1.Soft Tissue Perineurioma : A Case Report .

Soft tissue perineurioma is a very rare tumor composed of entirely of neoplastic perineurial cells. A 54-year-old woman presented with a palpable mass in the right lower leg. The mass was excised. Grossly, the tumor measured 2.0x2.0x1.5 cm. The cut surface was well circumscribed, pale pinkish gray, and rubbery soft. Histological examination showed that the tumor was composed of spindle cells within collagenous and myxoid stroma. The tumor cells had elongated, tapering nuclei with long and thin cytoplasmic processes, and were arranged in fascicular, whorled, and storiform pattern. The tumor cells were positive for epithelial membrane antigen and collagen type IV and negative for S-100 protein. Ultrastructurally, tumor cells showed long and thin cytoplasmic processes, pinocytic vesicles, and incomplete external lamina. The diagnosis of soft tissue perineurioma was confirmed by immunohistochemical stain and ultrastructural study.

2.Amniotic Membrane Transplantation for Tube Eerosion after Ahmed Glaucoma Valve Implant Surgery: Two Cases.

PURPOSE: We report two cases of tube erosion after Ahmed valve implantation, treated with Amniotic Membrane Transplantation. METHODS: Erosion of conjunctiva over tube occured in 2 female patients with Ahmed valve implantation. They were treated with pericardial patch grafts and conjunctival free graft with amniotic membrane epithelial side up transplantation. RESULTS: No recurrent tube erosion has been found since then. Intraocular pressure has been maintained well for 12-14 months of follow-up. CONCLUSIONS: In cases of tube erosion, additional amniotic membrane transplantation can improve surgical success.

4.A Case of Hepatic Vein Thrombosis Associated with Ulcerative Colitis.

Patients with inflammatory bowel disease are known to have hypercoagulability and an increased risk for venous thromboembolism. The deep veins of the lower extremities and the pulmonary veins are the most common sites of thrombosis in ulcerative colitis. However, hepatic vein thrombosis (Budd-Chiari syndrome) is a very rare extra-intestinal complication of ulcerative colitis in children. We describe a case of hepatic vein thrombosis in a 15-year-old girl with ulcerative colitis who presented with abdominal pain and hematochezia. Doppler ultrasonography and an abdominal CT scan revealed the characteristic filling defects caused by large thrombi in both hepatic veins. These lesions were successfully treated with conventional management for ulcerative colitis and anticoagulation therapy.

5.A Case of REM (Reticular Erythematous Mucinosis) Syndrome.

We present a case which seems identical to REM(Reticular Erythematous Mucinosis) syndrome in 26 year-old male. The lesion was initiated about 2 years ago and waxed and waned. The clinical findings were localized, grouped, erythematous papules and vesicles on the anterior middle chest with intermittent mild itching. Histopathology showed normal epidermis, extensive mucin deposition, telangiectasia and mild perivascular round cell infiltrations on the upper dermis. The mucin was stainable with alcian blue at pH 2. 5, but was not stained by either PAS or mucicarmine. Skin lesions were not responded to the oral steroid, but improved slowly after topical application of 5% para-aminobenzoic acid in ethanol for 9 months. The lesion does not recur until present time.

6.Value of the Left Portal Vein Angle (LPVA) on CT for the Diagnosis of Liver Cirrhosis: Comparison with the Caudateto Right Lobe (C/RL) Ratio.

PURPOSE: To verify the usefulness of left portal vein angle (LPVA) on CT scan in the diagnosis of liver cirrhosis and to compare its diagnostic value with that of caudate to right lobe ratio (C/RL ratio). MATERIALS AND METHODS: LPVA, an angle formed by a vertical line and a line connecting the center of the vertebral body to the umbilical point of the left portal vein, and C/RL ratio were measured on CT scans of 100 cirrhotic and 100 normal livers. Diagnostic values of LPVA and C/RL ratio were compared statistically. RESULTS: The mean of LPVA was 18.9 degrees(SD; 7.6) for normal livers and 25.8 degreesSD; 8.4) for cirrhotic livers (P<0.001). The mean of C/RL ratio was 0.47(SD; 1.10) for normal livers and 0.58(SD;0.14) for cirrhotic livers (P<0.001). When LPVA was greater than 30 degreesliver cirrhosis was diagnosed with 36% sensitivity and 92% specificity. When C/RL ratio was greater than 0.60, the diagnose of liver cirrhosis was with 41% sensitivity and 90% specificity. There was no significant difference of the diagnostic accuracy between LPVA and C/R L ratio in ROC analysis. CONCLUSION: Both LPVA and C/RL ratio are useful diagnostic indices of liver cirrhosis on CTscan. LPVA is more convenient to measure than C/RL ratio.

7.Doxycycline - Induced Esophageal Ulcers.

