2.The effects of Broad Spectrum Antibiotics and Endotoxin to the Carbon Tetrachloride-induced Liver Injury.

This study was performed to investigate the effect of endotoxin to the CCl4-induced liver injury. Twelve Sprague-Dawley rats were intraperitoneally injected 1.6 g/kg CCl4 as control group. Another 24 rats were orally administrated 300 mg/kg of neomycin at 16 and 3 hours prior to CCl4 injection as experimental group. Twelve among them were intraperitoneally infected 1.0 mg/kg of endotoxin(E-Coli, 0111:B4, No L-2630, lipopolysaccharide, Sigma, USA) and CCl4 simultaneously for offsetting neomycin effect. The rats were sacrificed at 1, 4, 10, and 24 hours after CCl4 injection. The liver tissues from all experimental groups were observed by light and electron microscopy. The results obtained were summarized as follows: In the CCl4 only group, the hepatocytes revealed sweling of ER and mitochondria with many lipid droplet in the cytoplasm. Focal cellular necrosis was seen at the later phase. The Kupffer cells were activated and showed many cytoplasmic processes, secondary lysosomes, and vaculoles. The endothelial cells were edematous. Several neutrophils, platelets, and microthrombi were scattered in the sinusoid. In the neomycin-CCl4-endotoxin administrated group, both hepatocytic destruction and intrasinusoidal microthrombi formation were more pronounced. In the neomycin pretreated group, the hepatocytes revealed mild cellular destruction without necrosis. There is no intrasinusoidal microthrombi. According to these results, it would be concluded that the small dosage of gastrointestinal tract-derived endotoxin affects to the liver injury caused by CCl4. The synergistic effects of CCl4 and gastrointestinal tract-derived endotoxin which can not be detoxified by damaged Kupffer cells, may be more important in the pathogenesis of CCl4-induced liver injury.

3.A Case of Surgically Extracted Cysticercus Cellulosae from the Anterior Chamber and Three Cases of Subconjunctival Cysticercosis Cellulosae.

The authors experienced a case of anterior uneitis associated with secondary glaucoma caused by a cycitsercosis cellulosae in the anterior chamber of the left eye of a 39-year-old Korean male. The cyst appeared from the posterior surface of the iris through the pupillary margin of 10-o'clock position and grew larger and larger with automatic active movements, filling finally the nasal upper one third of the anterior chamber, but the cyst did not change the position to the 6-o'clock chamber angle. The cysticercus grown for almost four months was extracted successfully with a teethless, serrated, curved forceps with some resistance after opening the anterior chamber from 7~11-o'clock position without iris prolapse and any damage to the anterior lens capsule. The extracted cyst with the size of 8 mm X 6 mm X 3 mm was confirmed as a cysticercus cellulosae by parasitological examinations (Fig. 2,3). After the extraction of the cysticercus the uveitis and the increased intraocular tension subsided completely, remaining some brownish fine keratic precipitates, pigment masses on the anterior surface of the lens and irregularly somewhat dilated pupil due to partial posterior synechia. The naked vision O.S., however, was 20/25 using pin hole, and the eye has been still quiet during last 3 and half years following surgical extraction. In addition the authors have recently seen three cases of subconjunctival cystiticercosis cellulosae. In the first case the cysticercus with the size of 8 mm X 7 mm X 4 mm appeared at the nasal superior part of the bulbar conjunctiva O.D. of a three-year-old Korean male. In the second case the cyst with the size of 7 mm X 5 mm X 3 mm was seen at the nasal inferior part of the bulbar conjunctiva O.S. of a 12-year old Korean female. In the last case the cyst with the size of 10 mm X 8 mm X 5 mm was located beneath the plica semilunaris of 3-o'clock position O.D. of a 58-year-old Korean female. All the cysts were extracted successfully without any rupture, and they were confirmed as the cysticercus cellulosae histologically.

4.A Case of Acute Panautonomic Neuropathy.

