PURPOSE: This study was designed to verify intermediate-term functional and radiological results of limb salvage operation using endoprosthetic replacement system (MUTARS(R)) used in patients with a malignant bone tumor. MATERIALS AND METHODS: Thirty one cases which used MUTARS(R) tumor prosthesis were reviewed. The mean age of the patients was 49.2 years and the mean follow up was 39.8 months. We retrospectively reviewed complications, and evaluated functionally and radiologically by Enneking functional score, ISOLS radiological implants evaluation system at last follow-up. RESULTS: 3 patients had died of disease, distant metastasis was seen in 4 patients and local recurrence was seen in 1 patients. Complications were developed in 12 patients. (infection 6, leg length discrepancy 2, aseptic loosening 2, periprosthetic femoral fracture 1, screw loosening 1) Mean value of total functional scores were 81.2% in proximal femur, 77.4% in distal femur, 78.1% in proximal tibia, and 80.2% in proximal humerus. The overall radiological result was relatively satisfactory. CONCLUSION: Our results suggest limb salvage with the MUTARS(R) endoprosthesis is successful with good functional and radiological results. But we should be careful with complications such as infection.

The "Dandy-Walker syndrome" has been the source of much controversy not only as to its embryogenesis but also concerning the correct diagnosis and surgical management of this disorder. The original description by Dandy and Blackfan emphasized the association of hydrocephalus, posterior fossa cyst and maldevelopment of the cerebellar vermis and later, Taggart and Walker described the genesis of these anomalities to atresia of the foramina of Magendie and Luschka. The Neuroradiologic findings associated with the presence of a Dandy-Walker cyst are pathognomonic. These findings relating to the cystic dilatation of the fourth ventricle which accompanies atresia and its outlets, gave a detailed clinicoanatomical information about the hydrocephalus in the CT scan. The CT scan contributed to the new advanced fields in the diagnosis and surgical management of hydrocephalus combining with the posterior fossa, lesion, because it was easily able to detect the lesion site stereoscopically and also find out the relationship to the involved surrounding area comparing with each of the slice in the CT scan. The purpose of this paper is to attempt to clarify the differentiation of the Dandy-Walker cyst variant from the Dancy-Walker cyst and anatomically detect the damaged brain correlated with the ventricular dilatation on the CT scan. A 7 year-old boy was admitted to the Department of Neurosurgery, Catholic Medical Center on Dec, 22, 1977 with one month history of progressive headache, nausea, vomiting and gait disturbance. On his examination he showed moderate enlargement of head, papilledema, diplopia, horizontal nystagmus, and gait disturbance. We confirmed the diagnosis under the Dandy Walker Cyst Variant by conray ventriclography and CT scan. He underwent lateral ventriculo-peritoneal shunt and then his symptoms progressively improved. He was discharged to his home after the 14th day of hospitalization.

This study was carried out to determine the accuracy of Doppler echocardiography for predicting the pulmonary arterial pressure from right ventricular systolic time intervals in 52 patients with ventricular septal defect. The diagnosis of ventricular septal defect was made by cardiac catheterization and angiocardiography at Dong San hospital, Keimyung University during the period of one year from jan. 1988 to Dec. 1988. Doppler measurements of acceleration time (AT), right ventricular ejection time (RVET), right ventricular preejection period (RPEP), AT/RVET, and RPEP/AT were compared with pulmonary arterial pressure (PAP), measured by cardiac catheterization. The patients were divided into 3 groups : PAP< or =30mm Hg, PAP 31-59mm Hg, PAP??0mm Hg. The following results were obtained. 1) In the groups of PAP< or =30mm Hg, AT was 0.12+/-0.01sec, AT/RVET was 0.47+/-0.07 and RPEP/AT was 0.50+/-0.05. 2) In the groups of PAP> or =60mm HG, AT was 0.06+/-0.01sec. AT/RVET was 0.28+/-0.05. RPEP/AR was 1.51+/-0.21. As the level of PAP increased, Doppler AT, AT/RVET and RPEP/AT showed significant change(P<0.001). 3) The Doppler AT showed relative high correlation(r=-0.76) with PAP measured by cardiac catheterization in all group. 4) The Doppler AT/RVET showed correlation(r=-0.70) with PAP. 5) The Doppler RPEP/AT showed high correlation(r=0.91) with PAP. The Doppler echocardiography was easy to apply in all age groups, and was found useful for detecting pulmonary hypertension in ventricular septal defect and for the follow-up check of the patients. It may help to determine the optimal time for surgery and evaluation of the treatment.