Esophageal ulcers induced by doxycycline is a rare complication. These patients usually complain of sudden onset of symptoms, ie acute substernal or chest pain and odynophagia without prior hietory of esophageal syraptoms. On esophagoscopic examination, there are upper or midesophageal ulcers, which heal after diseontinuation of the drug within 2 weeks. A history of ingestion of the doxycycline,with liquid jost before bedtime can be elicited. The exact eause of the xaucosal ulceration is not clear, but a direct irritant effeet on esophageal mucosa seems most likely. We report 5 cases of esophageal uleeration secondary to the ingestion of doxycydine. Esophagoscopy revealed esophageal ulcers in all patients and the patients hecame asymptomatic following stopping of tbe drugs and taking antacids.

8.The Changes in Field of Binocular Single Vision after Surgery for Blowout Fracture.

Blowout fracture of the orbit is one of the common sequelae to blunt facial trauma. Although diplopia is a common finding in patients following orbital trauma, quantative measurements of diplopia has not been well studied. We evaluated the extent and duration of improvement in field of binocular single vision(BSV)using quantitative scoring method. We prospectively studied 17 patients who had undergone repair of orbital blowout fracture. The indication for surgery was diplopia. A minimum of 3 months follow-up was available for all the patients included in the study. Binocular single visual field was tested with Goldman arc perimeter pre-and postoperatively. The analysis of the results was performed using model of Sullivan`s funcitional scoring method. We evaluated the recovery time and the factors which affect the process of recovery. BSV was acutely recovered by 1 month postoperatively and then slowly recovered(p=0.003, Wilcoxon signed ranks test). At 2 months after operation, average BSV score was over 80. We evaluated the delayed recovery group, 5 patients. The factors affecting the process of recovery were trap-door fracture type and hypertropia 10 prism diopter or more in primary position(p=0.02, chi-square test). In blowout fracture patients, the BSV scoring method presented improvement of diplopia quantitatively and it helped to assess the state and prognosis of the patients.

9.Clinical Experience with the Ahmed Glaucoma Valve.

The purpose of this study is to assess clinical outcomes of the patients who underwent Ahmed glaucoma valve implantation. Twelve eyes of 11 patients underwent Ahmed valve implantation for intractable glaucoma unresponsive to conventional management from March 1997 to April 1998 at department of ophthalmology of Kangbuk Samsung Hospital and had minimum follow-up of three months. Success(survival)was defined as an intraocular pressure(IOP)between 6 and 21 mmHg and with no additional glaucoma surgery, phthisis, implant removal, or loss of light perception. The mean follow-up period was 8 months(range, 3-17 months). There were 7 neovascular glaucomas, 3 secondary angle close glaucomas and 2 juvenile open angle glaucomas. The postoperative 1 year cumulative survival rate (Kaplan-Meier survival analysis)was 83. 3%. High survival rate was shown in secondary angle close glaucoma and JOAG(100%). The survival rate was not affected by sex, age, preoperative IOP, preoperative diagnosis, or number of preoperative glaucoma medications. There was failure in 2 eyes. One eye had qualified success by secondary Ahmed valve implantation. In the other eye, the tube in anterior chamber was too short and was plugged by blood clot and inflammatory membrane. This patient could not be reoperated due to poor general condition. Complications were hyphema and hypotony, and they were well controlled. Ahmed valve implantation in patients with intractable glaucoma may offer a better chance of surgical success.

10.Correlation of Endoscopic Redness with Histological Findings in Superficial Gastritis.

Superficial gastritis has been classified as a type of chronic gastritis, since a report of Schindler in GASTRLTIS, 1947. Howev~er, Benedict reported that superficial gastritis is only acute gastritis or shows normal mucosa histologically. The com mon endopical findings of chronic superficial gastritis were adherent mucus, edema, redness. The common redness which were encounterd are patchy redness and comb-like redness(Kammrotung). We studied the relationship between the redness of superficial gastritis and acute inflammatory changes histologically. Each case of superfieial gastritis was biopsied to redening and non-redening mucosa respectively. We collected 24 cases with 48 biopsy specimens. The results are as follows: Acute inflammatory changes were found only one case in redness and none in non-redness groups. There was no difference in acute inflammatory changes in two groups. The degree of mucosal atrophy were 62.5%, 66.7% in normal mucosa, 12.5%, 16.7~% in mild atrophy, 25%, 12.5% in moderate atrophy, 0%, 4.l% in severe atrophy with respect to redness and non-redness mucosa respectively. There was no difference in degree of mucosal atrophy in two groups. Mucosal atrophies were higher in older ages above 41 years old than below 40 and with increasing age, there was increasing tendency of mucosal atrophy. In conclusion, There was no relationships between mucosal redness and acute in flammatory changes histologically and also between mucosal redness and degree of atrophy.
