2.Treatment of Herpes Simplex Keratitis with IDU and Cryocautery.

The effect of IDU and cryocautery in the treatment of Herpes Simplex Keratitis was presented. The cryoapplicator was a model of Krwawlcz probe (2 mm in diameter) which was refrigerated in a mixture of dry ice and methyl alcohol. The applicator was appJied on the lesions 7 seconds each and antibiotics and atropine solutions were instilled. A good result was experienced in the treatment of several cases of dendritic kenititis by freezing the corneal lesions by means of cryoappJicator, which had not been responded to IDU therapy speedy. The deep neovascularization of the cornea in a case of stromal keratitis was disappeared after cryocautery for the treatment of the superficial corneal ulcers.

3.The Role of Serum Pepsinogen in Detection of Gastric Cancer.

PURPOSE: This study was done to determine the usefulness of serum pepsinogen (PG) levels as a screening method for gastric cancer, and to assess the relationships between serum PG and clinicopathologic factors of gastric adenocarcinoma. MATERIALS AND METHODS: Serum PG concentrations were measured in 94 subjects who were classified into (a) a control group (50 subjects) without abnormal endoscopic finding on a health checkup, or (b) a gastric cancer group (44 subjects) who had surgery at Daegu Catholic University Hospital between Nov. 2008 and May 2009. Receiver operator characteristic curves were utilized to select the most suitable test. Using different cutoff points, sensitivity and specificity were calculated. We compared preoperative serum PG levels with several clinicopathologic findings for patients with gastric adenocarcinoma. RESULTS: The Serum PG I:II ratio was the most useful as a screening test. The sensitivity and specificity of PG screening for gastric cancer were, respectively, 81.8% and 82%. The cut off point correlated with the type of intestinal cancer (Lauren classification; P=0.003), tumor stage (P=0.001), and gastric adenocarcinoma with peritumoral chronic atrophic gastritis (P=0.036). CONCLUSION: Serum PG levels were found to be a potentially useful screening test and to correlate with clinicopathologic factors in gastric cancer patients. But, in order to use serum PG found in a health checkup for gastric cancer as a clinical application a large scale study is recommended.

5.Closed Reduction and External Fixation of Displaced Fractures of the Proximal Humerus.

PURPOSE: This is a retrospective study to analyze the functional results of closed reduction and external fixation of unstable fractures of the proximal humerus. MATERIALS AND METHODS: Ten unstable proximal humerus fractures were managed with closed reduction and external fixation in which other operative methods are not proper due to comminution, osteoporosis or poor general condition of patients. 4 cases of 2-part and 6 cases of 3-part fracture were included. Radiologically union of fracture, malunion and the evdence of avascular necrosis of humeral head were assessed and the functional results were analyzed with Neer scoring system. RESULTS: Radiologically all fractures were healed but in 2 cases malunion was resulted because of reduction loss in proximal fragment. Pin site infection was developed in 7 cases and oral antibiotics were needed. The functional results were excellent in 4, satisfactory in 3 and unsatisfactory in 3 cases. 2 cases with malunion and one case with lack of postoperative cooperation resulted in functionally unsatisfactory. CONCLUSION: External fixation is an alternative method in the treatment of unstable proximal humerus fractures in which open reduction or percutaneous pinning are not proper due to comminution, osteoporosis or poor general condition of patient.

7.Characteristic Symptoms of Chronic Prostatitis/Chronic Pelvic Pain Syndrome.

