1.The change of exercise according to time and its predicting factors in the followed - up hypertensive patients of family physicians.

BACKGROUND: Hypertension is one of the common diseases which family physicians encounter to manage. Various research proved that appropriate aerobic exercise had effect on lowering blood pressure efficiently. This study was to find out the status of exercise, the change of exercise according to time and its predicting factors in hypertensive patients. METHODS: A total of 193 of the followed-up hypertensive patients of family physicians in Seoul and kyoungkido from May 11, 2001 to June 10, 2001 were interviewed by doctor according to the previously designed structured questionnaire. RESULTS: Eighty two patients (42.57) were male and mean and was 57.4 +/- 9.9 fears. Using Kaplan-Meier's method, 8% of non-exercise group (N= 115) in diagnosing hypertension started exercise after 1 fear and 40%, after 5 years. Otherwise, 5% of exercise group(N=78) stopped exercise after 1 year and 30%, after 5 years. The characteristics of 5 exercise groups according to sex, age, level of education and adequacy of exercise showed significant difference. In average blood pressure, lately started group showed the lowest (137/84 mmHg) and non-exercise group, the highest (146/91 mmHg) In the degree of recommendation. sporadic group showed the highest and non-exercise group, the lowest which showed significant difference (p =0.0024) . CONCLUSION: I conclude that lately started group among non-exercise group and continuous exercise group were mainly affected by recommendation to exercise of physician. As well as exercise, other nonpharmacologic treatment should be promoted by physician as an important strategy for treatment of hypertension.

2.Cutaneous Lesion due to Mycobacterium Fortuitum.

Nontuberculous mycobacteria usually cause systemic disease and often appear as a primary pulmonary infection. However, a cutaneous lesion may be the first or only sign of infection. The most frequent human diseases caused by Mycobacterium fortuitum are cutaneous or soft tissue infections, usually preceded by injection, trauma or surgery. We report a case of a cutaneous lesion due to Mycobacterium(M.) fortuitum occuring in a 50-year-old man with a subcutaneous flat nodule on his left anterior tibia. He had received a small injury to his left shin in a swimming pool. The culture from the skin biopsy specimen on Lowenstein-Jensen medium yielded clolnies after 5 days. The microorganism was subsequently identified as M. fortuitum by selected biochemical reactions. Therapy was instituted with minocycline for 5 months.

Mediastinal fibrosis is pathologically characterized by chronic inflammation and fibrosis of mediastinal soft tissue. Mediastinal fibrosis is local expression of a family of systemic fibrosing syndroms. This can result in compression of adjacent mediastinal structures. Idiopathic fibrosing syndromes include retroperitoneal fibrosis, sclerosing cholangitis of the orbit and fibrosis of the thyroid gland(Riedel's struma). The cause of these disorders is obscure, in some instance there is an underlying malignancy, infection, history of drug ingestion, or trauma with retoperitoneal bleeding. Treatment of mediastinal fibrosis depends on structures involved by the fibrotic process. The disease is self limited in most case or improved by steroids uses. We experienced a case of idopathic solerosing mediastinitis with orbital fibrous dysplasia of unknown cause, which was confirmed by open lung biopsy, so reported it with a review of literature.

4.A case of malignant melanoma of the urinary badder.

Malignant melanoma of the urinary bladder is very rare, 7 cases were reported in the literature. The tumor shows early metastasis with poor prognosis. Treatment measures include surgical extirpation, radiotherapy and chemotherapy but the results were bad.Herein, we report a case of malignant melanoma of the urinary bladder in a 45-year-old man which was managed by transurethral resection and combination chemotherapy.

5.Comparison of the Short-Term Effects of Intravitreal Triamcinolone Acetonide and Bevacizumab Injection for Diabetic Macular Edema.

PURPOSE: To compare the short-term effects of intravitreal triamcinolone acetonide (IVTA) with those of intravitreal bevacizumab (IVB) injection for diabetic macular edema (DME). METHODS: The present retrospective, comparative case study included 58 eyes of 35 consecutive patients (IVTA group, 20 eyes; IVB group, 38 eyes) with DME. IVTA (4 mg) or IVB (1.25 mg) injection was performed under local anesthesia. The effects of injection for DME were evaluated using best-corrected visual acuity (BCVA), central macular thickness (CMT) by optical coherence tomography and intraocular pressure (IOP) by applanation tonometer. Patients underwent eye examinations, including BCVA, CMT, and IOP at pre-injection, 2, 4, and 8 weeks after injection. RESULTS: BCVA (logarithm of the minimum angle of resolution) +/- SD at pre-injection, 2, 4, and 8 weeks after injection was 0.67 +/- 0.40, 0.56 +/- 0.35 (p = 0.033), 0.55 +/- 0.33 (p = 0.041), and 0.43 +/- 0.31 (p = 0.001) in the IVTA group and 0.51 +/- 0.31, 0.42 +/- 0.26 (p = 0.003), 0.43 +/- 0.32 (p = 0.001), and 0.43 +/- 0.27 (p = 0.015) in the IVB group, respectively. CMT (microm) +/- SD at pre-injection, 2, 4, and 8 weeks after injection was 400.4 +/- 94.9, 332.8 +/- 47.4 (p = 0.002), 287.5 +/- 49.1 (p = 0.007), and 282.5 +/- 49.6 (p = 0.043) in the IVTA group and 372.6 +/- 99.5, 323.2 +/- 72.4 (p = 0.077), 360.9 +/- 50.3 (p = 0.668), 368.2 +/- 88.6 (p = 0.830) in the IVB group, respectively. CONCLUSIONS: The effects of IVTA for BCVA were more favorable than were those of IVB and were consistent throughout the eight weeks after injection. IVTA significantly reduced CMT during the eight weeks after injection, while IVB did not.

