1.A Case of Antipsychotic-Regression Syndrome in Haloperidol Treated Tourette's Syndrome.

Authors report a case of separation anxiety disorder, which developed as a side effect during haloperidol treatment of Tourette syndromes(TS). In this case, 14 years old boys developed attention deficit symptoms during his infancy. At 4th grade of primary school, he developed vocal tic, motor tic, and coprolalia. With 5mg/day of haloperidol treatment his symptoms of TS were subsided. During the treatment, he developed features of separation anxiety disorder, including dependence, pleading, clinging, and sadness. Symptoms of attention deficit and separation anxiety disorder were improved by 25mg/day of imipramine treatment. During haloperidol treatment of TS, careful observation may be needed whether separation anxiety disorder-like symptom develops.

2.The effect of respiratory syncytial virus infection on neutrophil adherence to airway epithelial cells.

BACKGROUND: Respiratory syncytial virus (RSV) is the most common cause of lower respiratory infection in infants and young children, but the pathogenesis of RSV-induced inflammation is not well defined. MATERIAL AND METHOD: In order to examine the potential interactions between virus-infected airway epithelial cells and neutrophils, we studied the ability of neutrophils to adhere to yirus-infected airway epithelial cell monolayers by myeloperoxidase assay. Also we measured the ability of airway epithelial cells to secrete interleukin-8(IL-8) and inter-cellular adhesion molecule-1(ICAM-1) in virus-infected airway epithelial cell cultures by enzyme-linked immunosorbent assay(ELISA). The degree of IL-8 and ICAM-1 gene expression in the RSV-infected BEAS-2B cell cultures were analyzed by reverse transcription-polymerase chain reaction(RT-PCR). RESULTS: The RSV-infected BEAS-2B cell resulted in significantly enhanced level of neutrophil adherence compared to the uninfected control(p (0.001). IL-8 and ICAM-1 production significantly increased by RSV infection(p<0.05). There was a significant positive correlation between neutrophil adherence and IL-8 level(r=0.73, p=0.002), and ICAM-1 level (r=0.843, p=0.001) in RSV-infected cells. The degree of both IL-8 and ICAM-1 mRNA expression increased in the RSV-infected cells compared with the uninfected ones. CONCLUSION: These results suggest that RSV infection significantly enhances the production of IL-8 and ICAM-1 in airway epithelial cells which then results in increased neutrophil adherence.

5.Transscleral Diode Laser Photocoagulation for Retinopathy of Prematurity: Five Years' Experience.

PURPOSES: We analysed the five-year outcomes and associated complications of transscleral diode laser photocoagulation (TSDLP), used to treat acute proliferative retinopathy of prematurity (ROP), to evaluate the efficacy and safety of this therapy. METHODS: We performed a retrospective analysis of the records of 163 eyes of 82 infants with acute ROP, who were treated with TSDLP by a single surgeon. Follow-up periods ranged from 15 to 278 weeks. RESULTS: The outcome was favorable in 158 eyes (96.9%) and unfavorable in 5 eyes (3.1%) of the 163 eyes that were treated. Vitreous hemorrhage was found to be an indicator of poor prognosis. Of the complications noted, symblepharon was observed in three eyes, hyphema was observed in four eyes, and a complicated cataract was observed in one eye. CONCLUSIONS: The results suggest that TSDLP is a safe and effective in the treatment of ROP. Vitreous hemorrhage should be regarded as a sign of poor prognosis.

