3.Dermographism ( III ): Dermographism in Acute and Chronic Urticaria.

The prevalence of dermographism in 96 patients of acute or chronic urticaria was studied using dermographic tester. The results were summarized as follows : 1. The prevalence rate of dermographism in acute or chronic urticaria was 26.0% (25/96). 2. Dermographism was more common in acute or chronic urticaria than in the general population(p<0.005). 3. There was no significant difference in the prevalence rate of dermographiam between acute and chronic urticaria. 4. The prevalence rate of dermographism was not significantly correlated with the level of serum IgE.

4.A Study on the Distribution of NADPH-Diaphorase and Neuropeptide Y Positive Neurons in the Cerebral Cortex of Apodemus Agrarius.

Apodemus agrarius has been using for experimental purpose to identifying the route of infection and pathogenesis of korean hemorrhagic fever. However, despite the increasing amount of information being published at present about the physiologic and ecologic characteristics of Apodemus, few data are availalle about the morphologic findings in the brain. The NADPH-diaphorase[NADPH-d] positive neurons, uniquely resistant to toxic insults and neurodegenerative diseases, have been colocalized with neurons in the brain and peripheral tissue containing nitric oxide synthase, which generates nitric oxide, a recently identified neuronal messenger molecule. In this study we used NADPH-d histochemistry to evaluate the distribution of neuropeptide Y-immunoreactive[NPY-IR] cells within neurons which contain nitric oxide synthase. In the cerebral cortex of Apodemus agrarius, NADPH-d positive and NPY-IR neurons were observed in all cortical layers, but they were concentrated in two bands layer II/III and V/VI extending into the subcortical white matter. Double labeling for NADPH-d and NPY showed colocalization of NPY with NADPH-d in numerous neurons of the cerebral cortex. The data obtained showed that about 74-79% of NPY-IR neurons contained NADPH-d in the neocortex and that 77-89% in the allocortex. The number of NPY-IR/NADPH-d positive neurons was about 10-13 per unit area [2,500,000 micrometer] in the neocortex and about 11-25 in the allocortex except retrosplenial cortex. In the retrosplenial cortex, the number of double labeled neurons was about 5 per unit area. NPY-IR and NADPH-d positive neurons were predominantly medium-sized with extended, multipolar or bipolar dendritic branches which belong to fusiform or stellate cell types. A moderately dense network of fine, varicose NADPH-d positive fibers was present throughout all cortical layers.

5.Topical fibronectin treatment in persistent corneal epithelial defects and corneal ulcers.

Topical fibronectin, autologous and homologous, was used to treat nine patients (eleven eyes) with persistent corneal epithelial defects and corneal ulcers that failed to improve with standard therapy. The fibronectin was purified from autologous and homologous plasma by gelatin-Sepharose 4B affinity chromatography and administered topically, 500 micrograms/ml five times a day, for three weeks. Complete or nearly complete reepithelialization was achieved in all patients regardless of the source of fibronectin, autologous or homologous. But healing times varied. The average healing time was 41.7 +/- 14.7 days (35.7 +/- 12.4 days for autologous, 50.8 +/-14.4 days for homologous). Ocular symptoms were relieved significantly, and no side effects were observed. Over an average follow-up period of 5.2 months, no recurrences were noted. The results showed that homologous, as well as autologous, fibronectin was effective in patients with persistent corneal epithelial defects and corneal ulcers.

6.A Case of Hereditary Trichilemmal Cyst with Proliferating Trichilemmal Tumor.

We report a case of hereditary trichilemmal cyst with proliferating trichilemmal tumor in a 64-year-oid man, who has generalized 157, variable sized firm cutaneous nodules for 40 years. The cystic nodules are chiefly located on the scalp and trunk which showed characteristic histopathologieal findings of trichilemmal cyst and proliferating trichilemmal tumors. His son also has multiple trichilemmal cysts and his mother seems to have had clinically similar lesions. The patient and his son were treated with surgical excision and therapeutic trial of oral isotretinoin for 2 months. Isotretionoin was ineffective in the patient, but the lesions of his son were decreased in size.

