2.Expression of Epidermal Growth Factor Related Peptides, EGF-R, and c-erbB-2 and Their Relationship with the Prognostic Factors in Gastric Carcinoma.

Recent investigations have revealed that autocrine growth factors and their receptors are closely related and play an important role in controlling cancer cell growth. We performed an immunohistochemical study on the expression of epidermal growth factor (EGF), transforming growth factor-alpha (TGF-alpha), epidermal growth factor receptor (EGF-R), c-erbB-2, and PCNA labelling index in 60 cases of human gastric carcinomas. TGF-alpha was detected in 38 cases (63.3%), EGF in 26 cases (43.3%), EGF-R in 44 cases (73.3%), and c-erbB-2 in 18 cases (30%). These growth factors, EGF-R and c-erbB-2, were found more often in advanced gastric cancers. The PCNA labeling index was significantly higher in tumors with the expression of EGF-R or c-erbB-2. Tumors with simultaneous expression of EGF, TGF-alpha, EGF-R and c-erbB-2 was associated with a high PCNA labeling index. A correlation was observed between the synchronous expression of growth factors and its receptors and histological differentiation. The results suggest that the expression of EGF, TGF-alpha, EGF-R and c-erbB-2 are closely related and plays an important role in the growth and progression of human gastric carcinoma.

4.Adenoid Basal Cell Tumor of the Prostate: A case report.

Adenoid basal cell tumor of the prostate is a rare tumorous lesion that can be misdiagnosed as adenocarcinoma of the prostate. The malignant potential of adenoid basal cell tumor remains uncertain due to small number of reported cases. This 66-year-old man presented with symptoms of urinary tract obstruction. Under the impression of benign prostatic hyperplasia, a transurethral resection of the prostate (TURP) was performed. The patient was alive with no evidence of recurrence or metastasis 15 months after TURP. Microscopically, most of the lesions were composed of nodular collections of small nests of basaloid cells with peripheral palisading, and clusters of tumor cells forming cribriform pattern. Multiple areas of basal cell hyperplasia and atypical basal cell hyperpalsia were also observed. The coexistence of basal cell hyperplasia, atypical basal cell hyperpalsia, and adenoid basal cell tumor with cribriform pattern in this case supports a morphologic continuum from the benign hyperplastic lesion to malignant neoplasia.

5.Herpes Zoster in the Patients with Malignant Tumor.

A study was made of the differences in clinical features and immunological aspects between herpes zoster patients with malignancies and those without malignant tumors. The results obtained from the retrospective review of medical records are as follows: Herpes zoster occurred more frequently in younger patients(less than twenty) with malignanciee as compared with the control group of the same ages,' Male was dominant among zoster patients with malignancies as is the contrary to contral group; Tihere were no marked differences in primarilly involved dermatomes between the two groups; Generalized varicelliform eruptiona were more common in zoster pa,tients with malignaneies than in control group, It seemed that zoster patients with malignancies complained of milder degree of pain. Zoeter patienta associated with malignancies were more frequently DNCB-negative and had decreased OKT3+ pan T cells, OKT4+ helper/inducer T cells as compared with control group, which suggest impaired cell mediated immunity in the former.

6.Is 'Lower Body Parkinsonism' A Distinctive Subgroup of Parkinsonism?.

FitzGerald and Jankovic(1989)' reported 10 patients with marked gait difficulty and no or only minimal upper limb involvement and coined the term 'lower body parkinsonism' (LBP). They further suggested that LBP was a homogeneous subgroup of parkinsonism, distinct from idiopathic Parkinson's disease (IPD), attributable to vascular etiology. In order to verify the validity of this clinical entity, five cases with LBP were compared with 44 cases with typical idiopathic Parkinson's disease (IPD) with respect to their clinical and neuroradiological findings. Mean age at onset was significantly older in LBP group (63.2*6.3 years) than IPD group (53.9*5.9 years). Gait disturbance was the initial motor symptom in 80% of LBP cases and 16% of IPD cases. Duration of symptoms tended to be shorter in LBP group than IPD group (3.2*2.7, 5.2*3. Lyears, respectively), although statistically not significant. Response to, L-dopa was poor in all but one case with LBP, while most cases with IPD showed good response (43 of 44 cases). On brain MRI in LBP group, four of five cases had multiple small ischernic lesions in both periventricular white matter and basal ganglia. However, one case with LBP showed normal brain MR finding and improved with L-dopa treatment, whose diagnosis proved to idiopathic Parkinson's disease. We conclude that LBP is unusual but not rare, and may be a part of clinical spectrum of either vascular parkinsonism or idiopathic Parkinson's disease. Although LBP is a heterogeneous group, most cases may be caused by multiple lacunar infarctions.

