1.Changes of Urinary Prostaglandin in Female Patients with Overactive Bladder.

PURPOSE: Prostaglandin levels in the urinary bladder can be affected by lesions of the bladder.Also, this change can be noted in the urine. This study investigated the change in urinary prostaglandin in female patients with overactive bladder. MATERIALS AND METHODS: The study groups included 80 female patients with overactive bladder and 15 healthy female controls. Evaluation included history taking, urinalysis, voiding diary and urodynamics. The urinary concentrations of prostaglandin E2(PGE2) and prostaglandin F2alpha(PGF2alpha) in voided urine were analyzed by enzyme linked immunosorbent assay(ELISA) and these results were compared with the control group. RESULTS: From an analysis of the voiding history, the most common complaint was urgency. Also, the capacity for first voiding sense and maximal capacity were decreased in patients from the urodynamic study. Bladder instabilities were noted in 29 patients. The urinary concentrations of PGE2 and PGF2alpha were significantly increased in the patient group compared with the control group. PGE2 level in patients was proportional to the capacity for first voiding sense and maximal capacity. However, PGF2alpha was not correlated with clinical symptoms or measurements in urodynamic studies. CONCLUSIONS: PGE2 and PGF2alpha may play a role in lower urinary tract symptoms, and their changes can be detected in urine sample. Therefore, changes in urinary prostaglandin level may be a useful marker to evaluate lower urinary tract symptoms.

3.An Ultrastructural Study of Recovery of Photoreceptor Layer from Visible Light-Induced Damage.

Visible light can induce damage to the susceptible retina and its injurious effect has been studied morphologically and functionally in detail. In order to evaluate the regenerative processes of the retina damaged by visible light in correlation with the degree of the cellular damage, the rat retina was exposed to blue visible light (400-500nm) for 30, 60 and 120 minutes, and examined electron microscopically, immediately and 1, 2, 3, 4, 5 weeks after the exposure. The results were as follows. The more increased time of exposure by visible light, the more damage to the photoreceptor, and the more delay in the recovery. We observed the recovery of damage was generally slow, and the pigment epithelium necessary for regeneration of discs was not destroyed in spite of long term exposure of visible light.

4.The Genetically Modified Polysialylated Form of Neural Cell Adhesion Molecule-Positive Cells for Potential Treatment of X-Linked Adrenoleukodystrophy.

PURPOSE: Cell transplantation of myelin-producing exogenous cells is being extensively explored as a means of remyelinating axons in X-linked adrenoleukodystrophy. We determined whether 3,3',5-Triiodo-L-thyronine (T3) overexpresses the ABCD2 gene in the polysialylated (PSA) form of neural cell adhesion molecule (NCAM)-positive cells and promotes cell proliferation and favors oligodendrocyte lineage differentiation. MATERIALS AND METHODS: PSA-NCAM+ cells from newborn Sprague-Dawley rats were grown for five days on uncoated dishes in defined medium with or without supplementation of basic fibroblast growth factor (bFGF) and/or T3. Then, PSA-NCAM+ spheres were prepared in single cells and transferred to polyornithine/fibronectin-coated glass coverslips for five days to determine the fate of the cells according to the supplementation of these molecules. T3 responsiveness of ABCD2 was analyzed using real-time quantitative polymerase chain reaction, the growth and fate of cells were determined using 5-bromo-2-deoxyuridine incorporation and immunocytochemistry, respectively. RESULTS: Results demonstrated that T3 induces overexpression of the ABCD2 gene in PSA-NCAM+ cells, and can enhance PSA-NCAM+ cell growth in the presence of bFGF, favoring an oligodendrocyte fate. CONCLUSION: These results may provide new insights into investigation of PSA-NCAM+ cells for therapeutic application to X-linked adrenoleukodystrophy.

5.Expression of Oncogene Product in the Colorectal Carcinoma.

