1.An Analysis of Deaths Caused by Cancer in Metropolitan Areas of Korea: Seoul, Pusan, Taegu, Incheon.

For the purpose of preparing the basic data for further cancer epidemiologic study and cancer patients control, we conducted the analysis on the degree and structure of deaths from cancer in metropolitan areas of Korea with 7,934 certified cancer deaths records of Seoul, Pusan, Taegu and Incheon in 1982. The analyzed results were as follows : 1) The total number of cancer deaths in metropolitan areas were 7,934 (male: 4,749, female: 3,185) as 14.1% of deaths from all causes in the same area. 2) The rate of physician's certification on cancer deaths was 77.4% and most of cancer deaths (84.4%) occurred at their home. 3) Cancer specific death rate was 51.7 per 100,000 population (male: 62.9, female: 41.9) and age-adjusted cancer death rate was 82.4 in male and 51.6 in female per 100,000 population. And the difference was statistically significant (p<0.01). 4) Age-specific cancer death rate was generally increased with age and most of cancer deaths (male: 75%, female: 65%) occurred from 45 to 74 years old. 5) The first three orders of cancer site were stomach (32.7%), liver (28.8%), lung (11.7%) in male and stomach (30.6%), uterus (18.4%), liver (13.8%) in female. And the relative frequency of these three cancer sites among total cancer deaths was corresponded to 73.2% in male and 62.8% in female. 6) The ratio of male to female cancer death rate was 1.5:1. And the ratio was above 3.0 in esophagus, liver, larynx, bladder cancer and the ratio was similar to 1.0 in stomach, pancreas, leukemia, brain, colon cancer, but the ratio was reversed in gall bladder and bile duct, and thyroid cancer.

2.Surgical Treatment of the Distal Radius Fracture

Fractures of the distal radius are one of the most common injuries met in the orthopedic field. Once it was thought that good function comes despite of poor anatomic restoration in the distal radius fractures. But now there are wide consensus that more aggressive surgical treatments are needed in such complex fractures to fulfill the better clinical results. We reviewed 56 consecutive distal radius fractures of 54 patients treated surgically in Orthopedic Department of Asan Medical Center between October 1989 and July 1994. The mean length of follow-up was 29.2 months. Clinical end results were statistically analyzed according to each anatomi- cal variables and state of distal ulna, and anatomies and results were analyzed according to different surgical modalities in each fracture classes. Radial length and radial inclination was significantly correlated with clinical results. The clinical results were significantly worse when dorsal tilt exceeded 10. In intraarticular type of fractures, residual articular incongruency seems to be the significantly affecting factor of development of arthritis and the clinical result. The associated fracture of distal end of the ulna did not significantly affect the result. In same type of fracture class, achievement and loss of reduction, arthritis, and Demerit score show no significant differences between groups of different surgical modalities. Probably this is due to proper choice of modalities in each different situations.

3.Drugs Most Frequently used in OPD of Yeungnam University Hospital: March to August, 1985.