7.Clinical Study of Tibial Fracture: Comparison of 3 Methods

There is much controversy concerning the method of treatment on the tibial fracture because of frequent complications, such as non-union, delayed union and infection. Authors reviewed and analyzed 110 tibial fractures which had been treated at the Department of Orthopedic Surgery, Sung Ae General Hospital from June 1983 to March 1987 by the groups, conservative treatment, open reduction and internal fixation with compression plate and closed flexible intramedullary nailing. The results were as follows :1. In regard to the fracture location, there was no difference in the healing time among the treatment methods. The I.M. nailing group healed 1.5 weeks earlier than other groups in each location. 2. In minor and moderate severity by Ellis, the I.M. nailing group revealed better result, and in major one, the compression plate group showed better result. More complications occured in major severity. 3. Union time among the trestment methods was not affected by the fibular fractures, but most of the complications occured in the concomitant fibular fracture case. 4. In open fracture, the healing time showed no difference among the groups, and was prolonged about 2 weeks than closed fracture. In closed case, the I.M. nailing group showed good result. 5. The cases, which were reduced earlier in the conservative group, and treated within 3 days in the I.M. nailing group revesled better result. 6. The radiological union time from the definite treatment was 15.80 weeks in the conservative treatment group, 15.18 weeks in the compression plate group and 13.92 weeks in the I.M. nailing group.

9.Immunohistochemical Study on the Proliferative Activity of Human Thyroid Tumors.

For the estimation of the proliferative activity, related to the biologic behaviour, malignant potential, and prognosis, of human thyroid tumors, PCNA(proliferating cell nuclear antigen) immunohistochemical staining was performed on paraffin-embedded sections of 9 normal thyroid tissues, 9 adenomatous goiters, 9 follicular adenomas, 4 Hurthle cell tumors, 12 papillary carcinomas, 4 follicular carcinomas, and 3 anaplastic carcinomas. The results were as follows: 1) The PCNA labeling indices in adenomatous goiter, follicular adenoma, and Hurthle cell tumor were 1.1, 1.5, and 2.4, respectively. They were significantly higher than the labeling index in normal thyroid. 2) The PCNA labeling indices in papillary carcinoma and follicular carcinoma were 3.5 and 4.4, respectively. They were significantly higher than the labeling indices in adenomatous goiter and follicular adenoma, but there was no significant difference between papillary and follicular carcinoma. 3) The PCNA labeling index in anaplastic carcinoma, 14.1, was significantly higher than those in benign and other malignant tumors. According to the results, the PCNA labeling index was well correlated with the malignant potential of a tumor. So the PCNA immunohistochemical staining is thought to be a useful method for the evaluation of the malignant potential and prognosis of a tumor.

10.Pleomorphic Xanthoastrocytoma in a 58-year-old Woman: A case report.

A case of left parietal pleomorphic xanthoastrocytoma that occurred in a 58-year-old woman is reported clinicopathologically. Histopathologic diagnosis of pleomorphic xanthoastrocytoma was made because of the unique pleomorphic histologic features, positive glial fibrillary acidic protein in immunohistochemical staining. The flow cytometric analysis reveals DNA aneuploidy and relatively high S-phase fraction. The pleomorphic xanthoastrocytoma is considered as a special subgroup of glioma on the basis of superficial cortical location, GFAP expression, marked cellular atypia, xanthomatous cells, and relatively favorable prognosis.
