Pheochromocytomas are rare catecholamine-secreting neuroendocrine tumors arising from chromaffin cells in the adrenal medulla. Typical classic triad are consisted of headaches, palpitations, and profuse diaphoresis. But some patients with pheochromocytomas have other cardiovascular manifestations such as left ventricular hypertrophy, congestive heart failure, and cardiac arrhythmia. Rarely, pheochromocytomas manifest as acute myocardial infarction leading to delayed diagnosis and treatment. We experienced one case of pheochromocytoma initially manifesting as acute myocardial infarction which showed normal coronary artery on coronary angiography. Pheochromocytoma should be suspected and evaluated in patients with acute myocardial infarction whose coronary angiography shows normal coronary without definite thrombosis.

BACKGROUND: This study was performed to examine the relationship between alcohol consumption and obesity according to facial flushing in Korean males. METHODS: The 1,198 men in this study were divided into four groups according to the amount of alcohol they consumed: non-drinkers (ND), lower moderate drinkers (LM, < or =7 standard drinks per week), higher moderate drinkers (HM, 7 to 14 drinks per week), and heavy drinkers (HD, >14 drinks per week). They were also separated into two groups according to facial flushing: non-flushers and flushers. Obesity and abdominal obesity were defined as body mass index of 25 kg/m2 or higher and waist circumference of 90 cm or higher, respectively. RESULTS: In LM and HM groups without flushing, the risks of obesity and abdominal obesity were not significantly increased compared to those of non-drinkers. However, in the HD group without flushing, those risks were significantly increased [OR, 1.55; confidence interval (CI), 1.01 to 2.40, and OR 1.63; CI 1.02 to 2.58, respectively]. In the LM group with flushing, those risks were not significantly increased. However, in HM and HD groups with flushing, the risks of obesity and abdominal obesity were significantly increased (OR, 2.10; CI, 1.07 to 4.16, and OR, 2.06; CI, 1.05 to 4.06, respectively, in HM; and OR, 2.16; CI, 1.08 to 4.34, and OR, 2.50; CI, 1.26 to 4.98, respectively, in HD). CONCLUSION: This study suggests that the risk of obesity is increased in moderate flushing drinkers compared to non-drinkers and in heavy non-flushing drinkers.

OBJECTIVE: To compare the pregnancy outcomes of transvaginal multifetal pregnancy reduction (MFPR) according to the gestational period when the procedure was performed METHODS: From January l995 to February 1998, total 27 patients with multiple pregnancy were included in this study. The patients were grouped to early MFPR group (<8 weeks, n=16) and delayed MFPR group (>8 weeks, n=ll) according to the gestational age that MFPRs were performed. All MFPRs were performed by transvaginal sonography-guided fetal aspiration or mechanical trauma. The complete pregnancy loss rate before 24 weeks of gestation, spontaneous loss of embryo, procedure-related complication, gestational age at delivery, and pregnancy complication were compared between the two groups. Statistical analysis of data was performed using Students t-test and Fishers exact test as appropriate. Statistical significance was defined as p<0.05. RESULTS: There was no significant difference in the complete pregnancy loss rate between the early MFPR group (6.3%) and the delayed MFPR group (27.3%). The incidence of partial spontaneous loss of embryo in the two groups were not differed significantly (6.3% vs. 18.2%). The procedure-related complication of the delayed MFPR group (36.4%) seemed to be higher than that of the early MFPR group (6.3%), however there was no statistical difference (p=0, 07). Especially, all 3 patients in whom the MFPR was performed after 10 weeks suffered from the procedure-related complication. The mean gestational age at delivery of the two groups were not differed significantly (36.3+2.8 weeks vs. 37.0+1.3 weeks). There was also no significant difference in the mean birth weights of the two groups (2378.8+563.7 gm vs. 2427.1+436.2 gm). CONCLUSION: Although there was no statistically significant difference, the early transvaginal MFPR might be a safe and useful method without significant adverse complications compared to the delayed MFPR.

Salmonellosis in poultry of Korea is a significant health problem, which causes substantial economic losses. The most common causative agents of chicken salmonellosis ar S. gallinarum and S. pullorum. Traditional methods used to detect Salmoenella spp. In chicken are tedious, time consuming and confer little guarantee of sensitivity and species specificity. Therefore, a rapid and sensitive method for the differentiation of Salmonella serogroup D was assessed. We first amplified the rfbS genes by PCR and characterized the amplified product by nucleotide sequence analysis. The homology of nucleotide sequence was 99.7% between S. gallinarum and S. pullorum rfbS genes. Further comparisons of the sequences of S. gallinarum, S. gallinarum fied strain, S. pullorum and S. typhi(GenBank Accession No.M29682) showed a homology of 98.3%. The predicted amino acid sequence homology was 97.1%, 97.1% and 97.5%, respectively. Based on this comparison of these nucleotide sequences, we found unique restriction enzyme sites within the rfbS genes of S. gallinarum and S. pullorum. Thus, the PCR products could be further digested with restriction enzymes TfiI and PleI for use in a restriction fragment length polymorphism (RELP) technique. This method can be applied in the differential diagnosis between S. gallinarum and S. pullorum.

