1.A Case of Mauriac Syndrome.

In 1930s, Mauriac described a syndrome in diabetic children consisting of stunted growth, hepatomegaly, and delayed puberty. This syndrome was related to poorly controlled diabetes of long duration. A 14-year-old girl, who had been diagnosed as insulin dependent diabetes mellitus three years ago but had not been well controlled for diabetes, visited to hospital because of short stature, delayed puberty, and visual disturbance. On physical exam her height was 146cm(<3 percentile) and her weight was 34kg(<3 percentile) and sexual maturation was delayed as Tanner stage I. The liver was 3FB palpable below the right subcostal margin. She had hyperglycemia and elevated plasma cortisol. She was diagnosed as Mauriac syndrome. On ophthalmologic examination, the cataracts were observed on both eyes and she had also peripheral neuropathy on lower extremities. Her blood glucose was controlled strictly by regular insulin during admission, and her weight was increased and hepatomegaly was improved at discharge, two months later. We report a case of Mauriac syndrome with a brief review and related literatures.

3.Change of Values of Intraocular Pressures Measurements after Laser In Situ Keratomileusis.

There have been reports of a reductioin in the intraocular pressure(IOP) measured with the Goldmann applanation tonometer(GAT) in eyes that have undergone excimer laser photorefractive keratectomy(PRK). The authors determined whether laser in-situ keratomileusis(LASIK) for myopia also affected GAT readings. The IOP was measured with the GAT before LASIK and 1 and 3 months postoperatively on 45 eyes of 30 patients. Measurements were performed at the central and temporal parts of the cornea; the paired Student`s t test was used for statistical analysis. The mean preoperative IOPs at the central and peripheral cornea were 13.2+/-2.5mmHg and 13.2+/-2.5mmHg, respectively. Three month after LASIK, IOPs at the central and peripheral cornea decreased to 8.9+/-1.8mmHg and 12.3+/-2.3mmHg, respectively. Similar to PRK, the GAT readings from the central cornea after LASIK for myopia were significantly lower than the preoperative IOPs(P<0.001) and the postoperative GAT readings from the peripheral cornea(p<0.001).

4.Hypotony with Cyclodialysis after Blunt Trauma to the Eye.

The hypotony with cyclodialysis after blunt trauma to the eye rarely happens and the cyclodialysis is diagnosed by finding the cleft in the anterior chamber anglel It is treated with cycloplegics conservatively or with surgery. In this study, the authors reviewed the hyphema, the onset of hypotony, the size of cyclodialysis cleft and transient scleral buckling that is one of the surgery in 7 eyes of 7 patients . The hyphema was seen in all cases after blunt trauma to the eyes and the duration between the trauma and the onset of hypotony was average 7.2 days. The traumatic cyclodialyses clefts were half an clock hour in 3 patients and 1 to 4 clock hours in 4 patients . Six patients had an initial conservative treatment, 3 patients required surgical treatment, scleral buckling, for 3 weeks for the full recovery of intraocular pressure.

5.Henoch-Scholein Purpura Presenting with Acute Abdominal Pain Preceding Skin Rash: Review of 23 Cases .

PURPOSE: For the early diagnosis of Henoch-Schonlein purpura(HSP) presenting with acute abdominal pain preceding skin rash. METHODS: The clinical, endoscopic and radiological records of 23 cases of HSP, presenting with gastrointestinal symptoms preceding skin rash were reviewed. RESULTS: The intervals from the onset of abdominal pain to the development of the skin rash were one day to 30 days(median five days), most of them were within two weeks. The presenting abdominal symptoms were abdominal pain(23 cases), vomiting(16 cases), hematochezia or melena(eight cases) and hematemesis(three cases). The abnormal endoscopic findings include coalescing erythematous lesions, areas of submucosal hemorrhage and superficial erosions and ulcers. The upper gastro intestinal endoscopy showed the abnormalities in 21 of 23 cases, which were observed in the duodenum(21 cases), the stomach(12 cases) and the esophagus(one case). Duodenitis with hemorrhage and/or erosions in the descending duodenum was the sole endoscopic abnormality in two cases and was the most marked finding in three cases. Sigmoidoscopy showed the abnormalities in six of eight cases. The abdominal ultrasonogram showed abnormalities in 12 of 17 cases, which included small bowel wall thickening(eight cases) and intramural hemorrhage(three cases). Recurrences after three months of symptom free intervals developed in four cases; three of them had persistent nephritis beyond one year. CONCLUSION: The erosive hemorrhagic duodenitis in the descending duodenum in the upper endoscopy and the small bowel wall thickening in the abdominal ultrasonogram can be useful findings in the diagnosis of HSP presenting with acute abdomen.