PURPOSE: Chronic prostatitis/chronic pelvic pain syndrome (CPPS) presents various symptoms, such as various pains, voiding and sex-related symptoms. To elucidate the characteristic symptoms of CPPS we compared BPH patients with healthy men. MATERIALS AND METHODS: We obtained questionnaires from 530 men, consisting of three groups: 243 CPPS patients aged 40 or below with small sized prostate (<25ml); 115 BPH patients aged 45 or older with non-small sized prostate (> OR =25ml); and 172 healthy men aged below and equal 40 years without pyuria. We compared various pains and voiding symptoms among the three groups, and the sex-related symptoms between 145 CPPS patients and 64 healthy men who had been married at least 6 months, and had regular sexual intercourse with their wife. RESULTS: In comparison with healthy men, the CPPS patients had a higher prevalence of the various pains and most of the voiding symptoms. The CPPS patients had a higher prevalence of pain over the penis, urethra, perineum and prostate gland compared to the BPH patients (p<0.05). There was no statistical significance in voiding symptoms between the CPPS and BPH patients. In respect to the sex-related symptoms, the CPPS patients had a higher prevalence of decreased maintenance time, rigidity at erection, libido, orgasmic feeling and a more painful ejaculation than the healthy men (p<0.01). The frequencies of sexual intercourse and orgasmic sensation following ejaculation were slightly decreased, bur with no statistical significance (p>0.05). CONCLUSIONS: The CPPS patients showed a higher prevalence of various pains and voiding symptoms than the healthy men. Pains over the penis, urethra, perineum and prostate gland are characteristic in CPPS patients. Both the CPPS and BPH patients felt trouble due to the voiding symptoms. The CPPS patients had a higher prevalence of most of sex-related symptoms, but had similar frequencies of sexual intercourse and orgasmic sensation following ejaculation compared to the healthy men.

8.Evaluation of Serum Magnesium Concentration Changes in Orthotopic Liver Transplant Patients.

BACKGROUND: During orthotopic liver transplantation, magnesium monitoring has been done frequently because of its important role in the cardiovascular system. Generally hypomagnesemia may occur frequently during an operation, but some investigators reported serum magnesium levels returned to normal after reperfusion which is different from our result. Therefore, this study was done to confirm the changes of serum magnesium in the postanhepatic stage and also to confirm the need for prophylactic magnesium administration. METHODS: Thirteen patients plasma magnesium concentrations were measured 8 times during the operation on each patient. We also checked the total transfusion amount and required CaCl2 amount in every patient. The significance of all data were evaluated with a paired t-test and correlation method. RESULTS: Serum magnesium levels were significantly decreased after the beginning of the postanhepatic stage during an operation (P < 0.05). Correlation between transfused blood amounts and CaCl2 requirements showed significant correlation (r: 0.709), but no correlation between magnesium concentrations and transfused blood amounts was found. CONCLUSIONS: We concluded that serum magnesium concentrations were significantly decreased especially in the postanhepatic stage and an appropriate amount of a magnesium supply may be needed.

9.Effect of Heel Raise Exercise with NMES on Peroneus Longus Muscle Strength and Postural Control Ability in Subjects with Functional Ankle Instability: Randomized Controlled Trial

Purpose:
This study examined the effects of heel raise exercises combined with neuromuscular electrical stimulation (NMES) on the muscle strength and postural control ability of subjects with functional ankle instability (FAI).
Methods:
Twenty-two subjects with FAI participated in this study. They were assigned randomly to two groups: 11 each in the NMES and the sham-NMES groups. Heel raise exercise was applied, and NMES electrodes were attached to the peroneus longus muscles. The NMES group applied NMES during the heel raise exercise. NMES was applied for 20 minutes during the heel raise exercise. The heel raise exercise was performed four times a week for five weeks. The muscle strength and balance error scoring system (BESS) were measured before and after the intervention.
Results:
A comparison of before and after the intervention within the groups revealed improved muscle strength in the NMES and Sham-NMES groups, but the BESS was improved under all conditions only in the NMES group. The Sham-NMES group showed no improvement in the unstable support surface. Furthermore, when comparing the amount of change before and after the intervention between the groups, there were significant differences in the total score and unstable support in the BESS and muscle strength.
Conclusion
NMES had a positive effect on the functional activities of the functional ankle instability subjects, such as balancing on an unstable support surface during postural control and increasing muscle strength.