The expression of p62"' and p2l can be seen in many solid tumor, but the pattern and incidence of expression were different according to organ, countries, and examiners, thus it is not definitely defined. Total 67 colorectal carcinoma in paraffin sections are analysed by immunohistochemically for evaluation of the p62and p21- expression according to the age, sex, chief complaints, location, differentiation, modified Dukes stage, using the specific monoclonal antibodies. The results were summarized as follows : The age of patients ranged from 32 years to 82 years. The mean age was 57.6 years. The expression of p 62y' and p21 was not correlated with age. Male was 29 cases(43.3%) and female was 38 cases(56.7%). The male to female ratio was 1:1.31. The and 95.0% in moderately differentiated adenocarcinoma, 90.0% and 90.0% in poorly differentiated <,;, adenocarcmoma, 100.0% and 100.0% in mucinous carcinoma. The positive rate of p62!and and p2l' was 94.1% and 88.2% in Dukes stage B,, 96.0% and 96.0% in Dukes stage B,, 100.0% and 100.0% in Dukes stage C,, 100.0% and 88.9% in Dukes stage C, and 100.0% and 100. o% in Dukes stage D. The expression of p62 y' in metastatic colorectal carcinoma showed diffuse and strongly positive reaction than primary colorectal carcinoma. The expression of p21 in primary colorectal carcinoma showed diffuse and strongly positive reaction than metastatic colorectal carcinoma.

7.A Case of Trichoadenoma.

Trichoadenoma is a rare cutaneous tumor that was first described by Nikolowski in 1958. It usually occurs as a single tumor on the face and its size varies from 3 to 50mm in diameter. It may arise any time during adult life. This tumor is less mature than trichofolliculoma and more differentiated than trichoepithelioma, and it is considered as a benign tumor with differentiation toward the infundibular portion of the pilosebaceous canal. In this report, we describe a 61-year-old women who developed trichoadenoma on the left shoulder of 3 years duration. The lesion was asymptomatic and had grown slowly. Histopathological findings showed numurous horn cysts with central keratinous material and solid tumor islands reaching to the deep dermis. Immunohistochemical studies showed negative findings for eccrine differentiation.

8.Correction of Frontal Sinus Hypertrophy by Frontal Bone Remodeling and Forehead Lift.

A patient with frontal sinus hypertrophy complained about an bossing in eyebrow region, increased width of the upper nasal pyramid, inferior displacement of the eyebrow and deep furrows in his forehead, etc. This case report illustrated a method of correction for frontal sinus hypertrophy. Surgery was performed via the bicoronal incision. After the anterior wall of the frontal sinus was removed, we tried a frontal bone remodeling by dividing it into three pieces, trimmming each piece at the margin and fixed with microplate. Bilateral thick bony portions around the frontal sinus were shaved with a surgical burr. At the same time, a 1cm-forehead lift and frontal muscle partial resection was performed. A satisfactory aesthetic result with a smooth and wide forehead could be obtained by employing this technique.

10.Effects of Intravesical Instillation of Cyclooxygenase-2 Inhibitor on Cyclophosphamide-induced Overactive Bladder.

PURPOSE: This study was performed to investigate the effects of intravesical instillation of cyclooxygenase-2 (COX-2) inhibitors on the cyclophosphamide-induced overactive bladder. MATERIALS AND METHODS: The 40 Sprague-Dawley rats were divided into 3 groups; the control group, the overactive group, and the COX-2 inhibitor treated group. Cystometrograms (CMG) were performed and the contraction interval, inter-contraction interval, contraction time and contraction pressure were measured. After CMG, the bladders of each group were dissected out, and weighed. RESULTS: On CMG, the contraction interval and inter-contraction interval for the overactive group were significantly decreased compared with the control group. After treatment with COX-2 inhibitor, the contraction interval and inter-contraction interval were significantly increased compared with the overactive group (p<0.05). The contraction time in the overactive group was significantly increased compared with the control group, and it was also decreased in the COX-2 inhibitor treated group compared with the overactive group (p<0.05). The contraction pressure in the overactive group and the COX-2 inhibitor treated group were significantly increased compared with the control group. There were no significant differences between the overactive and COX-2 inhibitor treated groups. The bladder weights of the overactive and COX-2 inhibitor treated groups were significantly increased compared with the control group (p<0.05). CONCLUSIONS: Intravesical instillation of COX-2 inhibitor can suppress cyclophosphamide-induced detrusor overactivity. Therefore, intravesical instillation of COX-2 inhibitor may be considered as a possible treatment for the overactive bladder.