6.Short-term Efficacy and Safety of Ranibizumab for Neovascular Age-related Macular Degeneration in the Real World: A Post-marketing Surveillance Study

PURPOSE: To investigate the short-term efficacy and safety of ranibizumab in the routine clinical setting in patients with neovascular age-related macular degeneration and to analyze the associated factors for visual outcome. METHODS: This was a post-hoc analysis of a ranibizumab regulatory post-marketing surveillance study in which 4,136 patients were enrolled and followed for 12 weeks. Change in best-corrected visual acuity (BCVA), size of choroidal neovascularization, and the presence of hemorrhage and exudate were analyzed and the association between BCVA change and baseline characteristics were investigated. Data on ocular and systemic adverse events were collected. RESULTS: Mean BCVA improved significantly and mean BCVA change was the logarithm of the minimal angle of resolution 0.13 ± 0.01 (p < 0.001). A lower baseline BCVA and younger age were significant predictive factors for visual improvement or maintenance (≥0 lines). For greater visual acuity gain (≥3 lines), no treatment history, lower baseline BCVA, younger age, and classic-type choroidal neovascularization were significant predictive factors. No new safety signals were found. CONCLUSIONS: In this study, conducted in real-world clinical practice with a large number of neovascular age-related macular degeneration patients, visual and anatomical outcomes improved significantly after three monthly ranibizumab treatments. Treatment-naive patients had a higher chance of greater visual gain (≥3 lines) than non-naive patients.

7.Clinical Results of Combined Surgery: Pars Plana Vitrectomy, Phacoemulsification and Posterior Chamber Intraocular Lens Implantation.

PURPOSE: To evaluate the clinical outcomes and complications of combined phacoemulsification and vitrectomy surgery in patients who have vitreoretinal diseases with a concomitant cataract. METHODS: Combined phacoemulsification-vitrectomy surgery was performed in 50 eyes of 45 patients who had various vitreoretinal diseases and a significant cataract that is considered to disturb the postoperative visual rehabilitation or fundus examination. Main outcome measures were visual acuity and intraoperative and postoperative complications. RESULTS: We performed combined surgery consisting of pars plana vitrectomy (PPV), phacoemulsification, and posterior chamber intraocular lens (PC-IOL) implantation. Intraocular lens were inserted in the bag with one case of exception inserted in the ciliary sulcus. Thirty-seven eyes (74%) had improved postoperative visual acuity, in 9 eyes (18%) the acuity was unchanged and 4 eyes (8%) it deteriorated. Intraocular pressure elevation, recurrent vitreous hemorrhage, macular edema, retinal detachment and endophthalmitis were considered to be clinically significant postoperative complications. CONCLUSIONS: The results of this study suggest that combined phacoemulsification-vitrectomy surgery is a viable option in the management of most posterior segment diseases in the presence of cataract.

The appendiceal mucocele is very rare disease of 0.2% incidence. About 24% of patients are asymptomatic and symptomatic patients present with pain in the right lower quadrant of abdomen in 64%, plapable maas in the right lower quadrant of abdomen in 50%, and rarely, melena, hematochezia, anemia, diarrhea, malaise, and abdominal distension. The gastrointestinal bleeding may be presented in the patient with intussusception, but the massive bleeding is generally absent. We report a case of appendiceal mucocele accompanying with gastrointestinal bleeding and review of literature.

10.Concurrent Malignant Lymphoma of the Colon and Small Bowel as the Primary Origin.

Primary gastrointestinal lymphoma primarily confined to gastrointestinal tract is relatively rare diaease. Although the lymphoma primarily involved colorectum or small bowel was commonly reported than primary gastric lymphoma in Korea, the concurrent primary lymphoma of colon and small bowel is very rare in reported cases. We report a case of combined primary malignant lymphoma of the jejunum and cecum, who was admitted due to melena and anemia. She had right hemicolectomy and end-to-side ileotransverse colostomy at 2 months ago, due to primary colon lymphoma. The laparotomy was performed and identified hard, 2 x 3 cm sized masses on distal 25 cm, 80 cm and 150 cm from the Treitz ligament. The histology of these small ma showed histiocytic lymphoma as same as cecal specimen. In general, the small bowel lymphoma shaws poor prognosis than gastric or colorectal lymphoma due to diffieulty in diagnosis and late symptoms, but this problem could be resolved through the knowledge about primary gastrointestinal lymphoma and the development of diagnostic methods.
