2.Benefit From Directional Microphone Hearing Aids: Objective and Subjective Evaluations.

OBJECTIVES: The aims of this study were to find and compare the effect of directional (DIR) processing of two different hearing aids via both subjective and objective methods, to determine the association between the results of the subjective and objective evaluations, and to find out individual predictive factors influencing the DIR benefit. METHODS: Twenty-six hearing aid users fitted unilaterally with each two different experimental hearing aid performed modified Korean Hearing in Noise Test (K-HINT) in three DIR conditions; omnidirectional (OMNI) mode, OMNI plus noise reduction feature, fixed DIR mode. In order to determine benefits from DIR benefit within a hearing aid and compare performance of the DIR processing between hearing aids, a subjective questionnaire was administrated on speech quality (SQ) and discomfort in noise (DN) domain. Correlation analysis of factors influencing DIR benefit was accomplished. RESULTS: Benefits from switching OMNI mode to DIR mode within both hearing aids in K-HINT were about 2.8 (standard deviation, 3.5) and 2.1 dB SNR (signal to ratio; SD, 2.5), but significant difference in K-HINT results between OMNI and OMNI plus noise reduction algorithm was not shown. The subjective evaluation resulted in the better SQ and DN scores in DIR mode than those in OMNI mode. However, the difference of scores on both SQ and DN between the two hearing aids with DIR mode was not statistically significant. Any individual factors did not significantly affect subjective and objective DIR benefits. CONCLUSION: DIR benefit was found not only in the objective measurement performed in the laboratory but also in the subjective questionnaires, but the subjective results was failed to have significant correlation with the DIR benefit obtained in the K-HINT. Factors influencing individual variation in perceptual DIR benefit were still hard to explain.

4.Standardization Study for Discharge Abstract Data.

The purpose of this study was to identify important items from the medical records to be used in the standardized discharge abstract. Common items were identified by analyzing medical records from the 11 largest hospitals in Seoul. Non-common items were identified by a questionnaire survey from the directors of medical record departments of 152 teaching hospitals. The results of research was follows; 1. Thirty eight common items were included in the analyzed sheet of 11 hospitals. 2. Eighty two non-common items were identified from the analyzed. Of these,10 items were found to be important items for the discharge abstract. 3. Another 26(half) or 18(first quarter) important non-common items were identified from the survey. 4. It was notified in the non-common standardized items group that the importance of some items like the patient's occupation, underlying cause of death, nosocomial infection, complications, house staff code in charge of completing records, and items concerning quality improvement showed difference by the number of beds. The importance of house staff code who is responsible for completion of the record also showed statistically significant difference by the number of beds per medical record professional and by regions. The item of the types of nosocomial infection also showed statistically significant difference between the regions. Most hospitals obtain a lot of medical information from the computerized discharge abstract. One of the results of the study showed that the concerned sheet can housed as both the data for the medical insurance claims and the basic data for medical quality improvement. Therefore, the discharge abstract should be regarded as the most necessary sheet to be standardized. It was found that 92.8% of the directors of medical record departments of nationwide teaching hospitals acknowledged the necessity of standardization of medical record data set.

6.The Effect of Intraocular Pressure-lowering Eye Drops on Myopic RetinoschisisPatients

Purpose:
To investigate the influence of intraocular pressure (IOP)-lowering eye drops on myopic retinoschisis.
Methods:
We investigated myopic retinoschisis patients with high myopia (defined as a myopic refractive error more than -8.0 dioptersor an axial length longer than 26.5 mm), who were suspected of having glaucomatous optic neuropathy, using IOP-loweringeye drops between April 2014 and December 2018. We retrospectively analyzed the changes in optical coherence tomographyfindings after 6 months using IOP-lowering eye drops. The progression of retinoschisis was assessed by analyzing retinalvolume changes. A decrease in the total retina volume and a decrease in volume more than 10% in one section out of five withoutmore than a 10% volume increase in any other section was defined as resolution of myopic retinoschisis. The opposite casewas defined as an aggravation.
Results:
We analyzed 17 eyes of 15 patients with high myopia. Six of 17 eyes (35.3%) showed a resolution of myopic retinoschisisat 6 months after using IOP-lowering eye drops. Of the 17 eyes, two (11.8%) experienced progression of myopicretinoschisis. Seven out of 14 eyes (50.0%) who were followed-up over 1 year showed resolution of myopic retinoschisis, andtwo eyes (14.3%) experienced progression of myopic retinoschisis. There was no macular hole development or posterior vitreousdetachment during the follow-up period in the seven eyes, and there was no significant correlation between the absolute valueof the initial IOP, axial length, IOP change, and degree of improvement of myopic retinoschisis.
Conclusions
The use of IOP-lowering eye drops on highly myopic eyes with retinoschisis showed a significant improvement ofmyopic retinoschisis, when compared to previous studies. These findings suggest the possibility of IOP-lowering eye drops delayingor improving the natural course of myopic retinoschisis.
7.Academic Performance, Communication, andPsychosocial Development of Prelingual Deaf Childrenwith Cochlear Implants in Mainstream Schools