Dysfunction of autonomic nervous system had been reported in a number of neurologic and medical diseases, but acquired primary dysautonomia is uncommon and usually tends to show a chronic progressive pattern. Acute panautonomic neuropathy characterized by selective involvement of sympathetic and parasympathetic nerves with complete or relative sparing of somatic motor and sensory functions was first described by Young et. Al, in 1969 and since then, about 26 additional cases had been reported in the literatures, but there have been no such reports in Korea. The present report describes a 22 year-old female patient who had been admitted to Severance hospital because of abruptly developed fever, generalized myalgia, hyperesthesia and difficulties in swallowing and voiding. On admission, physical and neurological examinations revealed orthostatic hypotension, unreactive pupils to light and accomodation, dereased gag reflex, and decreased sensation of pain and temperature on the face and neck. A few days following admission, progressive loss of deep tendon reflew was noticed, followed by distal muscle weakness and atrophy. The serial electrophysiologic studies revealed progressive peripheral axonal polyneuropathy. Autonomic function tests suggested pandysautonomia of postganglionic lesion. Spinal fluid protein was 16mg/dl on admission, but was increased up to 80mg/dl without pleocytosis on the twenty first hospital day. Recovery was poor over a four -month period and she died of disseminated pneumonia.

5.Effect of Intertrochanteric Femoral Derotational Osteotomy on Sagittal Plane Kinematic and Kinetic Study of the Hip and Pelvis in Spastic Cerebral Palsy: A Preliminary Report.

To evaluate the effect of intertrochanteric femoral derotational osteotomy(IFDO) on the sagittal plane kinematics and kinetics of the hip and pelvis in spastic cerebral palsy, we compared the preoperative and post-operative results of 3 dimensional gait analysis. Intertrochanteric femoral derotational osteotomy alone without psoas procedure was performed in 34 hips of cerebral palsy patients with increased femoral anterversion regardless of preoperative dynamic or static hip flexion contracture. Those who had other concomitant hip procedures were excluded. There were 24 diplegics and 10 hemiplegics. Mean age of the patients at the time of operation was 9.1 years (range, 4.9 to 22). They were divided into three subgroups according to the degree of dynamic hip flexion contracture; 13 patients with normal hip extension in terminal stance (group I), 13 patients with maximum hip extension in terminal stance between 0 and 15 degrees (group II), and 8 patients with maximum hip extension in terminal stance of more than 15 degrees (group III). The gait analysis included clinical assessment, video-taping, 3D-kinematics and kinetics, and dynamic EMG. Linear parameters of gait, kinematic parameters, sagittal plane hip moment parameters, and total hip energy parameters were compared. Postoperatively, cadence and double support time decreased, whereas walking velocity and stride length increased in all groups. Maximum and minimum pelvic tilt were improved in all groups. The range of pelvic tilt improved in all groups except group I. Hip flexion-extension curve shifted into extension in all groups. Marked improvement in maximum hip extension in stance and the range of hip motion were observed in all groups. The sum of extensor moment decreased, whereas that of flexor moment increased significantly in all groups. The conversion timing from extensor to flexor moment significantly improved in group I and II. The decrease of power generations of Hl and the increase of power absorptions of H2 were significant in all groups. However, there were no significant changes in power generation of H3 in all groups. The changes of peak power generation timing of H3 was not consistent among the groups. Femoral derotational osteotomy at the intertrochanteric level brings the lesser trochanter forward resulting in iliopsoas lengthening effect. We found significant improvement of the sagittal plane kinematics and kinetics of the hip and pelvis when IFDO alone was performed without psoas tenotomy. The psoas lengthening procedure may be considered secondarily at the time of hardware removal after full evaluation of the psoas lengthening effect.

6.The Change of Axial Length after Laser in-Situ Keratomileusis.

PURPOSE: We prospectively conducted this study to evaluate the effect of laser in-situ keratomileusis (LASIK) on axial length because axial length is an important varible for IOL power calculation. METHODS: One hundred three eyes of fifty two myopic patients had measurements of axial length, ultrasonic central pachymetric readings and otherwise preoperatively and 3 months after LASIK. RESULTS: The axial length and central corneal thickness were reduced statistically significant 3 months after LASIK by 66+/-0.28 micrometer and 57+/-23 micrometer respectively (P<0.05). IOL power is about 15.8+/-3.25 diopter preoperatively and 15.9+/-3.21 diopter postoperatively. The mean difference is -0.13 diopter.(p value>0.05) CONCLUSIONS: But the reduction of axial length after LASIK is so small that very small influence is expected on IOL power calculation for the patient who had previous LASIK. So when preoperative IOL power calculation is done using historical method, post-LASIK axial length can be replaced for pre-LASIK axial length to calculate proper IOL power for cataract surgery.