We report a case of hepatoma with duodenal metastasis in a 53 year-old male patient. Hepatoma was confirmed by fine needle aspiration cytology technique, and duodenal metastasis by gastrofiberscopic biopsy. Duodenal metastasis of hepatoma is rare. We briefly review the role of fine needle aspiration cytology technique in diagnosis of hepatoma.

8.A Case of Solar Retinopathy.

The damaging effect o flight on the eye has been recognized for centuries. Solar retinopathy is a well-recognized clinical entity of retinal damage caused by direct or indirect exposure to the sun. Symptoms of solar retinopathy include decreased vision, metamorphopsia, micropsia, and central or paracentral scotoma. Typical solar retinopathy presents clinically as a small yellowish-white foveolar lesion. The authors experienced a case of solar retinopathy which occurred in a 25-year-old female on both eyes upon gazing at the sun for 4 hours.

9.Ambulatory Blood Pressure and Heart Rate of the Workers Exposed to Industrial Noise.

OBJECTIVES: This study was carried out to evaluate the effects of industrial noise on blood pressure and heart rate. METHODS: Resting blood pressure, hearing loss, and general characteristics of the 102 subjects who were engaged in a factory in Pusan were measured from March to June for two years, in 1998 and 1999. With noise dosimeter, noise exposure level was measured from 8 A.M. to 4 P.M. Ambulatory blood pressure and heart rate were also measured every 30 minutes from 8 A.M. to 10 P. M. RESULTS: Controlling for age, smoking, and Quetelet's index, in subjects of under 40 years old exposed to higher than 85dBA, noise exposure and systolic blood pressure had a statistically significant correlation, and the same result was obtained in all subjects. The daily variability of ambulatory blood pressure and heart rate were observed in older than 40 years old group, but only heart rate in under 40 years old. CONCLUSIONS: The blood pressure and heart rate would be elevated when the workers exposed to noise. And in under 40 years old, the systolic blood pressure was elevated to the workers exposed to higher than 85dBA.

10.Effect of Diet and Apoliprotein E (Apo E) Polymorphism on the Variation of Serum Lipid Profile in Korean Males.

BACKGROUNG AND OBJECTIVES: Diet is the basic and principal therapeutic modality for hyperlipidemia. However, diet therapy alone showed variable responses in lowering lipid levels in different studies. This research is to prove the effect of diet and Apo E polymorphism on the variation of serum lipid profile in Korean males. MATERIALS AND METHODS: To evaluate the gene-diet interation, serum total cholesterol (Chol), triglycerides (TG), HDL-cholesterol and LDL-cholesterol (HDLc, LDLc), lipoprotein (a) (Lp (a)), and fasting blood glucose (FBS) were measured with Apo E genotyping in vegeterians (group A, n=154) and in healthy Korean male adults (group B, n=150) of similar mean age (50.1 vs. 49.3). RESULTS: Lipid profiles showed significantly lower levels in group A compared to group B (Chol 168.3+/-30.5 mg/dL vs. 181.3+/-33.4 mg/dL, p<0.001;TG 131.0+/-62.9 mg/dL vs. 149.4+/-76.7 mg/dL, p=0.023;HDLc 56.0+/-11.0 mg/dL vs. 56.9+/-11.5 mg/dL, p=0.509;LDLc 92.5+/-28.1 mg/dL vs 100.6+/-29.9 mg/dL, p=0.016;Lp (a) 22.1+/-14.6 mg/dL vs. 26.9+/-13.8 mg/dL, p=0.004;FBS 85.1+/-14.1 mg/dL vs. 102.7+/-16.6 mg/dL, p<0.001). The Apo E genotyping showed Epsilon3/3, 64.1%; Epsilon3/4, 20.7%;Epsilon2/3, 11.8%;Epsilon2/2, 1.3%;Epsilon4/4, 0.6% in the combined groups. The distribution was similar in both groups. Chol and LDLc were significantly (p<0.05) higher in Epsilon3/4 allele group compared to other allele groups among non-vegetarians. On the other hand, Chol and LDLc were significantly (p<0.01) lower in vegetarians compared to non-vegetarians only in Epsilon3/4 allele group. CONCLUSION: Vegetarian diet significantly lowered Chol, TG, LDLc, Lp (a) and FBS levels. Significant lipid lowering effect of vegetarian diet was noted in Apo E allele Epsilon3/4 group which had significantly higher Chol and LDLc levels without diet intervention. These data suggest that the influence of diet on serum lipid profiles differ according to apo E genotypes.