8.Mutagenic Activity of Organic Pollutans in Drinking Water in Seoul.

To measure the mutagenic activity of micro-organic pollutants in drinking water, mutagenic test was conducted using Salmnella typhimurium TA 98 strain on the water sample taken from three water supply station and six tap water in Seoul in July and November 1987. The results were as follows : 1. The average amounts of organic matters in raw, treated, and tap water sampled in July were 0.38mg/l, 0.28mg/l, and 0.45mg/l, respectively, and sampled in November were 0.34mg/l, 0.24mg/l, and 0.22mg/l, respectively. The amount of organic matters of tap water sampled in November did not increase while that of tap water sampled in July increased compare to those of raw or treated water. 2. The amount of organic matters is the highest in neutral fraction compare to acidic and basic fractions. 3. In the five out of six tap water and raw water of Paldang and Kuui station sampled in July, the mutagenic ratios were greater than two (both direct and indirect mutagenicity). 4. In the three out six tap water and raw and treated water of Kuui station sampled in November, the mutagenic ratios were greater than two. 5. While mutagenic were low in acidic and basic fraction, they were high in neutral fraction. The samples which had high mutagenic activity in the total amount also showed high mutagenic activity in neutral fraction. 6. While mutagenic activity was decreased after the treatment of water, it was increased in tap water as the distance from the water supply station increases.

9.One-Year Health Related Quality of Life and Its Comparison With Various Clinical and Functional Scale in Hospitalized Patients With Acute Ischemic Stroke: Seoul National University Bundang Stroke Registry Study.

BACKGROUND: Knowing the magnitude of a problem is the first step to improving it, and quantifying the health- related quality of life (HRQOL) allows us to do it with respect to stroke. To identify the HRQOL in patients with ischemic stroke and its affecting factors, and compare it with other clinical and functional scales. METHODS: A consecutive series of patients with acute ischemic stroke (AIS) or transient ischemic attack (TIA) was recruited. Their HRQOL and utilities were assessed using the EQ-5 Dindex-which evaluates the health profile of an individual in five dimensions (mobility, self-care, usual activity, pain/discomfort, and anxiety/depression) and three levels (no problem, some/moderate problems, and extreme problems)-at 1 year after stroke. The Modified Rankin Scale (MRS), Barthel Index (BI), and NIH Stroke Scale (NIHSS) were used to measure clinical and functional statuses. The EQ-5D index and the clinical and functional statuses were compared by clinical parameters, and the relationships among those scales were examined. Additionally, the EQ-5D index in the patients was compared with that in age- and sex-matched healthy controls. RESULTS: Among 538 patients with AIS or TIA, 465 patients (males, 59.8%; age, 65.3+/-11.8 [mean+/-SD]; TIA, 7.7%; and initial NIHSS, 4.5+/-5.1) were evaluated. The EQ-5D index and the clinical and functional statuses were clearly aggravated by age, but did not differ with most of the risk factors. The EQ-5D index was strongly and significantly correlated with scores on the MRS, BI, and NIHSS. The EQ-5D index was lower in patients (especially older ones) than in healthy controls. CONCLUSIONS: This study shows that the EQ-5D is a feasible and valid tool for measuring HRQOL in patients with AIS or TIA.

10.A Case of Congenital Cutaneous Candidiasis.

We report, a 1-day-old male with congenital cutaneous candidiasis who had generalized erythematous papules and pustules on the whole body including palms and soles from birth. At 5th month of pregnancy, his mother had a history of vaginal candidiasis for a month KOH mount of pus from pustules of several different sites showed budding spores and pseudohrphae and fungus culture revealed the growth of Candida albicans We believe this to he the first documented case of congenital cutaneous candidiasis in Korean literature.
