Anti-GQ1 antibody is found in patients with Miller-Fisher syndrome (MFS), atypical MFS, and Bickerstaff's brainstem encephalitis (BBE). These conditions are various manifestations of post-infectious autoimmune disorders, and anti-GQ1b antibodies play a core pathogenic role. So they are referred as the 'anti-GQ1b antibody syndrome'. We report two cases of recurrent anti-GQ1b antibody syndrome.

Paraneoplastic encephalitis presenting with partial clonic seizure is rare. We report a 57-year-old man with clonic seizure in his left arm. He had past medical history of recurrent thymoma and thymomectomy. Magnetic resonance imaging showed multiple brain lesions, but none of them were congruent with the partial seizure. His symptoms and brain lesions resolved after steroid therapy. This is the first reported case extralimbic encephalitis presenting as partial seizure with invasive thymoma in Korea.

5.Follicular Variant of Papillary Thyroid Carcinoma: Clinicopathological Features According to Histologic Subgroup.

PURPOSE: The follicular variant of papillary thyroid carcinoma (FVPTC) is difficult to diagnose due to pathologic features. There is also debate on the optimal extent of surgery. We separated FVPTC into two groups and compared the clinical features in an attempt to apply the treatment. METHODS: All 40 patients with FVPTC who were diagnosed between 1990 and 2009 were reviewed and separated into two groups, an encapsulated group and an infiltrative group, based on whether a capsule was formed or infiltration occurred. These two different subtypes of FVPTC were compared on the traits of sensitivity of diagnosis and clinicopathologic features. RESULTS: After review by a pathologist, 21 of 40 patients (55%) were found to have encapsulated tumors, and 18 patients (45%) had infiltrative tumors. There was no difference in age, sex, or size. Patients with encapsulated FVPTC had a significantly lower rate of lymph node metastasis (4.5%), multicentric tumors (18.2%), and thyroid capsular invasion (9.1%) compared with the infiltrative tumor group (50%, 50% and 50%, P<0.05). There was no difference in FNA sensitivity between the two groups, but the sensitivity to frozen biopsy was higher in the infiltrative group. There was no recurrence in the encapsulated group, but 4 patients (22.2%) experienced recurrence in the infiltrative group. CONCLUSION: FVPTC can be separated into two subgroups by histologic features, and there are some clinicopathologic differences between the two groups. Patients who had encapsulated FVPTC had a lower rate of lymph node metastasis, multicentric tumors, and thyroid capsular invasion. They also showed a lower rate of recurrence than the infiltrative group. It is suggested that the encapsulated group can be treated with limited surgery and the infiltrative group needs aggressive treatment.

7.The Diagnostic Value of Digital Subtraction Angiography Considering the Pathomechanism of Symptomatic Cerebral Developmental Venous Anomaly.

Cerebral developmental venous anomaly (DVA) is generally benign. However, we have experienced two cases of DVA causing symptoms. In the first case, the patient demonstrated DVA with venous infarction. DVA was visualized in the arterial phase using digital subtraction angiography (DSA), and was diagnosed as arterialized DVA. The second case presented as transient right homonymous hemianopia. DSA revealed venous congestion; the transient aggravation of venous congestion may have caused the symptom. DSA is useful for diagnosing the pathomechanism of symptomatic DVAs.

8.Spontaneous regression of the mild cervical dysplasia and its related factors.

OBJECTIVE: The aim of this study was to assess the regression rate of the mild cervical dysplasia and to examine the factors associated with the regression. METHODS: One hundred and nine women pathologically confirmed with mild cervical dysplasia were recruited into this study. They were followed up by cytology, colposcopy and human papillomavirus DNA test at 3 months and 6 months after the diagnosis. The participants filled out a questionnaire on their demographic characteristics and sexual history. RESULTS: The rate of spontaneous regression for the mild cervical dysplasia was 59.6%(65/109). Multivariate analysis showed that initial ASCUS cytology, negative HPV status, non-smoker, condom user and age under 40 years old were associated with higher regression rate. CONCLUSIONS: The rate of regression for mild cervical dysplasia seen in our study was particularly higher than the result in previous study. Therefore, we recommend that the patients with mild cervical dysplasia can be followed up by cytology and HPV DNA study without any intervention, especially in young patients. Initial ASCUS cytology, negative HPV status, non-smoker, condom user and age under 40 years old were good related factors for regression.

9.Kinesin Superfamily KIF1A Protein Binds to Synaptotagmin XI.

The kinesin proteins (KIFs) make up a large superfamily of molecular motors that transport cargo such as vesicles, protein complexes, and organelles. KIF1A is a monomeric motor that conveys synaptic vesicle precursors and plays an important role in neuronal function. Here, we used the yeast two-hybrid system to identify the neuronal protein (s) that interacts with the tail region of KIF1A and found a specific interaction with synaptotagmin XI. The amino acid residues between 830 and 1300 of KIF1A are required for the interaction with synaptotagmin XI. KIF1A also bound to the tail region of synaptotagmin IV but not to other synaptotagmin in the yeast two-hybrid assay. KIF1A interacted with GST-synaptotagim XI fusion proteins, but not with GST alone. An antibody to synaptotagmin XI specifically co-mmunoprecipitated KIF1A associated with synaptotagimin from mouse brain extracts. These results suggest that KIF1A motor protein transports of synaptotagmin XI-containing synaptic vesicle precursors along microtubule.

10.Magnolol Inhibits LPS-induced NF-kappaB/Rel Activation by Blocking p38 Kinase in Murine Macrophages.

This study demonstrates the ability of magnolol, a hydroxylated biphenyl compound isolated from Magnolia officinalis, to inhibit LPS-induced expression of iNOS gene and activation of NF-kappaB/Rel in RAW 264.7 cells. Immunohisto-chemical staining of iNOS and Western blot analysis showed magnolol to inhibit iNOS gene expression. Reporter gene assay and electrophoretic mobility shift assay showed that magnolol inhibited NF-kappaB/Rel transcriptional activation and DNA binding, respectively. Since p38 is important in the regulation of iNOS gene expression, we investigated the possibility that magnolol to target p38 for its anti-inflammatory effects. A molecular modeling study proposed a binding position for magnolol that targets the ATP binding site of p38 kinase (3GC7). Direct interaction of magnolol and p38 was further confirmed by pull down assay using magnolol conjugated to Sepharose 4B beads. The specific p38 inhibitor SB203580 abrogated the LPS-induced NF-kappaB/Rel activation, whereas the selective MEK-1 inhibitor PD98059 did not affect the NF-kappaB/Rel. Collectively, the results of the series of experiments indicate that magnolol inhibits iNOS gene expression by blocking NF-kappaB/Rel and p38 kinase signaling.
