3.A Clinical Study on the Antihypertensive Effect of Nipradilol.

beta-receptor antagonist as antihypertensive agent has adverse effect such as vasoconstriction, bronchoconstriction and aggravation of atherosclerosis. So beta-receptor antagonist with alpha-receptor blocking property has been studied for avoidance of adverse effects of beta-receptor antagonist. Nipradilol, a new beta-receptor antagonist with alpha-receptor blocking property was administrated in 30 essential hypertensive patients for 10 week in order to evaluate the antihypertensive effects and adverse effects. The results were as follows: 1) Nipradilol was effective in 20 patients(67%) on systolic blood pressure 22 patients (73%) on diastolic blood pressure at the end of 10 weeks treatment. 2)Before medication, mean systolic and diastolic blood pressure in sitting position were 160+/-14mmHg and 102+/-18mmHg,and after 10 weeks of medication, mean systolic and diastolic blood pressure were significantly declined to 143+/-22mmHg and 89+/-12mmHg(p<0.001). 3) There was no significant change in pulse rate, from 78+/-12 beats per minute the beginning of the treatment to 75+/-11 beats per minute after 10 week of treatment. 4) There was no significant changes in sodium, potassium, GOT, GPT, bilirubin, alkalin ephosphatase, BUN, creatinine, LDH, CPK and lipid. 5) The adverse effects of nipradilol were bradycardia(7%), dyspnea(3%), chest discomfort(3%) and headache(3%). In summary, nipradilol seemed to be effective antihyertensive drug in treating mild to moderate hypertension.

7.Two Cases of Cutaneous Cytomegalovirus Infection in Immunocompromised Patients.

Cytomegalovirus (CMV) is a major cause of morbidity and mortality in immunocompromised patients. CMV can cause pneumonia, retinitis, gastrointestinal ulcers, and widely disseminated disease, but cutaneous CMV is rare. We report two cases of cutaneous CMV infection presenting as perianal ulcers. A 54 year-old male who had liver transplantation and a 72-year-old male, who was treated with chemotherapy for angioimmunoblastic T-cell lymphoma, presented with perianal ulcers and had systemic symptoms of CMV infection with CMV antigenemia. They had multiple ulcerations with erythematous bases on the perianal area, and histopathologic examinations showed large atypical cytomegalic cells in the dermis, and immunohistochemical stains, with the anti-CMV antibody showed positive reactions.

8.Determination of High Density Lipoprotein Cholesterol in Psoriasis Patients.

In orper to measure the levels of high density lipoprotein cholesterol in psoriasis, 60 psoriatic patients and 30 healthy subjects were included in this study. Lopez-Virells methos was applied for measuring the serum level of high density lipoprotein cholesterol. The results were obtained as follows. The level of serurn HDL cholesterol was 58. 39+17. 40 mg/dl in psoratcs and 50.3+0.31 mg/dl in healthy subjects and 50. 43+ 10. 31 mg/dl in healthy subjects. No significant differences were noted between psoriatics & healthy subjects. 2. The level of serum HDL cholesterol was 56. 40+19. 10 mg/dl in male group of psoriatics and 60. 00+15.47 mg/dl in female group of psoriatics and 48.3+9. 50 mg/dl in healthy male group and 52.60 -10.59 mg/dl in healthy female group. No significant differences of serum HDL cholesteol levels were noted in both sexes.3. The mean value of serum HDL cholesterol by age groups of 10, 20, 30, 40, and 50 years old with psoriasis were 50. 70 mg/dl, 61. 97 mg/dl, 57. 44 mg/dl, 49. 11 mg/dl and 70. 36 mg/dl, respectively and those of healthy groups were 57. 25 mg/ dl, 45. 17 rng/dl, 50. 97 rng/dl, 48. 07 mg/dl and 46. 98 mg/dl, respectively.

9.The Asian-Pacfic Gamma Nail for Intertrochanteric Fracture of the Femur.

PURPOSE: To evaluate the treatment results of geriatric intertrochanteric fractures by using Asian-Pacific Gamma nails. MATERIALS AND METHODS: From June 1994 to June 1997, 58 cases of the intertrochanteric fractures were treated with Asian-Pacific Gamma nail, particularly compression hip screw fixation complications such as communited fracture involving lesser trochanter, transverse or reverse oblique intertrochanteric fracture. We evaluated the bone union time, neck-shaft angle, lag screw sliding by follow up radiographs, and studied complications and intraoperative cautions. RESULTS: The average age was 74.4 years old, the mean duration of follow-up was 17 months, the mean duration of bone union was 14.2 weeks, the average neck-shaft angle was postop. 133+/-2.78 degree and last follow-up was 129.5+/-3.37 degree, and the average lag screw sliding was 4.7+/-3.16 mm. Intraoperative complications were difficulty in distal drilling (posterior drilling) in 4 cases, fracture displacement by nail insertion in 3 cases, and postoperative complications were coxa vara in 13 cases, superior cutting out of lag screw with nonunion in 1 case and superficial infection in 1 case. The entry portal must be formed by awling. The fracture displacement when inserting nail could be prevented by proper reaming after inserting the C-shaped guide pin, and posterior drilling could be avoided by confirming the locking between target device and nail. CONCLUSIONS: We conclude that the Asian-Pacific Gamma nail could appropriately treat the osteoporotic, communited intertrochanteric fracture in elderly patients and we obtained satisfactory results.
