feature of the bodies of the both lateral ventricles(8/10) in parallel. Sagittal and coronal planes of MR demonstrated exact extent of dysgenesis of the corpus callosum(5/5) and radiating pattern of gyri in medial surface of cerebral hemisphere extending to the 3rd ventricle without interruption by cingulate sulcus(4/5). CONCLUSION:MR image of mid-sagittal plane appeared most useful in diagnosis of CCD because of its direct depiction of the whole length of corpus callosum. In most cases, however, CCD can be easily diagnosed with CT scan by characteristic findings of dilatation of the posterior part of lateral ventricles and paralleling of the lateral ventricles.
