2.The Result of Anterior Interbody Fusion on Low Back Problem

Low back pain and sciatica is one of the most frequent and troublesome problem in the orthopedic field. A study of the effectivity and reliability of the anterior interbody fusion with autoiliac bone graft for the spondylogenic and Discogenic back pain was done on 166 patients who had been hospitalized from January 1978 to December 1984. The approach to the lesion was retroperitoneal with anterior approach. The anterior interbody fusion was performed by using one block or two block from autoiliac bone. The status of the spine fusion after operation was decided by the plane roentgenogram or clinical sign, and the tomogram or dynamogram in some case. We analysed 116 patients having anterior fusionto study whether fusion rate has any relation with the number of the grafted bone, preoperative diagnosis and the level of fusion. At 9 months after operation, the clinical result was assesed and was compared between the group of the preoperative diagnosis. The following results were obtained from analysis of the cases studied. 1. The fusion rate for the grafted bone was 92.2%. There was no difference in fusion rates between the group of patients operated using one block and two-block. 2. The fate of the grafted bone could be categorized into 5 different types. 3. In patients with spondylolisthesis or spondylolysis, there was high incidence of non union.Also in terms of the vertebral level, the fusin between the L5-Sl showed high incidence of non union. 4. High fusion rate was obtained by using back brace without cast immobilization. The average duration of brace wearing was 6.6 months. 5. Overall clinical results were as follows:excellent or good 64.6%, fair 31.9%, poor 3.5%,and spondylolisthesis showed better clinical results than other diseases. 6. Post sympathectomy symptoms were noticed in 12 patients after operation and there were no permanent genitourinary problem.

4.Benign Neonatal Hemagiomatosis with Unusually Persistent Lesions and Conjunctival Hemangioma.

Most neonatal cutaneous hemangiomatosis are complicated by visceral involvement and. are associated with a high mortality rate in the first month of life. Some neonates with multiple cutaneous hemangiomas, however, may follow a benign course of spontaneous resolution without visceral involvement. Such cases are called benign neonatal hemangiomatosis (BNH). BNH is characterized by a lack of mucosal or symptomatic visceral involvement. rapid spontaneous regression of cutaneous hemangimas, and an excellent prognosis. We report herein a case of BNH which is atypical compared to previously reported BNFI cases. Our patient had a more prolonged course than those of usual cases with BNH and showed a conjunctival involvement.

5.MR Imaging of a IVlature Teratoma in Third Ventricle: Case Report.

Teratoma is very rarely developed in the third ventricle. We report a case of third ventricular mature teraroma in 12 year old boy with headache and precocious puberty. In TlWl and Gd-DTPA enhanced TlWl, the mass in the third ventricle showed mixed signal intensities with signal void and partial contrast enhancement. The tumor was confirmed as a mature teratoma including teeth and fatty tissue.

6.MR measurement of normal pituitary gland height on midsagittal section: age and sex differentiation.

Measurement of the normal pituitary gland height was performed on magnetic resonance (MR) images of 144 subjects with no known or suspected pituitary or hypothalamic diseases. Midsagittal T1 weighted images (T1WI) were used for measurement, and mean vertical height according to age and sex group was obtained. In all age groups, the pituitary height was greater in females than in meles, The group of 0-9 years in both genders showed the minimum mean pituitary height, The maximum mean height was observed in the 10-19 years age group in both genders. The height gradually decreased with increasing age after age 20 years. There was no subject with a height more than 9.0 mm in females or 8.0 mm in males. In conclusion the measurement of the normal pituitary pland height using mid sagittal MR imaging can be used for the evaluation of the pituitary gland lesions.

8.Allium Sativum-mediated Therapy for Experimental Prostate Cancers with In Vivo Tumor Transduction: Effects on Tumor Growth and Development.

PURPOSE: Allium sativum (AS) has been known to have widespread benefits in reducing some human cancer risk by immune stimualtion and anticarcinogenic activity. In the present study, we evaluated the preventive and antitumor properties of AS as an effective anticancer modifier for human prostate cancer in vivo. MATERIALS AND METHODS: Subcutaneous prostate cancers were established in athymic nude mice with 5x10(5) PC-3 human androgen-indenpendent prostate cancer cells. AS was injected at the site of tumor transplantation on day 1 and one week intervals up to 5 weeks (Experiment I), and into the established tumors sized by 50-60mm(3) weekly for 5 weeks (Experiment II). Therapeutic responses and efficacies of AS for prostate cancers in vivo were determined in separate controlled experiments, and definite histopathological studies were also performed. RESULTS: In vivo studies indicated statistically significant reduction in the incidence of tumor formation with programmed and continuous AS intralesional treatment. For established prostate cancer, AS treatment also demonstrated an inhibitory effect of tumor growth compared with control. Histomorphological and immunohistochemical studies demonstrated marked apoptosis after 5 weeks-AS continuous treatment in Experiment II. CONCLUSIONS: AS had a definite antitumor activity to inhibit tumorigenesis and may modulate tumor growth of prostate cancer in vivo. It is non-toxic, readily avaliable and inexpensive. AS, in the future, may be developed as a novel and effective treatment in chemoprevention for human prostate cancer.

9.A Case of Flexural Darier' s Disease.

Dariers disease is a domirantly inherited skin disease that is characterized by keratotic papules predominantly on the sebornieic areas, palmar pits, and a nail dystrophy. The diagnosis is difficult in the small group with flexural involvement because the features of flexural Dariers disease overlap clinically and histolopica.lly with those of Hailey-Hailey disease. The diagnosis of Dariers disease may be confirmed if the patient has the pathognomonic nail dystrophy. We presents a case of flexured. Dariers disease in a 65 year old woman. She has suffered from skin lesions which have some resemblance to those of Hailey-Hailey disease. We could diagnose her skin lesions as Dariers disease because she had the eharacteristic nail dystrophy.

10.A Clinical Study on the Antihypertensive Effect of Nipradilol.

beta-receptor antagonist as antihypertensive agent has adverse effect such as vasoconstriction, bronchoconstriction and aggravation of atherosclerosis. So beta-receptor antagonist with alpha-receptor blocking property has been studied for avoidance of adverse effects of beta-receptor antagonist. Nipradilol, a new beta-receptor antagonist with alpha-receptor blocking property was administrated in 30 essential hypertensive patients for 10 week in order to evaluate the antihypertensive effects and adverse effects. The results were as follows: 1) Nipradilol was effective in 20 patients(67%) on systolic blood pressure 22 patients (73%) on diastolic blood pressure at the end of 10 weeks treatment. 2)Before medication, mean systolic and diastolic blood pressure in sitting position were 160+/-14mmHg and 102+/-18mmHg,and after 10 weeks of medication, mean systolic and diastolic blood pressure were significantly declined to 143+/-22mmHg and 89+/-12mmHg(p<0.001). 3) There was no significant change in pulse rate, from 78+/-12 beats per minute the beginning of the treatment to 75+/-11 beats per minute after 10 week of treatment. 4) There was no significant changes in sodium, potassium, GOT, GPT, bilirubin, alkalin ephosphatase, BUN, creatinine, LDH, CPK and lipid. 5) The adverse effects of nipradilol were bradycardia(7%), dyspnea(3%), chest discomfort(3%) and headache(3%). In summary, nipradilol seemed to be effective antihyertensive drug in treating mild to moderate hypertension.
