Relining and rebasing are essential for long-term success and oral health in removable prosthodontics. Major features of failures between metal base and relining resins are adhesive failure due to lack of chemical bonding. The purpose of this study was to find a better metal primer and metal surface treatment method that enhance the bonding strength with relining resin materials. The surfaces of ticonium alloys were treated with 25micrometer sandblasting (Group A), stone wheel (Group B), stone wheel and EZ oxisor (Group C), 75micrometer sandblasting (Group D) and EZ oxisor application after 75micrometer sandblasting (Group E). They were subdivided into no primer application (Group I), MR bond application (Group II) and Metafast bonding liner (Group III). Then specimens were completed though being bonded with relining resins. The specimens were stored in 38degrees C water for 48 hours and tensile strength was measured using the universal testing machine. The results were as follows, 1. Primer application groups showed higher bond strength than no primer application group(p<0.05). 2. In comparison with primer application groups, MR bond group showed higher bond strength than Meta fast bonding liner application group(p<0.05). 3. In comparison with surface treatment methods. Bond strengths of group A and B were significantly different with group C, D, and E, and group C were significantly different with group D, and E in no primer application group(p<0.05). In primer application groups, group A, B, C were significantly different with group D and E(p<0.05). According to results of this study, Metal primer application and metal surface roughening were considered to be advantageous for relining of metal base dentures.

PURPOSE: The aim of this study was to evaluate whether there is any typical deformation pattern existing in complete denture when it was dried by using the 3D scanner and surface matching program. MATERIALS AND METHODS: A total of 28 denture bases were fabricated with heat curing acrylic resin (each 14 upper and lower denture bases), and 14 denture bases (each 7 upper and lower denture bases) were stored in the water bottle (water stored), and another 14 denture bases were stored in the air (dry stored). Each specimen was scanned at 1st day after deflasking, 14th day after deflasking, and 28th day after deflasking, and digitalized. Three dimensional deformation patterns were acquired by comparison of the data within storage group using surface matching program. For evaluating differences between groups, these data were compared statisticallyusing Kruskal Wallis and Mann Whitney-U test (α=.05). RESULTS: When evaluating 3D deformation of denture base, obvious deformations were not found in maxillary and mandibular water storage group. However, in dry stored group, typical deformation pattern was detected as storage time passes. It occurred mostly in first two weeks. Major deformations were found in the bilateral posterior area in both maxillary and mandibular group. In maxillary dry stored group, a statistical significance was found. CONCLUSION: It was proved that in both upper and lower denture bases, dry storage caused more dimensional deformation than water storage with typical pattern.

STATEMENT OF PROBLEM: The most commonly reported problem associated with dental implant restoration is the loosening of the screws. PURPOSE: This study compared the efficacy of an implant system incorporating an anti-rotational locking sleeve(Anti-Rotating Inner Post Screw System(ARIPS-system)) with other, traditional implant systems as a means of minimizing vibration loosening. MATERIALS AND METHODS: Three implant systems were examined; the conventional external hex type, the ARIPS-system, and the internal taper type implant system. 30 specimens(10 samples per group)were fabricated and each abutment screw was secured to the implant fixture with 32Ncm of torque force and loosening torque was measured using a Torque Gauge. The procedure was repeated 3 times, recording initial loosening torque each time. The re-tightened abutment screw was subjected to a cyclic load having a maximum force of 200N and minimum of 20N at 2Hz over a period of 12,600 cycles, after which the loosening torque was measured. Measured values were calaulated for statistical analysis. Analysis of measured value was performed by 3 methods: (i) as a percentage average of the initial 3 loosening-torque values(initial loosening value) to the tightening torque of 32Ncm, (ii) as a percentage of the loosening torque value after a load of 200N(experimental value) to the initial loosening value, and (iii) as a percentage of the experimental value to the 32Ncm of tightening torque. The analyses shows the amount of initial loosening at the screw, loosening by repetitive load and the the final loosening value. RESULTS: The results of this study were as follows. (1) Percentage of initial loosening value to tightening-torque was increased in order of external hex, ARIPS-system and internal taper and all values between each groups showed statistical significance (p<0.05). (2) Percentage of experimental value to initial loosening value was increased in order of external hex, ARIPS-system and internal taper. Value of internal taper showed significant difference with those of external hex and ARIPS-system (p <0.05). (3) Percentage of experimental value to tightening torque was increased in order of external hex, ARIPS-system and internal taper and all values between each groups showed statistical significance (p <0.05). CONCLUSION: The results of the analysis of the final loosening level value, which are closely correlated to clinical use, show that the ARIPS-system can be a useful means of minimizing abutment screw loosening when compared to the external hex type system. Although further clinical studies need to be made, the ARIPS-system should be considered to maximize the long-term success of the implant prosthesis.