This report offers descriptive data about the drugs utilized in outpatient department (OPD) of Yeungnam University Hospital (YUH) in the period of March to August in 1985. The data in this report were produced by the computerized totalization of the number of mentions of individual drugs included in the prescriptions. The 100 drug entries that were most frequently recorded are listed in rank order. The listing is arbiturarily restricted to the drugs that were prescribed as single preparations, the drugs of basis of compound preparations and the drugs of adjuvant or corrective of compound preparations that have significant therapeutic effects either by generic names. And in addition, the listing also involves the compound preparations used in relatively large frequency, and the individual components of which have the unique pharmacological actions each other by proprietary names. And all routes of administrations were allowed. The 10 drugs most frequently named are diazepam, aluminum compounds, acetaminophen, isoniazid, metoclopramide, polaramine®, carboxymethylcystein, ephedrine, codeine and caroverine in order. The 521, 855 drug mentions listed as above are described by the chief therapeutic usage that each is intended to apply generally. The drugs which account the largest proportion of total mentions were those acting on the central nervous system (20.57%), including tranquillizers and sedative hypnotics (11.71%), analgesic antipyretics (5.55%), antidepressants (2.15%) etc. Gastrointestinal drugs and smooth muscle preparations (18.64%) included antacids and antiulcer drugs (9.24%), antiemetics (3.57%), spasmolytics (3.14%) and others. Respiratory drugs (16.11%) included expectorants and cough preparations (10.99%) and bronchodilators (5.12%). Chemotherapeutic agents (15.12%) included the antiTbc drugs (7.09%) most frequently, and the penicillins (3.33%) accounted the largest proportion among the antibiotics. Cardiovascular drugs (5.64%) included cardiac drugs and coronary vasodilator (4.12%) and antihypertensives and vasodilators (1.06%). And antiinflammatory drugs (4.33%), vitamins of single preparations (3.76%), hormones and their antagonists (3.29%), common cold preparations (3.12%), diuretics (2.81%), drugs supporting liver function (2.02%), drugs affecting autonomic nervous system (1.89%) including antiglaucomas, atropine and cerebral vasodilators, antihistamine drug (1.02%) and disinfectants (0.74%) were following in order. The data in the report were compared to those reported by H. Koch, et al. in United States (US), 1981 as “Drugs Most Frequently Used in Office Practice : National Ambulatory Medical Case Survey, 1981.” Cardiovascular drugs prescribed in YUH were much less in proportion than in US (10.56%), but gastrointestinal drugs accounted the larger proportion than in US (3.72%). Expectorants and preparations in YUH also accounted the larger proportion than in US (2.74%). In conclusion, in the period of March to August, 1985, OPD of YUH prescribed the CNS drugs including diazepam most frequently, and gastrointestinal, respiratory and chemotherapeutic drugs in next orders. It is supposed that the eating habits of Koreans and a unique atmospheric condition in Taegu as a basin were some important factors that affected the proportions of drugs acting on gastrointestinal and respiratory tracts.

4.Effect of Imipramine on the Contractility of Single Cells Isolated for Canine Detrusor.

The objective of this study was to establish a good methodology to isolate single smooth muscle cells that are alive and respond properly to pharmacological agents. Canine urinary bladders were employed as the source of single cells, and acetylcholine, atropine and imipramine were used as indicators of pharmacological responsiveness. Imipramine, an antidepressant drug exhibited the anticholinergic and calcium antagonizing properties on rat detrusor muscle. To establish a control value for a further experiment to elucidate the mechanism of action of imipramine on detrusor muscle, we measured the concentration-response of single cells to acetylcholine in the presesnce of imipramine by length of the cells and compared the result with the response in the presence of atropine. Tiny chops of smooth muscle taken from anesthetized canine urinary bladder were incubated in collagenase solution at 36℃ for 17-20 minutes. The collagenase solution included collagenase 1.2 mg/ml, soybean tryspin inhibitory 0.08 mg/ml, bovine serum albumin 2% in 10 ml Krebs-Henseleit buffer solution aerated with a consistent breeze of 95/5% O2/CO2 to maintain the pH at 7.4. After washing with plain K-H solution on 450 mesh, cells were dissociated from the digested tissue for 12-15 minutes. Cell suspension was transfered in 5 ml test tubes and acetylcholine was added for the final concentration to be 10⁻¹⁴~10⁻⁹M. To find the optimal time to fix the cells to determine the contractile responses, 1% acrolein was added 5, 10, 20, 30, 60 and 120 seconds after the administration of ACh. The length of cells fixed by acrolein were measured by microscaler vis CCTV camers on phaes-contrast microscope. The average length of 50 cells from a slide glass was taken as the value of a sample at the very concentration point. Single cells were isolated from canine detrusor. The length of untreated cells varied from 82 µm to 94 µm. The maximal response to actylcholine 10E-9M was accomplished within 5 seconds of exposure, and the shortening was 19±3%. Atropine reduced the contraction of the cells concentration-dependently. Lmipramine which exerts a cholinergic blocking action on some smooth muscles also reduced the contraction concentration-dependently and by a similar pattern as atropine. These findings document that imipramine may exerts a cholinergic blocking activity in the single smooth muscle cells isolated from canine urinary bladder.

