Multiple factors including gastroesophageal reflux disease (GERD) were evaluated for a case-control study in Chonnam area to investigate the causative entity of COPD. Data on the multiple causative factors from hospital records and interview survey were analyzed in three groups of COPD(64 cases as case group), normal lung (83 cases as control group 1) and non-COPD lung disease (45 cases as control group 2). Smoking status, history of adulthood pulmonary infection and frequent history of URI, socioeconomic status, and GERD were significantly different between COPD group and control group 1. Drinking status, physical height of the subjects and GERD were significantly different between COPD group and control group 2. If control group 1 was used, odds ratio of GERD and COPD was 5.68(95%confidence interval, 95% CI:2.59-12.45) and 4.81 (95% CI:1.89-10.53) when adjusted by age and smoking status. If control group 2 was used. Odds ratio of GERD and COPD was 4.22 (95% CI:1.69-10.56) and 4.59 (95% CI:1.64-12.86) when adjusted by alcohol and adulthood respiratory infection status. In summary, there results suggested that GERD might play a causative role in the development of COPD.

BACKGROUND: Isolated infraspinatus tear is very rare and clinical features are not as well known, therefore the purpose of this study was to evaluate clinical characteristics and outcomes of isolated infraspinatus tear that authors experienced. METHODS: Authors reviewed 288 cases of full-thickness rotator cuff tear involving infraspinatus between 2010 and 2015, and retrospectively analyzed six cases of isolated infraspinatus tear. Perioperative clinical characteristics, postoperative functional outcomes of 6 months were investigated. Functional evaluation included visual analogue scale (VAS), range of motions, American Shoulder and Elbow Surgeons (ASES) score, and Constant score. RESULTS: Calcific tendinitis was accompanied in 4 cases (66.7%). Three of them received steroid injection or aspiration or extracorporeal shockwave therapy. Mean preoperative pain VAS was 7.1 (range, 5–9), and mean postoperative pain VAS at 6 months later was 1.6 (range, 0–5). Preoperative muscle strength by isokinetic muscle performance test showed 52% deficit of abduction and 37.6% deficit of external rotation. All 6 patients had arthroscopic repair of the infraspinatus tendon. All the patients at the 6 months follow-up exhibited clinical improvement in the Constant score (67.8 [range, 45–77] to 89.3 [range, 81–100], p=0.029), and ASES score (52.3 [range, 30–77] to 90.0 [range, 80–100], p=0.002). There was no healing failure on imaging. CONCLUSIONS: Isolated infraspinatus tendon tear was frequently accompanied by calcific tendinitis, but pathophysiologic relationship should need more study. To rule out neurogenic etiology, magnetic resonance imaging and electromyography would be helpful. Arthroscopic infraspinatus tendon repair and supraspinatus debridement showed relatively good result in painful shoulder.

Thombolytic therapy and placement of vascular metallic stent can be used for the treatment of lilac venous stenosis and thrombosis, but these treatments increase the risk of pulmonary thromboembolism. Inferior vena cava filter was developed for the prevention of recurrent pulmonary thromboembolism due to lower extremity deep vein thrmobosis and has been regarded as relatively safe and effective treatment modality. We experienced good result of combined treatment of inferior vena filter, thrombolytic therapy and placement of right lilac venous metallic stent in a patient with severe stenosis and thrombosis at both common lilac veins.

PURPOSE: Choroid Plexus papilloma is a rare intracranial neoplasm that is most commonly found in the trigone of the lateral ventricle in children or in the fourth ventricle in adult. Extraventricular extension of choroid plexus papilloma has been rarely reported within the cerebellopontine angle (CPA) cistern. Authors report two cases of choroid plexus papilloma in the posterior fossa seen on magnetic resonance imaging (MRI). MATERIALS AND METHODS: MRI findings of two cases of choroid plexus papilloma in posterior fossa were retrospectively reviewed. RESULTS: In the first case, the tumor was in the fourth ventricle and extended to the left CPA cistern via the foramen of Luschka. In the second case, the tumor presented as an expansile mass of the of fourth ventricle and right lateral recess. Multiple signal voids of low intensity due to prominent feeding vessels and calcifications within the tumors were noted. CONCLUSION: This report demonstrates MR findings of choroid plexus papilloma in the posterior fossa with the brief review of the literatures.

Dysplasia epiphysealis multiplex is a rare syndrome, first discribed by Fairbank in 1935, but numerous publications have made it a well-recognized entity. The syndrome is caused by a congenital developmental error of unknown etiology, characterized by changes in the developing epiphyses, dwarfism and stubby digits in children of normal intelligence. Diagnosis is mainly by roentgenographic apperences of the epiphyses before fusion to the shaft. The centers of ossification of the epiphyses are late in appearing, slow in developing, deformed in shape and irregular in density. The spine is never affected, blood and biochemicaI studies show no abnormality. A case of dysplasia epiphysealis multiplex which revealed typical roentgenographic pictures is presented with a brief review of a literature.

One hundred and sixty five cases of the fracture of tibial shaft in adult have been treated and managed in the Department of Orthopedic Surgery, Kyunghook National University Hospital during the period from January, 1965 to December, 1970. Seventy one of 165 patients were analyzed clinically and we came to the following conclusions. 1. Most of the injuries were the results of traffic accident (67.6%). 2. The incidence of trauma was high in the young man, most frequent in the 3rd decade. 3. The closed fractures were three times more than open fractures and the middle third of tibial shaft was the most frequent site of fracture. 4. The average healing period was 16 weeks in mild case and 28 weeks in severe case. 5. Of 71 cases, we experienced 4 cases of non-union, 8 cases of residual joint stiffness at the knee and the ankle and 3 cases of leg shortening (more than 2 cm.) as complication.

PURPOSE: To find the rebleeding factors in bronchial arterial embolization for treatment of hemoptysis, a retrospective study was performed. MATERIALS AND METHODS:Medical records, anglographic findings and embolic materials of 35 patients who had undertaken arterial embolization for control of hemoptysis were reviewed. The period of follow-up for rebleeding was from 3 to 32 months after arterial embolization. We investigated the anglographic findings of extravasation, neovascularity, intervascular shunt, aneurysm and periarterial diffusion. Neovascularity was classified as mild(numerable neovascularity) and severe(innumerable). RESULTS: Rebleeding occured in 15(43%) among 35 cases. Only two of 11 cases with no past episode of hemoptysis showed recurrence, while 9 of 15 cases who had more than three episodes did. Severe neovascularity were seen in 11 of 15 recurred cases, but seven of 20 non-recurred cases showed severe neovascularity. More than three anglographic findings representing hemoptysis were seen on 11(73%) among recurred 15 cases and seven(35%) among non-recurred 20 cases. The lesion was supplied by more than two different arteries on 8(54%) of the recurred cases, but only three(15%) of the non-recurred cases. Six of seven cases persistent neovascularity after arterial embolization were recurred. CONCLUSION: The history of repeated hemoptysis, severe neovascularity, variable anglographic findings, and post-embolization persistency of neovascularity were the factors related with the rebleeding after arterial embolization for hemoptysis. Careful and active arterial embolization are required on these conditions.
