2.Successful Treatment of Rectus Femoris Calcification with Ultrasound-guided Injection: A Case Report.

Painful periarticular calcification most commonly occurs within the rotator cuff of the shoulder and rarely around the elbow, hip, foot, and neck. As acute inflammatory reaction develops, severe pain, exquisite tenderness, local swelling, and limitation of motion with pain occur. In case of calcific tendinitis of the shoulder, it can be easily diagnosed according to the symptoms and with x-ray. However, in lesions of the hip, as it is a rare location and usually involves pain in the posterolateral aspect of the thigh, which can simulate radicular pain from a lumbar intervertebral disc, it could be difficult to diagnose. Hence, physicians usually focus on lumbar lesions; therefore, misdiagnosis is common and leads to a delayed management. Here, we report the case of a 30-year-old female patient with calcific tendinitis of the rectus femoris that was successfully managed with ultrasound-guided steroid injection. This study offers knowledge about the rectus femoris calcification.

3.Evaluation of Vitamin C Supplementation and Effectiveness of Smoking Cessation Intervention in Adolescent Male Smokers.

Smoking can increase oxidative stress and thereby change the antioxidant defense system in the body. Supplementation of antioxidant vitamins might improve antioxidant status in the body. The purpose of this study was to evaluate the effectiveness of vitamin C supplementation and smoking cessation education on changes of antioxidant status and psychosocial factors related to smoking. To obtain above purposes, we investigated the effectiveness of intervention for male adolescent smokers were evalnate by assessing changes in dietary intakes, plasma antioxidant vitamin concentration, and psychosocial factors related to smoking after program completion. Subjects, male adolescent smokers, were assigned into four groups : Control group(19 students), Educ. group(19 students), Vit. C supple. group(19 students), and Educ. + Vit. C suppl. group(19 students). The Educ. group and Educ.+Vit. C suppl. group received nutrition and smoking cessation education once a week for 5 weeks. Vit. C suppl. group and Educ.+Vit. C suppl. group received 500 mg per day of ascorbic acid for 35 days. All data were collected before and after intervention. Vit. B2 and Vit. C intakes of all groups were increased, but the only Ca intake was increased in the Educ. group. Plasma Vit. C concentration and Ratio(plasma Vit. C/Vit. C intakes) were increased in the Vit. C suppl. group and Educ.+Vit. C suppl. group, and the Vit. C deficiency status of these groups(Vit. C suppl. group and Educ.+Vit. C suppl. group) disappeared. Showing the effects of Vit. C supplementation, plasma alpha-tocopherol was increased in the Educ. and Educ.+Vit. C suppl. group, and especially high increases were seen in the Educ.+Vit. C suppl. group. Psychosocial factors related to smoking changed after the education a little. This intervention program had an impact on nutrition intakes, plasma antioxidant vitamins, and some beliefs related to smoking in male adolescent students. Various programs of nutrition and smoking cessation education and vitamin supplementation for quitting smoking must be implemented for adolescent smokers, and further studies are needed regarding sorts and amount of antioxidant nutrients and supplementation periods.

5.Analysis of Work-Related Carpal Tunnel Syndrome Compensated with the Industrial Accident Compensation Insurance from 2000 to 2002.

OBJECTIVE: This study was to analyze the characteristics of work-related carpal tunnel syndrome, as listed by the Korea Labor Welfare Corporation (KLWC). METHOD: Using the database of the KLWC, 163 approved work-related carpal tunnel syndrome cases were collected, which occurred between 1st, 2000 and 31th, 2002. Investigations were conducted by mail survey and interviews. RESULTS: Women accounted for 57.1%. The mean age and disease-related working duration were 44.7 years and 6.7 years, respectively. The most common industrial type of enterprise was manufacturing (104 workers) and the most common job was craft and related trades (51 workers), respectively. The most common cause of disease was repetitive work (104 workers). The duration between job starting and occurrence of disease was shorter in excessive force (5.5 years), women (5.7 years), non-manufacturing industries (4.4 years), and elementary occupations (5.0 years) than those having bad posture (14.8 years), men (8.2 years), manufacturing (8.1 years) and craft and related trades (8.7 years), respectively. CONCLUSION: Through this study, we elucidated certain characteristics of workers such as age, sex, working duration and high risk groups (women, workers at small scale industries and at non-manufacturing industries and excessive- forced work) with approved occupational carpal tunnel syndrome.