7.Three Cases of Hepatitis Related to the Use of Famotidine and Ranitidine.

H2-receptor blockers are widely used for therapy of peptic ulcer disease and gastroesophageal reflux disease. H2-receptor blockers infrequently cause adverse hepatic effects, and when they occur they are usually asymptomatic. There are several previous reports of liver injury related to ranitidine. Until now, only two cases of acute hepatitis associated with the use of famotidine were reported in the world. We report three cases of clinical hepatitis that followed administration of famotidine (2 cases) and ranitidine (1 case). First, a 54-year-old woman received famotidine, 40mg, daily for treatment of erosive gastritis. After 6 weeks of treatment with famotidine, jaundice and itching sense developed. Second, a 45-year-old man was hospitalized for jaundice. He had a long history of duodenal ulcer and had been intermittently treated with famotidine. He had 6 weeks of treatment with famotidine prior to admission. Third, a 19-year-old woman was hospitalized for nausea, vomiting and urticaria. She had a history of acute hepatitis B virus infection and was discharged 4 weeks prior to readmission. She had been received ranitidine, 300 mg, daily for treatment of gastritis. After 17 days of drug ingestion, whenever she had taken her medication, she developed these symptoms of nausea, vomiting and urticaria. Other causes of hepatitis were ruled out and all patients recovered after discontinuation of drug ingestion.

9.The effectiveness of the intervention in the workplace on patients suffering from Cervicobrachial disorder.

OBJECTIVES: In order to reveal the effectiveness of the intervention in the workplace on patients suffering from cervicobrachial disorder (CBD), we reexamined 96 workers out of 137 workers suffering from CBD on the assembly lines of an electric manufacturing factory by using questionnaires, physical exams and neurological exams. METHODS: Analyses were made of the characteristics of the 96 workers of this company suffering from 1994 to 1996 with special concern for differences before and after the improvement of the working environment. After reexamination of the 96 patients the following results were obtained. RESULTS: We observed an improvement in the 10 most common subjective symptoms of CBD. These symptoms included general fatigue, pain on neck and shoulders, dullness of the upper extremities, back pain and discomfort and a tingling sensation in the hands and wrists (p < 0. 05). The percentage of workers who complained muscle tenderness of neck and shoulder were reduced (p < 0.05). the percentage of workers with abnormal Morley examination was reduced from 28. 5% to 2. 100 (p < 0.01), and of workers with abnormal EMG finding went from 12. 4% to 2. lao (p < 0. 01) and of grade 3 and grade 4 CBD was reduced from 42. 3% and 18. 3% respectively to 33. 3% and 2. ba (p < 0. 01). Grade 3 and over laborers of CBD had higher scores in somatization, anxiety, depression, hostility, interpersonal sensitivity, phobic anxiety, obsessive-compulsive, neurasthenic, hypochondriacal than other sufferers in the SCL-90-R exam (p < 0. 01). CONCLUSIONS: in conclusion intervention had effectiveness on subjective symptoms of neck and shoulder and decreasing abnormal finding of muscle tenderness, neurological examination and EMG.

10.SVC Syndrome Caused by Leiomyosarcoma of SVC: A Case Report.

Among the causes of SVC syndrome,intraluminal tumor,especially the leiomyosarcoma is very rare.We report a 39 year old female patient who had been suffering from headache and facial edema for 6 weeks before admission.On physical examination,facial edema and venous engorgement on upper extermities and upper chast wall were showed.The chest CT scan and SVC cavogram showed a long intraluminal mass lesion resulting in a near total obstruction of the SVC.Surgery was performed through median sternotomy.For complete resection of the tumor and thrombus,we used partial and total CPB.The follow up SVC cavogram revealed no abnormality 14 months after the operation.
