2.Diagnostic Significance of TNF-alpha in Tuberculous and Non-Tuberculous Pleural Effusion.

OBJECTIVES: The differentiation of tuberculous effusion from the other causes of exudative pleural effusion remained difficult even with aids of biochemical analyses and pleural biopsy. As the pathophysiology of tuberculous pleural effusion is an enhanced cell mediated immunity, Adenosine deaminase(ADA) and various cytokines including Inteferon-γ, tumor necrosis factor alpha(TNF-α) are considered as useful diagnostic tools in differentiating exudative pleural effusion The author would like to demonstrate the diagnostic usefulness of TNF-α in the differentiation of exudative pleural effusion, and compared the discriminating ability of TNF-α with ADA. METHODS: Pleural fluids obtained from 80 patients (tuberculous : 39, malignant : 31, parapneumonic : 10) with exudate pleural effusions were processed for cell counts and biochemical analysis including ADA and TNF-α RESULTS: Tuberculous pleural fluid showed higher levee of ADA and TNF-α, 48.7α 32.7U/L and 184.1±214.2pg/mL than that of non-tuberculous effusion 26.0α41.3U/L and 44.1α114.2pg/mL, respectively (ADA, TNF-α : p<0.05, p<0.01). Receiver operating characteristics(ROC) curves were generated for ADA and TNF-α, and the best cut-off value for adenosine deaminase and TNF-α were considered as 30U/L and 15pg/ml, respectively. Comparing the area under the ROC curves, there was no significant difference between ADA and TNF-α CONCLUSION: For the differential diagnosis of tuberculous pleural effusion from the other causes of exudative pleural effusions, TNF-α as well as ADA was considered as useful diagnostic method. However adding TNF-α to ADA has no further diagnotic benefit than ADA alone.

3.A Case of Cellulitis Associated with Coral Injury.

Corals are the marine organism that belongs to the phylum Cnidaria, and are one of the common causes of superficial injury in the marine environment. In addition to acute reactions such as burning or stinging pain and erythema, coral injuries may present with complications such as foreign body reactions, bacterial infections, and/or localized eczematous reactions. A 23-year-old male presented with an erythematous edematous tender patch with centrally grouped vesicles on the left ankle; the injury had occurred during skin-scuba diving 2 days before. A biopsy of the lesion treated with hematoxylin-eosin stain showed epidermal necrosis with subepidermal blisters and neutrophilic panniculitis. Herein we report a case of cellulitis caused by the nematocyst stings of corals.

4.A Case of Metastatic Neuroendocrine Carcinoma of the Lung Concomitant with Adenocarcinoma of the Stomach.

Neuroendocrine tumors derive from the diffuse neuroendocrine system producing hormonal peptides and/or biogenic amines. Histologically neuroendocrine differentiation includes ultrastructural demonstration of neurosecretory granules and staining with immunhistochemical markers such as neuron-specific enolase, chromogranin, synaptophysin, and cytokeratin. Neuroendocrine carcinoma of the skin is rare but can be shown as an initial manifestation of metastatic neuroendocrine carcinoma as well as primary cutaneous neuroendocrine carcinoma. Histological differentiation between primary cutaneous Merkel cell carcinoma and metastatic neuroendocrine carcinoma of the skin is difficult and clinical correlation is important. We report a case of metastatic neuroendocrine carcinoma of the lung occurring on the scalp as a single nodule in a 65-year-old female who previously diagnosed with adenocarcinoma of the stomach.

Elephantiasis nostras verrucosa is an uncommon disorder that is characterized by dermal fibrosis, hyperkeratotic verrucous and papillomatous lesions and this is all caused by chronic non-filarial lymphedema secondary to infections, surgeries, tumor obstructions, radiations, congestive heart failure and obesity. We report here on a case of elephantiasis nostras verrucosa that occurred on the left lower leg of a 54-year-old woman who had a past history of extensive skin grafts and recurrent infection.

6.Analysis of the Nursing Curricula in Graduate Programs in Korea.

This study was designed to analyze the nursing curricula in graduate programs and suggest the direction of future nursing curricula. Thirty-two nursing curricula for a master's degree and fifteen curricula for a doctoral degree from general graduate programs, fifteen nursing curricula for a master's degree from fifteen educational graduate programs, eight nursing curricula for master degree from eight out of twelve public health graduate programs, three nursing curricula for a master's degree from three administrative graduate programs, and four nursing curricula for a master's degree from four clinical nursing graduate programs, were analyzed. Consequently, It was hard to find a clear difference between the educational goals and objectives, the subjects open in nursing curricula for a master's degree and those for a doctoral degree of graduate programs. The educational graduate programs, public health graduate programs, administrative graduate programs, and clinical nursing graduate programs, each program showed ittle diffrent in each educational goal and objective. However, because the various kinds of subjects open in each program were not based on the core curricula, the quality of the nursing curricula need to be evaluated and develope the curricula to difference are the goal and objective. Accordingly, future studies need to be focused on developing core nursing curricula reflecting the characteristics of each graduate program.

7.A Case of Cutaneous Zosteriform Metastatic Carcinoma Arising on the Healed Site of Herpes Zoster.

We report a case of a 67-year-old man presenting with a 2-week history of erythematous zosteriform indurated confluent nodules and plaques on the right chest which was affected by a herpes zoster infection one month ago. The previous history revealed an adenocarcinoma with malignant pleural effusion on the right lung (stage IV, T4N1M1) 3 months ago. Histopathologic findings showed tumor cells infiltrated throughout the dermis, forming focal glandular structures. We think that the case was the isotopic response in the form of an occurrence of a new skin disorder at the site of another unrelated and already healed skin disease. Many cases of isotopic response have been reported in Korean literature such as lichen planus, granuloma annulare, benign fibrous histiocytoma and reactive perforating collagenosis occurring on the healed site of herpes zoster infection. This is the first report of cutaneous metastatic carcinoma as an isotopic response in Korea.

Background:
Most unintentional injuries that occur at home are preventable. However, it may be difficult to sufficiently reduce the number of falls occurring at home by only identifying risk factors focused on specific age groups. Therefore, this study aimed to identify the risk factors (especially age and places where injuries occurred at home) for intracranial injury (ICI) caused by unintentional falls at home.
Methods:
Using the Emergency Department (ED)-Based Injury In-depth Surveillance, we analyzed the data of patients who visited the ED due to unintentional falls at home. Risk factors were identified using multivariable logistic regression according to age groups and interactions between place of injury occurrence and age groups, and sex and age groups were assessed.
Results:
In total, 232,124 patients were included in the analysis; older adults had a higher adjusted odds ratio (aOR) 14.05 (95% confidence interval [CI], 12.74–15.49) of ICI than infants. The corridor was associated with ICI in the male pediatric group (aOR, 2.71; 95% CI, 1.08–6.84) and the balcony with the female pediatric group (aOR, 2.04; 95% CI, 1.03–4.04). In the adult group, aOR of kitchen was 1.38 (95% CI, 1.02–1.88) in females and 0.56 (95% CI, 0.48–0.66) in males.
Conclusion
In this study, we identified the risk factors of ICI caused by falls at home using ED-based injury surveillance data. The risk of ICI was different among places of occurrence in the home depending on the age groups and sex.

A chest computed tomography (CT) scan and fiberoptic bronchoscopy demonstrated a polypoid tumor in the left main bronchus, 0.5cm distal from the carina. Surgical resection of the tumor was performed, along with. A pathological evaluation and the immunohistochemical findings led to the diagnosis of a glomus tumor, which originated from the bronchus, an area where this type of tumor has rarely been reported.