To assess the academic performance, communication skills,and psychosocial development of prelingual deaf children with cochlear implants (CIs) attendingmainstream schools, and to evaluate the impact of auditory speech perception on theirclassroom performance. Subjects and Methods: As participant, 67 children with CI attendingmainstream schools were included. A survey was conducted using a structured questionnaireon academic performance in the native language, second language, mathematics, socialstudies, science, art, communication skills, self-esteem, and social relations. Additionally,auditory and speech performances on the last follow-up were reviewed retrospectively. Results:Most implanted children attending mainstream school appeared to have positive selfesteemand confidence, and had little difficulty in conversing in a quiet classroom. Also, halfof the implanted children (38/67) scored above average in general academic achievement.However, academic achievement in the second language (English), social studies, and sciencewere usually poorer than general academic achievement. Furthermore, half of the implantedchildren had difficulty in understanding the class content (30/67) or conversing withpeers in a noisy classroom (32/67). These difficulties were significantly associated with poorspeech perception. Conclusions: Improving the listening environment for implanted childrenattending mainstream schools is necessary. Pulmonary lymphangioleiomyomatosis is a disease involving the proliferation of atypical smooth muscle cells trom the perilymphatics, peribronchial and perivascular region of the lung and the retroperitneum. The disease usually affects women of child-bearing age. We recently experienced a case of pulmonary lymphangioleiomyomatosis in a 31-year-old women who had suffered from a chylous pleural effusion. Histologic confirmation of lymphangioleiomyomatosis was made upon a video-associated thoracoscopic lung biopsy. Here we report this case with a brief review of the literature.

9.Postoperative Echocardiographic Hemodynamic Comparison between Recently Available Bileaflet Mechanical Valves.

BACKGROUND: There was no difference between the bileaflet mechanical valves on the midterm and longterm clinical outcome. We reviewed the hemodynamic comparison between recently available mechanical valves by Doppler Echocardiography. MATERIAL AND METHOD: We retrospectively reviewed 396 postoperative hemodynamic datas (EOA, MDPG, and MSPG) by doppler echocardiography in 345 patients. Mechanical valves from 5 venders (Sorin Bicarbon, SJM, ATS, On-X, and Edward MIRA) were compared. There were 232 valves in mitral position, 162 in aortic, and 2 in tricuspid. RESULT: There were 178 men (mean age; 50.6+/-13.9 years old) and 167 women (52.6+/-14.6). MDPG/EOA of 27 mm in mitral position was Sorin; 4.2+/-1.5 mmHg/3.0+/-0.9 cm2, SJM; 2.3+/-1.2/3.5+/-0.6. In 29 mm, Sorin, SJM, ATS, On-X, MIRA revealed 3.4+/-1.2/3.1+/-0.6, 3.3+/-1.1/2.7+/-0.4, 3.8+/-0.8/3.2+/-0.6, 4.0+/-3.0/3.1+/-0.9, 2.9+/-0.9/3.0+/-0.8. In 31 mm, Sorin, SJM, ATS, MIRA revealed 3.9+/-1.9/2.9+/-0.6, 3.5+/-1.2/3.0+/-0.6, 3.4+/-0.8/2.8+/-0.2, 3.7+/-1.5/2.7+/-0.7. In 33 mm, Sorin, SJM, MIRA revealed 4.4+/-0.9/2.5+/-0.4, 3.4+/-1.5/3.3+/-0.5, 4.7+/-2.4/3.0+/-0.3. MSPG/EOA of 19 mm aortic position was Sorin, SJM, ATS, On-X, MIRA 18.0 mmHg/1.2 cm2, 25.6+/-8.7/1.1+/-0.3, 25.9+/-12.6/1.2+/-0.3, 23.0/1.3, 27.9+/-7.1/1.2+/-0.1 in that order. In 21 mm, SJM, ATS, On-X, MIRA revealed 18.3+/-6.7/1.5+/-0.5, 13.7+/-2.1/1.7+/-0.3, 17.0/1.4, 17.1+/-5.5/1.8+/-0.5. In 23 mm Sorin, SJM, ATS, On-X, MIRA revealed 14.0+/-4.6/1.7+/-0.6, 12.8+/-3.2/2.0+/-0.2, 16.8+/-12.2/2.1+/-0.9, 14.0/1.5, 15.0+/-5.5/1.8+/-0.5. In 25 mm, SJM and MIRA revealed 14.0+/-5.1/1.8+/-1.0, 11.0/2.3. There was no statistically significant difference in these values between the venders given the same position and size. 2 redo valve replacements were performed, 1 due to severe hemolysis in ATS and 1 due to leaflet immobilization in SJM. CONCLUSION: Postoperative hemodynamic comparison by doppler echocardiography shows no statistically significant difference between recently available mechanical valves in this country.