Theileria (T.) buffeli (formerly T. sergenti/T. orientalis) is the major hemo-protozoan distributed in the Far East Asian countries such as Korea, China and Japan. It is responsible for the clinical symptoms of anorexia, ateliosis, anemia, fever and icterus. It also causes abortion and sudden death under severe cases, resulting in economic losses for many livestock farms. The objective of this study was to analyze the genetic diversity of the major surface protein (Msp) gene in T. buffeli in Holstein in Korea, and we characterized the association of the diversification of the Msp gene and its relationship with the pathogenicity of Theileria. For this, complete blood counts and Theileria PCR sequence analysis were performed from 57 Holstein in Jeju Island. A total of 26 PCR positive Holstein (16 anemic and 10 non-anemic) were then randomly selected based on 18s rRNA sequence typing of the Theileria Msp gene. The DNA sequence of the T. buffeli Msp gene in Holstein showed 99.0%, 99.2%, 99.9%, 99.5%, 98.7%, 98.4% and 98.4% homology with T. sergenti, Theileria spp., T. sergenti, Theileria spp., Theileria spp., Theileria spp. and Theileria spp., respectively. The result showed a genetic variation of 57.7% (type I), 3.8% (type II), 15.4% (type III), 7.7% (type IV), 13.5% (type V) and 1.9% (type VI). Type I is the most frequent type in both anemic and non-anemic Holstein while type II was found in only non-anemic Holstein. This results of our study help confirm the diversity of Msp gene types and demonstrate that the gene type distribution of Msp genes varies among Theileria-infected Holstein in Jeju Island.

Informed consent is the procedure that respects the autonomy of patients ; doctors protect patients' choices and rights through informed consent. Competence and voluntariness are prerequisite for the informed consent. In recent years, the necessity of informed consent from the Psychiatric department has been raised. Regarding informed consent from the Psychiatric department, there is a need to consider from two perspectives : respect for the patient's decision-making ability and the psychiatrist's legal self-protection. In competence and voluntariness, psychiatric patients were excluded due to lack of decision-making ability and willingness. However, not all psychiatric patients were able to perform the given task. Therefore, informed consent is needed in the point of view regarding the decision-making ability of psychiatric patients. Psychotropic medications, particularly antipsychotics, can cause several side effects, including extrapyramidal syndrome and metabolic syndrome. Therefore, preannouncement of these patients for these side effects is needed. However, when the informed consent is only applied to psychiatric hospitals, it may increase prejudices and misconceptions about psychiatric medications and treatment. Therefore, the informed consent should first be considered carefully and thoughtfully.

BACKGROUND: Hepatitis B virus (HBV) DNA quantitation and detection assay is necessary when monitoring the disease progression or therapeutic effect of viral hepatitis. The TaqMan PCR system (TPS) that was recently developed for HBV DNA quantitation was known for not only capable and exact quantitation but also capable sensitive detection. Therefore, we evaluated the clinical utility of the TPS by comparing it with hybrid capture system (HCS) and nested polymerase chain reaction (PCR). METHODS: The dilution test was performed to evaluate reproducibility and the dynamic range of TPS and HCS. Further, the sensitivity of the diluted sera were also compared with TPS, HCS, and nested PCR. The sera of HBsAg positive patients (n=119) were tested to compare the sensitivity of the three methods and the quantity of HBV DNA by TPS and HCS. RESULTS: The TPS showed lower coefficients of variations (TPS, 6.5- 60.4%; HCS, 11.9-61.4%) and a wider dynamic range than HCS in the dilution test. The sensitivity was high in order of nested PCR, TPS and HCS (cover 10(6), 5 X 10(5), and 10(2) of diluted concentration) in the dilution test. But the sensitivity was high in order of TPS, nested PCR, and HCS (89, 68%, and 56%) in the 119 sera of HBsAg positive patients. The TPS and HCS revealed a significant correlation (R=0.90, P<0.0001). CONCLUSIONS: The TPS and nested PCR showed higher sensitivity than HCS, and TPS showed better sensitivity than nested PCR in clinical specimens. Also, the TPS showed more quantitation potential than HCS. Thus, it appears useful in accurately defining the viral replication state and antiviral therapeutic effects among persons with HBV infection.