5.Measurements of Thyroid Stimulation Blocking Antibody Activities by Chinese Hamster Ovary ( CHO ) cells Expressing Human TSH Receptors in Patients with Primary Hypothyroidism.

BACKGROUND: The Chinese hamster ovary cells transfected with human TSH receptor cDNA (hTSHR-CHO), expressing functional human TSH receptors, are known to be more sensitive in detection of thyroid stimulating antibodies than FRTL-5 cells. There has been no report on the usefulness of these cells to measure thyroid stimulation blocking antibody (TSBAb) activity which is frequently found in patients with primary myxedema, METHODS: We established the optimal assay condition of measurement of TSBAb using hTSHR-CHO cells, and simultaneously measured TSBAb activities with FRTL-5 cells and with hTSHR-CHO cells in 49 patients with primary myxedema, compared them with their thyrotropin binding inhibitor immunoglobulin (TBII) activities. RESULTS: 1) hTSHR-CHO cells specifically bound bTSH and were stimulated by bTSH in terms of cyclic AMP generation in a dose dependent manner. 2) Myxedema IgG suppressed TSH-stimulated cAMP production of hTSHR-CHO cells in a dose dependent manner reaching plateau at the concentration of I g/L. Normal pooled IgG has no suppressive action at the concentration of less than 1 g/L, but caused significant suppression at the concentration of greater than 1g/L. 3) TSBAb activities measured by hTSHR-CHO cells in 49 patients with primary myxedema were as follows: Four of 25 TBII-negative cases (16%) and 22 of 24 TBII-positive cases (92%) had TSBAb activities. Most of TSBAb positive patients (95%), especially in TBII positive cases, showed very high activities of more than 90%. 4) TSBAb activities measured by hTSHR-CHO cells and those by FRTL-5 cells were both positive in 24 patients (49%), both negative in 18 patients (37%), and were discrepant in 7 patients (14%). The TSBAb activities measured with hTSHR-CHO cells and those measured with FRTL-5 cells were significantly correlated (r=0.71, p< 0.01). 5) Forty five percent of patients with primary myxedema had all of 3 kinds of activities (TBII, hTSHR-CHO cell TSBAb, FRTL-5 cell TSBAb), 37% of them had none of 3 activities and 18% of them had 1 or 2 kinds of activities only. CONCLUSION: The usefulness of hTSHR-CHO cells in measurements of TSBAb activities were confirmed. The TSBAb activities of most patients with primary myxedema measured by hTSHR-CHO cells were concordant with those measured by FRTL-5 cells. However, a small subset of patients (18%) had discrepant results in assays of TSH receptor antibodies according to the differences in TSH receptors (rat, human and porcine) used in assay. Such discrepancy may be explained by heterogeneity in epitopes for blocking TSH receptor antibodies.

6.Periosteal chondroma arising in the Phalangeal Bone: Report of Three Cases

Periosteal chondroma is a slowly growing cartilage tumor of limited size, which develops within and beneath the periosteal connective tissue and characteristically erodes and induces sclerosis of the contiguous cortical bone. The typical morphology of this tumor consists of an area of outer cortex scalloping involving the metaphyseal lesion of a long bone with slightly overhanging edges and a small amount of cartilage calcified matrix adjacent to the scalloping. Histologically this benign tumor can mimic chondrosarcoma. Awareness of this fact is important in order to avoid overdiagnosis and resultant overtreatment of the benign lesion. We report three cases of periosteal chondromas arising in the phalangeal bones of the hand. All were treated by curettage. No local recurrence was seen following this treatment.

7.Effect of GABA on the contratility of small intestine isolated from rat.

