3.Succesful Treatment of Multiseptated Liver Abscess by Percutaneous Drainage with Urokinase.

Liver abscess is a rare disease but shows a high mortality rate. The standard treatment is complete drainage of the abscess through surgery or insertion of a percutaneous catheter and proper antibiotic treatment. However, successful percutaneous drainage in a multiseptated abscess has several drawbacks, such as the necessity for inserting more than one catheter for proper drainage, a necessity for repetitive cathterization and moreover, the ultimate failure of effective drainage causing surgery to become inevitable. The authors used 80,000 IU urokinase three times per day over 5 days to lyse the fibrous septum in a 9-year-old child with multiseptated liver abscess. Catheter drainage with urokinase under radiologic intervention yielded successful a outcome in the treatment of a multiseptated liver abscess.

4.Malignant Hypertension with Pulmonary Alveolar Hemorrhage Needing Dialysis.

A 35-year-old man presented with progressive dyspnea and hemoptysis. His blood pressure was 230/140 mmHg and serum creatinine level was 20.13 mg/dL. Chest radiography and computed tomography revealed pulmonary hemorrhage. His renal function was low, thus emergent renal replacement therapy was required. Malignant hypertension and acute kidney injury were diagnosed, and antihypertensive therapy and hemodialysis started immediately. Renal biopsy was performed to examine the underlying disease. Typical pathological changes of malignant hypertension, fibrinoid necrosis of the afferent arterioles, and proliferative endoarteritis at the interlobular arteries were observed. His renal function improved gradually and pulmonary hemorrhage completely disappeared with administration of antihypertensive agents. Here, we report this rare case of malignant hypertension with pulmonary alveolar hemorrhage and speculate that the hemorrhage may be related to vascular injuries at the alveolar capillary level caused by malignant hypertension.

5.A Case of Severe Hypercalcemia after Treatment with Teriparatide for Osteoporosis.

Recombinant human parathyroid hormone 1-34 (rHPTH; 1-34, teriparatide) increases bone mass and increases osteoporotic fracture by stimulating new bone formation. It was approved in the United States for treatment of osteoporosis in men and women, and its effectiveness and safety was proved. Mild hypercalcemia was observed, but persistent and severe hypercalcemia was not observed in the studies of teriparatide. In this case, severe hypercalcemia was developed from patient having gait disturbance who was treated with vitamin D, calcium and teripartide for two months to treat osteoporosis after subtrochanteric fracture. Hypercalcemia was resolved with discontinuation of teriparatide. Severe hypercalcemia is not a common complication of teriparatide and monitoring of serum calcium level is routinely not recommended. But it is necessary to pay close attention to patients who were treated with teriparatide, especially in immobilized patients.

7.Comparison of the ease of laryngeal mask airway ProSeal insertion and the fiberoptic scoring according to the head position and the presence of a difficult airway.

BACKGROUND: The sniffing position is recommended for conventional laryngeal mask airway (LMA) insertion. However, there has been a high success rate of LMA insertion with the head in the neutral position. The effect of a difficult airway on the ease of LMA insertion is not clear. In this study, we compared the ease of LMA ProSeal(TM) (PLMA) insertion and the fiberoptic scoring according to the head position and the presence of a difficult airway. METHODS: After obtaining informed consent from the subjects, we enrolled 144 adult patients (age range: 18-65) with an ASA physical status 1 or 2. After evaluation of the airway, all the patients were grouped into the EA (easy airway) group (n = 68) and the DA (difficult airway) group (n = 76). According to the head position, each group was divided into the EA-SE (extension) group (n = 35), the EA-SN (sniffing) group (n = 33), the DA-SE group (n = 39) and the DA-SN group (n = 37). The success rate and insertion time at the first attempt were evaluated. The position of the PLMA was fiberoptically scored from the mask aperture of the airway tube in the original head position. After the head position was changed to the sniffing and neutral positions in the SE and SN group, respectively, the position of PLMA was re-evaluated fiberoptically. RESULTS: The success rate and insertion time at the first attempt and the fiberoptic score showed no significant difference among the groups. After head position was changed, there were no significant changes in the fiberopitc scores. CONCLUSIONS: A difficult airway and the head position had no influence on the ease of PLMA insertion and the fiberopic score. Therefore, the head position can be selected according to the individual patient's situation.

8.Tick Bite by Male Amblyomma testudinarium in a Korean Woman.

A 53-year-old woman presented with asymptomatic pedunculated mass on her buttock. She was attacked by the tick while climbing the Jungbyung mountain located in Changwon City, the southern coastal area of the Korean Peninsula. It was identified as male Amblyomma testudinarium according to its morphological characteristics. Histological examination showed dense inflammatory infiltration of neutrophils and eosinophils, and dilated vessels on the dermis beneath the biting site of the tick. There have been only few reports of the tick bite by A. testudinarium in the Korean literature. To the best of our knowledge, our patient is the second case of tick bite from this species and the first tick bite from the male tick recorded in Korea. Here, we report a case of tick bite by male A. testudinarium in a woman, including a review of relevant literature.

9.Effect of Curcumin in a Mouse Model of Oxygen-Induced Retinopathy.

PURPOSE: To investigate the effect of curcumin, known to inhibit hypoxia-inducible factor-1, on retinal neovascularization in a mouse model of oxygen-induced retinopathy (OIR). METHODS: OIR was induced by exposing C57BL/6 mice on postnatal day 7 (P7) to 75% hyperoxia for 5 days, followed by 5 days in a room with normal oxygen level. Curcumin was administered intraperitoneally once a day for 5 days from P12 or intravitreally once on P13. Mice retinas on P17 were analyzed for retinal neovascularization, which was compared between curcumin-treated and control mice. RESULTS: After intraperitoneal and intravitreal administration of curcumin, qualitative assessment of retinal neovascularization of flat-mounted retina showed no significant difference compared to control retinas. Quantitative assessment of retinal neovascularization also showed no significant difference between curcumin-treated and control mice. CONCLUSIONS: Both intraperitoneal and intravitreal administration of curcumin did not reduce retinal neovascularization in an OIR mouse model. Further investigation including development of new formulations is required for the use of curcumin as an anti-angiogenic agent for retinal neovascularization.

10.Analysis on mandibular movement using the JT-3D system

Purpose:
This study aims to measure the mandibular movement using JT-3D system and provide a range of mandibular movement that can serve as a good reference for diagnosing the temporomandibular disorder.
Materials and Methods:
This study was conducted in 60 young male and female adults. The maximum opening and closing movement was recorded using JT-3D system, and by regarding 5 times of repetitive movement as 1 cycle of movement, total 3 cycles of movement were recorded. During the maximum opening, vertical position of lower jaw, antero-posterior position, lateral deflection position, and maximum opening distance were recorded. To evaluate the reproducibility of JT-3D system, statistical analysis were conducted (α = 0.05).
Results:
During the maximum opening, the average value appeared at 31.56 mm vertically and 24.42 mm rearwardly, lateral deflection position 0.72 mm, and maximum opening distance 40.32 mm. There was no statistical significance in all measured values for three cycles of movement recorded with JT-3D system (P > 0.05).
Conclusion
During the maximum opening, the average value appeared at 0.72 mm in lateral deflection position and the maximum opening distance at 40.32 mm, and the analysis on the maximum opening of lower jaw using JT-3D system showed sufficiently reproducible results.