6.Sleep Disorders Documented by Night Polysomnography in Patients with Chronic Insomnia Disorder

BACKGROUND: To investigate the sleep disorders related to chronic insomnia and compare the characteristics of each group. METHODS: We registered 191 patients (female 56.0%, age 61.7 years) who have complained about symptoms of insomnia for more than three months and assessed sleep disorders related chronic insomnia by analyzing their polysomnography (PSG) parameters, demographics, and questionnaires (Insomnia Severity Index, Beck Depression Inventory, Pittsburg Sleep Quality Index, and Epworth Sleepiness Scale, and World Health Organization Quality of Life Assessment Instrument abbreviated version). RESULTS: Patients were categorized into groups of primary insomnia (PI, 51.8%, n=99), comorbid insomnia with obstructive sleep apnea (CIO, 38.7%, n=74), and comorbid insomnia with periodic limb movement disorder (CIP, 9.4%, n=18). CIO was older and more obese than PI and CIP. The proportion of males was the highest in CIO. Arousal index of PSG was higher in CIO and CIP than in PI. Other parameters and the results of questionnaires were not different among groups. CONCLUSIONS: About a half of patients with chronic insomnia symptoms (48.2%) had considerable sleep disorders associated with insomnia. Our study suggests that the PSG as well as history taking and demographics are necessary to clarify the relevant conditions of chronic insomnia disorder for appropriate treatment.

7.A Case of SIADH Related to Drug-indeced Generalized Maculopapular Rash.

Syndrome of inappropriate antidiutetic hormone(SIADH) secretion is the most common cause of hyponatremia in clinical medicine. Before diagnosis of the SIADH is made, other causes for a decreased diluting capacity and nonosmotic stimuli for AVP release need to be rule out. Disorders associated with SIADH can be divided into 4 major etiologic groups: malignancies, pulmonary diseases, central nervous disorders, and drugs. A 45-year-old woman was admitted due to maculopapular skin eruption and fever after taking medications for fever and myalgia. Generalized tonic clonic seizure was developed nine days later, and laboratory results showed marked hyponatremia. During the evaluation, treatment, and subsequent follow-up, the diagnosis of SIADH was confirmed, but the definitive cause was obscure. With fluid restriction, sodium replacement and demeclocycline therapy, she recovered completely 6 months later. We suggest that the SIADH might be related to drug-induced generalized maculopapular rash via menmgitis-like reaction in CSF as one of systemic adverse side effects to drugs rather than direct effect of related drugs.

8.Significance of Transcatheter Arterial Embolization in the Treatment of Pseudoaneurysm ComplicatingPancreatitis.

PURPOSE: To evaluate the significance of transcatheter arterial embolization(TAE) of pseudoaneurysmcomplicating pancreatitis. MATERIALS AND METHODS: This study was based on a retrospective analysis of eightcases, in which TAE for the control of pseudoaneurysm complicating pancreatitis was attempted. All patients weremales, and were aged between 35 and 65(mean, 47) years. Seven had a history of episodes of chronic pancreatitisand one case was the result of acute pancreatitis. All patients underwent diagnostic angiography andsuperselective embolization. RESULTS: Arteries in which pseudoaneurysm had occurred were the gastroduodenal(n=5), inferior pancreaticoduodenal (n=1), superior mesenteric artery root (n=1), and the celiac axis (n=1). Sixcases were treated successfully without complications, but in two, embolization failed due to a wide aneurysmalneck arising from the superior mesenteric artery root and celiac axis. In four successful cases, pseudoaneurysmswere completely resolved within three to six months of embolization. One of the other two remained as apseudocyst, while in the other, also a pseudocyst, surgery was performed. CONCLUSION: Because TAE in patientswith pseudoaneurysm complicating pancreatitis has a high success rate, and also leads to absolute resorption of apseudocyst, TAE is the preferred pre-surgical treatment mode.

