2.A Clinical Study of Tetralogy of Fallot.

The clinicolaboratory findings, cardiac catheterization, and outcome of operation were analyzed in 78 patients with tetralogy of Fallot, which were diagnosed by echocardiography, cardiac catheterization, and a cardiac angiography and confirmed by operation at Dong San Hospital, Keimyung University, during a 4(1/2)-year period from January 1984 to June 1988. The following results were obtained : 1) Out of the 78 cases, 45 were male and 64 were female. 2) The electrocardiogram showed right axis deviation in 71 cases(91%) and right ventricular hypertrophy in 66 cases(84.6%). 3) Chest X-ray revealed that, the cardiothoracic ratio was normal or decreased in 68 cases(87.3%), and cardiac apex elevation was noticed in 57 cases(73%). 4) Pulmonary stenosis were chiefly valvular and infundibular type(53.8%). 5) The associated heart diseases with TOF were patent foramen ovale(64.1%), right sided aortic arch(19.2%), and secundum ASD(10.3%), in that order. 6) There was an intimate correlation between secondary polycythemia and thrombocytopenia. 7) Among the 78 Cases, 73 cases had total correction, 1 case had shunt operation, and 4cases had total correction after shunt operation. The highest mortality rate occurred in the cases of total correction after shunt operation(25%). The cases with a main pulmonary artery size of 1/3 to 2/3 against, the aorta had a higher mortality than the other group, and no cases expired in the group with a ratio of more than 2/3. The overall surgical mortality was 8.9%.

5.A Case of Pure Trigeminal Motor Neuropathy.

Trigeminal motor neuropathy is a rare disorder which involves only motor fiber in trigeminal nerve. We experienced a 58-year-old man with muscle weakness and wasting of left masticatory muscles for 3 weeks. He had suffered from the same event on the right side 13 years ago. He had no sensory dysfunction and Brain CT revealed no gross abnormality. All the laboratory findings were negative and the EMG revealed chronic denervating process in bilateral masseter and temporalis muscles. He has improved after steroid therapy.

6.Primary Total Hip Arthroplasty Using Summit® Stems in Korean: Minimum Four-year Follow-up.

PURPOSE: The aim of this study was to assess the mid-term results of primary cementless total hip arthroplasty (THA) using Summit® stems. MATERIALS AND METHODS: One hundred twenty-eight arthroplasties in 121 patients who were performed THA from December 2004 to March 2013, were reviewed retrospectively a minimum of 4 years follow-up (mean age, 60.3 years; mean postoperative follow up period, 86.3 months). For a clinical evaluation, the Harris hip score (HHS) was used to assess outcome. Radiographic evaluation was done through spot welds, subsidence, stress shielding, canal filling. RESULTS: The mean HHS was 69.8 points preoperatively, and it improved significantly to 96.3 points at the final follow-up. Postoperatively, 10 patients experienced thigh pain and 7 patients showed a moderate limping gait. There was not only early stem subsidence but there was no more subsidence at last follow up. One hundred eighteen hips showed radiographic signs of stem bone ingrowth, and 10 hips showed fibrous ingrowth at the last follow-up. Stress shielding occurred in 85 hips and third degree stress shielding was observed in 6 hips. Complication arose in 10 hips due to dislocation. Although revisions due to recurrent dislocations led to acetabular component revision in one hip, no revisions arose due to isolated aseptic loosening of the femoral component. CONCLUSION: The clinical and radiological mid-term results of primary THA using Summit® stems revealed solid initial fixation and bone ingrowth stable fixation. Considering the satisfactory clinical results, Summit® stems may be a good treatment option in Korean patients who were required hip arthroplasty

7.Evaluation of Clinical Usability and Effects of Photochromic Lenses.

PURPOSE: To evaluate stability and effects of photochromic lenses in light-sensitive patients. METHODS: Analyzing 65 patients, this research employed a standardized survey, fundus examination, and slit-lamp biomicroscopy to evaluate the changes in photophobia, tears, asthenopia, and the deterioration of outdoor visual acuity after wearing photochromic lenses for one month. Measurement of visual acuity was performed outdoors at noon on a bright day using Dr. Hahn's standard test chart for 3 m. RESULTS: Photochromic lenses, considerably decreased photophobia, tears, and asthenopia. The "normal" visual acuity of 38 patients was established without photochromic lenses. Re-examination after the lenses had been worn for a specified period of time showed that 29 subjects could discern at least one line higher than their "normal" acuity. CONCLUSIONS: Photochromic lenses reduced photophobia and the disabling effect of bright light such that comfort was increased. Not only did the lenses improve vision, they also enhanced the overall quality of vision.

8.ReVersible Change of Vision in Patients Receiving Deferoxamine.

PURPOSE: To report 2 cases of acute visual neurotoxicity in patients with paraquat poisoning receiving deferoxamine. METHODS: A 48-year-old woman and a 37-year-old man received intravenous infusion of deferoxamine at 3g/day for 7 days for treatment of paraquat poisoning. They presented with severely impaired visual acuity, generalized constriction of visual field, and decreased amplitude of visual evoked potential. They improved clinically upon drug-withdrawal. CONCLUSIONS: Deferoxamine is an effective agent for treatment of paraquat poisoning, but can cause visual neurotoxicity. We recommend careful regulation of the deferoxamine dosage and serial visual monitoring in patients receiving the drug.
