1.A Case of Limb-Body Wall Complex.

Limb-Body Wall complex is a complicated with the essential features of neural-tube defects, body-wall disruption, and limb abnormalities. This complex should be distinguished from other body-wall defects including omphalocele and gastroschisis since the prognosis for limb-body wall is uniformly poor. We have experienced one case of limb-body wall complex in a 22-year-old primigravida, which is presented with a brief review of the literature.

2.A Clinical Study on Softening E.C.T. & Comparison of Propofol and Pentothal as Anaesthetic Agents on Seizure Duration.

The authors performed this preliminary study to investigate the effect of softening E.C.T. and propofol was compared to pentothal for induction of anaesthesia for E.C.T on seizure duration. The results were follows ' 1) E.C.T. was performed in 60 psychiatric inpatients who were admitted during the study period. Of them 51.7% were diagnosed as schizophrenia, 21.6% as major depressive disorder, 16.7% as bipolar I disorder, manic and 10% of others. 2) Mean number of E.C.T. was 12.2 times a patient. 3) The most common target symptoms were persecutory delusion in schizophrenia, psychomotor retardation or agitation in major depressive disorder, and violent aggressive behavior in bipolar I disorder, manic. 4) Pre-ECT medication usually used were atropine 0.0093mg kg(-1), pentothal 2.76mg kg(-1) or propofol 1.42mg kg(-1). 5) The duration of seizure, as measured clinically, was reduced with propofol(20.5 sec) in comparison with pentothal(35.7 sec)(p<0.001). This suggest the possibility that additional treatment may be needed for the same clinical effect in psychiatric illness when propofol is used as the induction agent.

6.Adenosquamous Carcinoma of the Pancreas.

Adenosquamous carcinoma of the pancreas is a rare tumor but demonstrated an aggressive biologic behavior. A 60-year- old man with huge cystic mass in epigastrium was admitted complaining of abdominal discomfort. Initially the cystic mass was diagnosed to pancreatic pseudocyst. An abdominal computed tomography on 1 month later demonstrated a more enlarged mass in the body of the pancreas with central necrosis, which was infiltrating posterior wall of the stomach. Near total pancreatectomy, splenectomy and total gastrectomy were done. The resection specimen composed of squamous cell carcinoma components with a small area of adenocarcinoma. The patient died of cachexia 4 months after the operation. We reported an unusual case of adenosquamous carcinoma of the pancreas which was a giant cystic mass with central degeneration and gastric infiltration.

7.Giant Cavernous Hemangioma of the Liver.

Cavernous hemangioma is the most common benign tumor of the liver. This tumor is mostly small and asymptomatic, but giant cavernous hemangioma can produce symptoms and complications including abdominal pain, palpable mass and hemorrhage due to spontaneous or traumatic rupture. The authors encountered exceptionally large hemangioma with abdominal pain and anemia, and performed successful right hemihepatectomy, and present the case with the brief review of the literature.

8.Morphological Features of Metastatic Gastrointestinal Stromal Tumors after Gleevec Treatment: Two Cases Report.

We report two patients with metastatic gastrointestinal stromal tumors (GISTs) with a focus on the morphological features related to Gleevec treatment. In case 1, a 50-year-old woman presented with a 1.8 cm metastatic GIST in the liver after resection of a gastric GIST. Majority of the metastatic tumor showed fibrosis and hyalinization after 8 weeks of Gleevec treatment. CD117-positive cells were present in approximately 1% of the overall tumor. In case 2, a 2 cm and 14 cm metastatic liver masses were found in a 54-year-old man who had a rectal GIST. After 4 weeks of Gleevec treatment, metastatic tumors showed a decrease in size on CT scan. The metastatic tumors showed a decrease in number of tumor cells. The hemorrhage, cystic changes, necrosis, and fibrosis made up approximately 90% of the tumor. The morphological features related to Gleevec treatment are important for correct diagnosis and evaluation of tumor response and prognosis.

9.The Changes of Superoxide Dismutase Isozymes in the Placenta of Patients with Preeclampsia.

OBJECTIVE: The aim of this article was investigated whether changes of superoxide dismutase isozymes in the placenta of patients with preeclampsia contribute to radical-induced tissue injury. METHODS: The activities of antioxidant enzymes (superoxide dismutase (SOD), catalase, glutathione peroxidase (GSHPx)) and the contents of thiobarbituric acid reactive substance (TBARS) in the erythrocytes and in the placenta were assayed from 35 women with preclampsia and 35 normotensive pregnant women. RESULTS: The superoxide dismutase (SOD) activities were significantly reduced in the erhtyrocytes and the placenta of patients with pre-eclampsia compared with normotensive pregnant women. The activity of catalase was increased in the erythrocytes of patients with preeclampsia but the statistically significant difference of catalase activity in the placenta and GSHPx activity in both erythrocytes and placenta were not observed. The contents of TBARS were increased significantly in the erythrocytes and placenta of patients with preeclampsia. In preeclamptic placenta, copper and zinc containing superoxide dismutase (CuZn-SOD) was decreased (3.9+/-0.5 vs 5.1+/-0.6 U/mg protein) whereas manganeus containing superoxide dismutase (Mn-SOD) was increased (2.0+/-0.3 vs 2.7+/-0.4 U/mg protein). CONCLUSION: In these results, the decreased CuZn-SOD activity may some roles in increment of TBARS contents in pre-eclamptic placenta and decreased CuZn-SOD activity may be more prone to oxidative stress in the placenta.

10.The Effeet of GM-CSF on the Survival and the Changes in the Liver and Lung in Sptic Mice.

Sepsis is associated with high mortality, which correlates with the susceptibility of multiple organ dysfunction syndrome in the course of immunoinflammatory response during sepsis. Recently, granulocyte-macrophage colony stimulating factor (GM-CSF) is known to decrease the mortality of animals with severe stress, such as sepsis or thermal injury, by activating immunocytes associated with first line defence mechanism. Although GM-CSF may plays an important role in modulating immunoinflammatory cascade during sepsis, the exact mechanism of action is unclear and the information about the effects on the tissue damage is also poor. To assess the effects of GM-CSF on the mortality rate and on the tissue damage during sepsis, the authors conducted this animal study. Sepsis was induced by injection of 0.5 ml broth containing live Klebsiella pneumoniae (6x109 colony/ml) to the right thigh muscle of the mouse. The sepsis induced group was divided into four groups of 15 rats each. The first group was received normal saline and expressed as sepsis group. The second group was received antibiotics (cefoxitins) and expressed as A group. The third group was received GM-CSF (200 ng/body daily) and expressed as G group. The last group was received antibiotics and GM-CSF and expressed as AG group. The survival rates were obtained from each group. G and AG group showed significant improvement in survival rate compared to the sepsis and A groups (p<0.05). After 72 hours, all animals were sacrificed and submitted for the study of serologic and histologic changes. The level of interleukin-6 was significantly decreased in AG group compared to the sepsis guoup (p<0.05). The level of serum lactate and lung wet/dry ratio were significantly decreased in G group compared to the sepsis group (p<0.05). Glutathione content in liver tissue was significantly increased in the G and AG groups compared to the sepsis and A groups (p<0.05). Microscopic tissue examinations revealed that a significantly higher rate of tissue edema and necrosis, inflammatory cell infilteration, nuclear necrosis and endothelial swelling in sepsis group compare to the treated groups. However there was no significant difference between each treated groups. From these results, the beneficial effects of GM-CSF on the survival rate and tissue damage during sepsis were noted.