This study was designed to investigate the effect of GABA and related substances on the spontaneous contraction of rat small intestine. The rats (Sprague-Dawley), weighing 200-250g, were sacrificed by cervical dislocation, and the small intestine was isolated. Longitudinal muscle strips from duodenum, jejunum and ileum were suspended in Biancani's isolated muscle chambers and myographied isometrically. GABA and muscimol, a GABA A receptor agonist relaxed the duodenum and jejunum significantly, but baclofen-induced relaxation in those muscle strips negligible. The effectiveness of GABA and muscimol in various regions were the greatest on duodenum, and greater on jejunum than on ileum The effect of GABA and muscimol was antagonized by bicuculline, a competitive GABA A receptor antagonist and picrotoxin, a noncompetitive GABA A receptor antagonist. Duodenal relaxation induced by GABA and muscimol was unaffected by hexamethonium, but was prevented by tetrodotoxin. These results suggest that GABA inhibit the contractility of smooth muscle with distinct regional difference of efficacy, and the site of inhibitory action is the GABA A receptor existing at the presynaptic membrane of postganglionic excitatory nerves.

8.The levels of blood lead and zinc protoporphyrin for healthy urban population in Korea.

Hemoglobin(Hb), zinc protoporphyrin(ZPP) and blood lead(PbB) levels were determined for 1,851 blood samples collected from healthy urban population to establish reliable baselines for Hb, ZPP and PbB levels by age and sex. ZPP values were analyzed with a Hmatofluorometer and PbB determinations were concurrently carried out using flameless atomic absorption spectrophotometry. The blood sampling period was about 6 months from May, 1991 and the summarized results were as follows; 1. The mean value of Hb in male female were 14.55+/-1.81 g/dl and 12.61+/-1.81 g/dl respectively and there was statistically significant difference(p<0.05). 2. The mean value of ZPP in pre-schoolchildren was 37.49+/-13.31 microgram/dl for male, 35.77+/-11.85 microgram/dl for female and that of ZPP in after 7 years groups was 31.91+/-8.23 microgram/dl for male, 30.11+/-9.11 microgram/dl for female and there was statistically significant difference(p<0.05). 3. The mean value of PbB in pre-schoolchildren was 25.10+/-5.21 microgram/dl for male, 24.45+/-4.18 microgram/dl for female and that of PbB in after 7 years groups was 24.28+/-3.00 microgram/dl for male, 21.99+/-5.05 microgram/dl for female and there was statistically significant difference(p<0.05).

10.Effect of Carbamazepine on the Ouabain-Induced Arrhythmia in Rabbits.

Carbamazepine is a derivative of iminostilbene with carbamoyl group and related chemically to the tricyclic antidepressants. Carbamazepine has been introduced for treatment of trigeminal neuralgia. Recently it is used as an antiepileptic agent such as diphenylhydantoin. Antiepileptic drugs are known to affect experimentally induced cardiac arrhythmia and are now widely used clinically for treatment of ventricular tachyarrhythmias, particularly those produced by digitalis intoxication. Steiner et al. (1970) reported that carbamazepine was found to be very effective in converting ventricular tachycardia due to digitalis toxicity to normal sinus rhythm. Clinically bradycardia, complete heart block, ventricular standstill and Adams-stokes attack were reported in the course of carbamazepine treatment. The purpose of this study was to investigate the effects of carbamazepine on the ouabain-induced arrhythmia in vivo. The rabbits of either sex, weighing from 1.6 to 3.2 kg were anesthetized by urethane. After the trachea was cannulated, the rabbits were ventilated with room air using a respirator. Drugs were given into polyethylene cannula in the femoral vein. Blood pressure were recorded by physiograph via pressure tranducer connected with the cannula in the femoral artery. EKG were recorded by physiograph via electrode implanted in both fore leg and left hind leg. The results are summarized as follows 1. Arrhythmia was induced by continuous infusion of ouabain (65±8.8 µg/kg). 2. Single administration of ouabain (64 µg/kg) induced arrhythmia which was persisted for 7-8 min. 3. Ouabain induced arrhythmia was restored to normal sinus rhythm by administration of carbamazepine (the more dosage, the less frequent and the longer duration). 4. Severe bradycardia, A-V block, atrial fibrillation were seen on the EKG after injection of carbamzepine alone. By the above results, it may be concluded that carbamzepine inhibits the ouabain-induced arrhythmia by dose-dependent.