PURPOSE: Detecting and treating early colorectal cancers are most important for preventing advanced colorectal cancers which are difficult to manage. However, there is still a lack of data and guidelines in Korea about early colorectal cancers, especially when endoscopy is concerned. This study was undertaken in an effort to gather information in this area. METHODS: We experienced 107 early colorectal cancers from 1995 to 1998. We analyzed them with special reference to endoscopic characteristics and treatment. Ninty-six cases were followed for an average of 17 months. RESULTS: Ninty-four (87.9%) of the early colorectal cancers were detected during the last two years of the study. The male-to-female ratio was 2.1 to 1. The most common age group was the 7th decade. The rectum and the sigmoid colon were involved in 85% of the early cancers. Lesions smaller than 20 mm accounted for 54.2% of the early cancers and ones smaller than 10 mm 15%. There were no malignancies in flat-elevated lesions smaller than 10 mm; to the contrary, there was a 66.7% malignancy rate in depressed lesions. Malignancy developed in LSTs (Laterally spreading tumors) larger than 20 mm. Regarding configuration, protruded-type tumors accounted for 80.4% of the lesions, depressed-type tumors 5.6%, and LSTs 4.7%. The most common endoscopic characteristic of early colorectal cancer was redness. White spots, hardness, easy bleeding upon touch, and nodules were recognized in more than 20% of the tumors; depression and erosion were noticed in more than 10%. An expanded figure, convergence of mucosal folds, a whitish patch, and ulceration were useful in some cases. The submucosal cancer rate was 30.2% in protruded-type lesions and 66.7% in depressed-type ones. The overall adenoma-association rate was 93%. All lesions without an adenomatous component were submucosal cancers. The overall endoscopic resection rate was 87.9%. Among them, those who needed additional surgical resection accounted for 13.8% (13/94). Three recurrences were recognized. Those three were all early rectal cancers associated with villous tumors and were larger than 20 mm. All recurrences were treated endoscopically. There was one case of bleeding within 1 day after endoscopic resection. That was associated with a 20-mm, pedunculated lesion in the ascending colon and was treated using a detachable snare. CONCLUSIONS: It is important to recognize the endoscopic characteristics of early colorectal cancers, especially those of depressed lesions and LSTs. Those characteristics are also useful in selecting appropriate candidates for endoscopic resection.

PURPOSE: The objective of this study was to evaluate and compare the accuracy of impression body taking by the closed and the open tray impression technique with 3 types of impression tray. Individual tray, metal stock tray and polycarbonate tray were used. MATERIALS AND METHODS: Nine closed tray impressions were taken by individual tray, metal stock tray and polycarbonate stock tray, respectively with polyether impression material. 9 open tray impressions were also acquired by same manner. Precision analysis on the master models was performed by attaching the reference frameworks with alternate single screws and measuring the vertical fit discrepancy of respective analogues in working cast with a stereo microscope. Data were analyzed by 1 way ANOVA and independent t-test. RESULTS: The average fit accuracy of impression bodies was calculated. With the closed tray impression technique, there were significant statistical differences in vertical fit discrepancy according to the types of tray. The individual tray group showed the lowest value and the polycarbonate stock tray group represented the highest. With the open tray impression technique, there was no significant difference in vertical fit discrepancy. Significant statistical difference in vertical fit discrepancy was found between the open and the closed impression technique with the polycarbonate stock tray. CONCLUSION: From the results above, more precise impressions could be acquired by the rigid individual tray compared with the polycarbonate stock tray. It was hard to get consistent accuracy impressions by the closed tray impression technique with polycarbonate stock trays.

Computed tomography enterography (CTE) has become a main modality for the evaluation of inflammatory bowel disease (IBD). It simultaneously offers visualization of the small bowel and extraintestinal status, which is helpful for diagnosing IBD. Crohn disease has long segmental enhancing wall thickening related with the eccentric longitudinal distribution. In addition, mural stratification, fibrofatty proliferation, positive comb sign by increased mesenteric vascularity and internal/perianal fistula are characteristics of Crohn disease and can be identified on CTE. Short segmental inflammatory wall thickening and the central low attenuated lymph nodes are favorable CT finding of intestinal tuberculosis. A geographic, relatively large, and deep penetrating ulcer with bowel wall thickening and mural hyperenhancement in ileocecal area are characteristics of intestinal Behcet disease. Each of CTE findings for the IBDs is helpful for differential diagnosis. The main disadvantage of this technique is the requisite radiation exposure of patients, particularly in young patients. However, recent development of advanced CT techniques is promising for radiation dose reduction without compromising diagnostic image quality.

PURPOSE: The purpose of this study was to evaluate the phenomenon of diaschisis in the cerebellum and cerebral certex in patients with pure basal ganglia hemorrhage using cerebral blood flow SPECT. MATERIALS AND METHODS: Twelve patients with pure basal ganglia hemorrhage were studied with Tc-99m ECD brain SPECT Asymmetric index (AI) was calculated in the cerebellum and cerebral cortical regions as |CR-CL|/(CR-CL)x200, where CR and GL and the mean reconstructed counts for the right and left ROIs, respectively. Hypoperfusion was considered to be present when AI was greater than mean+2 SD of 20 control subjects. RESULTS: Mean AI of the cerebellum and cerebral cortical regions in patients with pure basal ganglia hemorrhage was significantly higher than normal controls (p<0.05): Cerebellum (18.68+/-8.94 vs 4.35+/-0.94, mean+/-SD), thalamus (31.91+/-10.61 vs 2.57+/-1.45), basal ganglia (35.94+/-16.15 vs 4.34+/-2.08), parietal (18.94+/-10.69 vs 3.24+/-0.87), frontal (13.60+/-10.8 vs 4.02+/-2.04) and temporal cortex (18.92+/-11.95 vs 5.13+/-1.69). Ten of the 12 patients had significant hypoperfusion in the contralateral cerebellum. Hypoperfusion was also shown in the ipsilateral thalamus (n=12), ipsilateral parietal (n=12), frontal (n=6) and temporal cortex (n=10). CONCLUSION: Crossed cerebellar diaschisis (CCD) and cortical diaschisis may frequently occur in patients with pure basal ganglia hemorrhage, suggesting that CCD can develop without the interruption of corticopontocerebellar pathway.