6.Kimura's Disease Involving the Ipsilateral Face and Extraocular Muscles.

Kimura's disease (KD) is a rare, chronic inflammatory disorder, which is characterized by tumor-like masses mainly located in the head and neck region. Extraocular muscle involvement in KD is uncommon. We report a case of KD that involved both the extraocular muscles and buccal area. A 13-year-old male presented to our clinic with a two-year history of exophthalmos of the left eye and facial swelling. Facial CT and MRI showed a 1.5 x 1.5 cm2 soft tissue mass located at the left masticator and buccal area, exophthalmos of the left eye, and diffuse thickening of the left extraocular muscles. We performed a lateral rectus muscle incisional biopsy of the left eye. Oral methylprednisolone therapy was initiated and tapered following the incisional biopsy.

8.Polymicrobial Keratitis of Pseudomonas aeruginosa, Acinetobacter baumannii, and Ochrobactrum anthropi

PURPOSE: To report polymicrobial keratitis involving Pseudomonas aeruginosa, Acinetobacter baumannii, and Ochrobactrum anthropi. CASE SUMMARY: A 53-year-old female complained of pain and secretion in her right eye, which started 6 weeks before her visit. She applied steroid ointment, which was received from the dermatologist, to her eyelid 7 days prior to her visit but this treatment worsened her symptoms. At the initial visit, the visual acuity of the right eye was light perception, and purulent secretions were observed. Using a slit lamp, severe conjunctival hyperemia, hypopyon, and a ring-shaped central corneal ulcer were observed. The anterior chamber and fundus were not observed due to corneal lesions but ultrasonography showed no intraocular inflammation. Infectious keratitis was suspected and cultured by corneal scraping. During the incubation period, 0.5% moxifloxacin, 2% voriconazole, and 1% cyclopentolate were administered. A total of 400 mg of moxifloxacin and 100 mg of doxycycline were given orally. In the primary culture, Pseudomonas aeruginosa and Acinetobacter baumannii were identified so 5% ceftazidime, which was sensitive for the antibiotic susceptibility results was further instilled. Thereafter, the keratitis improved but the keratitis again worsened while maintaining the topical treatment. A secondary culture was positive for Ochrobactrum anthropi. Treatment with 1.4% gentamicin, which was sensitive for the antibiotic susceptibility test was added and the keratitis improved. A conjunctival flap was performed because of the increased risk of perforation. CONCLUSIONS: We report polymicrobial keratitis involving Pseudomonas aeruginosa, Acinetobacter baumannii, and Ochrobactrum anthropi for the first time in the Republic of Korea.

9.Time Series Changes in Cataract Surgery in Korea.

PURPOSE: We analyzed time series changes in cataract surgeries in Korea, and provide basic data to enhance the efficiency of medical services for cataract surgery. METHODS: Among cataract surgery statistics registered in the Korean National Health Insurance Cooperation from 2006 to 2012, we used data regarding the number of patients and operations and the number of patients and operations per 100,000 people. We analyzed various time series changes, including differences by sex and age. RESULTS: The total numbers of patients from 2006 to 2012 by year were 207,370; 228,170; 250,289; 268,548; 289,867; 308,111; and 302,182, respectively. The total numbers of operations from 2006 to 2012 by year were 272,920; 305,807; 338,332; 365,874; 398,338; 428,158; and 420,905, respectively. The number of patients and operations per 100,000 people were highest in men 80 to 84 years old and women 75 to 79 years old. Comparing the number of operations in 2006 and after, the patient age group with the highest increase rate changed from over 85 years old to 75–79 years old since 2010 in men and from over 85 years old to 50–54 years old since 2009 in women. For each year investigated, the number of operations performed was higher than the number of patients who received operations. CONCLUSIONS: Over the study period, the number of cataract surgeries increased, while the age of cataract patients decreased. Additionally, the number of cataract-related surgeries increased in relation to the number of patients.

10.An epidemiological note on the taeniasis in Korea.

The present study was undertaken to evaluate the prevalence rate of Taenia spp. infection and distribution of taeniasis caused by T. solium among Koreans in several localities in Korea during the period from 1977 to 1980. A total of 1,946 stool specimens collected from students and inhabitants of Chungcheong Bug Do and Gyeongsang Nam Do were examined by cellophane thick smear method. Among them, 40 persons were detected as Taenia egg positive cases. The overall positive rate of Taenia spp. shows 2.1 percent. The prevalence rate of male (2.3 percent) is relatively higher than that of female (1.4 percent). In order to observe the distribution of Taenia solium infection, the whole worms or a part of proglottids of Taenia spp. were collected from the stools of egg positive cases by normal defecation or anthelmintic treatment. For the species identification, expelled proglottids were examined microscopically by the number of branches of the uterus, presence of a vaginal sphincter or the accessory ovarian lobe etc. Among 199 egg detected cases in this study 59 (39.1 percent) out of 151 cases in Seoul, 12(40.0 percent) of 30 cases in Gyeongsang Nam Do and 2 (25.0 percent) from 8 taeniasis cases in Cheju Do were infected with T. solium. But none of T. solium infection was found from 10 egg positive cases in Chungcheong Bug Do. As a whole, the composition of species shows 36.7 per cent of T. solium infection and 55.8 percent of T. saginata infection, and in 7.5 per cent the species were not identified.