9.Characterization of Adenoviruses Isolated from Children with Respiratory Illness in Busan, 1999~2000.

BACKGROUND: Adenoviruses (Ads) are a worldwide cause of endemic and epidemic respiratory infections, particularly in children, young adults and immunocompromised patients. They are responsible for 5% of the acute respiratory infections in children under the age of 4 years and account for about 10% of all cases of childhood requiring hospitalization in this age group. In this study, we report the characteristics of adenoviruses isolated from children with respiratory illness in Busan, 1999~2000. METHODS: A total of 765 children with acute respiratory illness from ten local clinics were studied. Isolation of Ads was performed by inoculating throat swab from patients into Hep-2 cells. The virus propagation was confirmed by the presence of cytopathic effect and adenoviruis specific PCR. Typing of isolated viruses was determined by sequencing analysis of hexon gene. RESULTS: Ads were isolated from throat swab of 17 (2.2%) out of 765 children. The epidemic of Ads infection was concentrated on winter months (December, January, and February). Seventeen Ad isolates showed four serotypes, with Ad3 being the most frequent strain. The distribution of Ads serotypes were eleven (64.7%) Ad3 strains, three (17.6%) Ad5 strains, two (11.8%) Ad2 srtains, and one (5.9%) Ad1 strain. CONCLUSION: Acute respiratory infections by adenoviruses in children were occurred particularly in winter season in Busan. More extended and systematic surveillance of adenovirus infection among populations is required to elucidate the extent of epidemic and disease burden of adenovirus infection.

10.Clinical Analysis of 1,068 Cases of Mid-trimester Genetic Amniocentesis.

OBJECTIVES: The objective of this study is to analyze 1,068 cases of prenatal genetic amniocentesis and to compare the results with reported studies. METHOD: We analyzed 1,068 cases of midtrimester prenatal genetic amniocenteses from September 1994 to February 1999, and investigated the fetal chromosomal abnormality, obstetric outcomes and complications by the indications of genetic amniocentesis and prophylactic antibiotic use at the Department of Obstetrics and Gynecology, Ajou University School of Medicine. RESULTS: Abnormal maternal serum markers were the most common indication of amniocentesis (57.7%) and the most common age distribution was 25-29 years (39.2%). One case of early amniocentesis (14 gestational weeks) was performed. The overall incidence of chromosomal aberration was 5.2% (56/1,068), of which there were 28 cases (50.0%; 28/56) of numerical aberrations and 28 cases (50.0%; 28/56) of structural aberrations. There were 50 cases (4.7%) of autosomal chromosomal aberrations and 6 cases (0.6%) of sex chromosomal aberrations. The pregnancy outcome was full-term delivery in 86.5%, preterm delivery in 7.6%, termination of pregnancy in 4.0%. There were no cases of serious complications including fetal death except for a case of self-limited amniotic fluid leakage(high leakage) in which the pregnancy was maintained. There were no significant differences between prophylactic antibiotics user group and non-user group in obstetric complications and outcomes. CONCLUSION: We could confirm that the trend in the indication of genetic amniocentesis had changed from advanced maternal age(35 year-old) toward abnormal maternal serum marker(triple test), and we recognized the importance of genetic amniocentesis according to the various antenatal screening tests of maternal serum marker, antenatal ultrasound, past history of fetal anomaly or family history of fetal chromosomal anomaly in the younger age groups(< 35 year-old) that are involved in more than half of the chromosomal anomaly. Further study will be needed to elucidate the efficacy of using prophylactic antibiotics in amniocentesis.