Wegener's granulomatosis is a disease of unknown etiology that is characterized by the clinicopathologic complex of necrotixing granulomatous vasculitis of the upper and lower respiratory tract, glomerulonephritis, and variable degrees of small vessel vasculitis. Recently Antineutrophil Cytoplasmic Antibody (ANCA) has been reported to be a highly specific test for the diagnosis of Wegener's granulomatosis. We have experienced a patient of Wegener's granulomatosis in a 11 year old girl who was admitted with complaints f arthralgia, hematuria, convulsion and associated with otitis media and sinusitis. Serologic test of C-ANCA was positive and histologic findings of the kidney showed crescentic glomerulonephritis with sclerosis and surrounding infiltration of multinucleated giant cells. Patient was treated with pulse methylprednisolone without improvement. The clinical course progressed rapidly and expired due to the renal failure, gastrointestinal bleeding and status epilepticus. A brief review of literatures was made.

The clinicolaboratory findings, cardiac catheterization, and outcome of operation were analyzed in 78 patients with tetralogy of Fallot, which were diagnosed by echocardiography, cardiac catheterization, and a cardiac angiography and confirmed by operation at Dong San Hospital, Keimyung University, during a 4(1/2)-year period from January 1984 to June 1988. The following results were obtained : 1) Out of the 78 cases, 45 were male and 64 were female. 2) The electrocardiogram showed right axis deviation in 71 cases(91%) and right ventricular hypertrophy in 66 cases(84.6%). 3) Chest X-ray revealed that, the cardiothoracic ratio was normal or decreased in 68 cases(87.3%), and cardiac apex elevation was noticed in 57 cases(73%). 4) Pulmonary stenosis were chiefly valvular and infundibular type(53.8%). 5) The associated heart diseases with TOF were patent foramen ovale(64.1%), right sided aortic arch(19.2%), and secundum ASD(10.3%), in that order. 6) There was an intimate correlation between secondary polycythemia and thrombocytopenia. 7) Among the 78 Cases, 73 cases had total correction, 1 case had shunt operation, and 4cases had total correction after shunt operation. The highest mortality rate occurred in the cases of total correction after shunt operation(25%). The cases with a main pulmonary artery size of 1/3 to 2/3 against, the aorta had a higher mortality than the other group, and no cases expired in the group with a ratio of more than 2/3. The overall surgical mortality was 8.9%.

Collateral circulation that develops in the chronic stage of branch retinal vein occlusion (BRVO) was known to affect the cliniral course of this disease. To study the development of collateral circulation according to the types of BRVO, 45 eyes with BRVO in which fluorescein angiographies(FAG) were taken more than two times were divided into 3 groups by the combination of the area of capillary non perfusion and the amount of cotton wool patches. Group I showed a small area of nonperfusion, group II, a wide-spread nonperfusion area with no or several cotton wool patches and group III, a wide-spread nonperfusion area with predominant cotton wool patches. The collateral and the venovenous anastomosis was further subdivided into high-low anastomosis, bridging vessel and recanalization. Then we compared the incidence of collateral vessels in each group. As a result, in group II and III, higher incidence of arteriovenous anastomosis, arteriovenous and venovenous anastomosis was recognized compared to group I. In the cases of major temporal vein occlusion, higher incidence of arteriovenous anastomosis compared to the cases of macular branch vein occlusion. And in group III, higher incidence of neovascularization was found compared to group I.
