1.Effect Of the Polyethylene Wear On Osteolysis in Cementless Primary Total Hip Arthroplasty: Minimum 5 Years' Follow - Up Study.

We measured poiyethylene wear radiographically in 85 cementless primary total hip arthroplasties by the modified technique of Livermore et al. All the patients were followed up for at least five years (mean, 6.31 years; range, 5 years-8 years 2 months). The mean extent of linear wear was 0.88mm (range, 0.0083-3.2839mm), and the mean rate of linear wear was 0.14mm/year(range, 0.00-0.47mm /year). The mean volumetric wear was 539.42mm (range, 5.11 2022.09mm), and the mean rate of volumetric wear was 87.69mm/year(range, 0.65-290.12mm/year). The polyethylene wear was correlated inversely with the patients age only, but was not influenced by weight or gender of the patients, Harris hip score, thickness of the polyethylene, the abduction angle of acetahular cup, or the duration of in situ implantation. Osteolysis developed in the femur of 34 hips(40%) and in the acetabulum of 11 hips(12.9%). Radiographic loosening was found in one case in the femur and none in the acetabul pm. The extent of polyethylene wear was significantly correlated with frequencies and sizes of the osteolytic lesion and was not directly associated with radiographic loosening of the prostheses. Calcar resorption was observed in 39 hips(45.9%) and was not associated with polyethylene wear. These results indicated that polyethylene wear of primary total hip arthroplasty should be considered one of major causes of osteolysis and has no direct relationship with prosthesis loosening. To reduce the extent of polyethylene wear, the material characteristics of polyethylene should be improved or new, durable, articulating materials should be researched.

2.Ultra-Structural Changes of the Chorioretina Following Intra-Vitreal Injection of Ornithine in Rabbit.

The ultrastructural changes of the chorioretina following the intravitreal injection of ornithine in rabbit eyes at 1,2,4,8 week time intervals were investigated employing electron microscopy in this animal experiment. 1. 1 week after injection, swelling of the mitochondria in the cytoplasm of the pigment epithelium was seen. 2. 2 weeks after injection, phagosomes and phagolysosomes were present in the cytoplasm of the pigment epithelium and nerve elements in the inner nuclear layer disappeared and were replaced by Muller cell. 3. 4 weeks after injection, stroma of the choroid was atrophied and abundant melanocytes were aggregated, and papillary formed pigment epithelium with detachment of the retina was seen. 4. 8 weeks after injection, stromal atrophy and attenuated choriocapillaries of the choroid were present, and proliferation of the microvilli of the pigment epithelium after detachment of the retina was seen. 5. In summary intravitreal injection of ornithine in rabbits initially caused marked changes in the pigment epithelium with subsequent degeneration of the choroid and sensory retina.

3.Morphological Changes in the Ciliary Epithelium by Instillation of Befunolol.

The introduction of beta-adrenergic blocking agents for the treatment of glaucoma was a major advance in the continued search for medications that effectively control the intraocular pressure with minimal side effect. Beta-adrenergic blocking agents block the aqueous pump of the ciliary epithelium resulting in decreased aqueous formation and drop in intraocular pressure. The fact that beta-adrenergic blocking agents had an affinity to intraocular tissues containing melanin granules has been reported. The purpose of this study is to know that mechanism of decrease of intraocular pressure and an affinity to melanin granules of the beta-adrenergic blocking agents. Two drops of 1% befunolol were instilled in the albino and pigmented rabbits, and the eyes were enucleated on 1, 2, 6 and 24 hours after instillation. These eyes were studied with electron microscopic examination. The results were as follows: 1. In the non-pigment epithelium of the ciliary body, mitochondria became swollen. 2. These changes appeared earlier and were more remarkable in albino rabbits than in pigmented ones. 3. In pigmented rabbits, these changes appeared slowly, but continued for longer duration and it seems to be due to affinity of beta-adrenergic blocking agents to the melanin pigment. 4. Swelling of mitochondria at the non-pigmented epithelium of the ciliary body after instillation of beta-adrenergic blocking agents seems to be the pharmacological mechanism of the drugs against aqueous formation in the ciliary body.

4.Bacterial Culture Study of the Hip Joint Fluid during Primary Total Hip Arthroplasty.

Infection in primary total hip arthroplasty may cause catastrophic results and is the major reason for implant failure. The purpose of this study was to evaluate the utility of the hip joint fluid culture as a method of predicting the possibility of a hip joint infection by calculating the sensitivity, specificity and accuracy. We performed 628 primary total hip arthroplasty and aerobic and anaerobic bacterial cultures for hip joint fluid between January 1989 and June 1996. The hip joint fluid culture was routinely performed to evaluate the utility of the femoral head for bone banking. Thirty-two cases out of the 628 hips showed positive intraoperative culture and 596 cases showed negative intraoperative culture. The isolated organisms from 32 positive cultures were 11 for Staphylococcus aureus, eight for Staphylococcus coagulase negative, seven for Enterococcus, three for E.coli and one each for Enterobacter, Acinetobacter and Pseudomonas. Anaerobic culture was negative in all cases. In the positive intraoperative culture cases, none had delayed infection during the follow-up period. But in the negative intraoperative culture cases, one case had acute infection and two cases had delayed infection. In the case with acute infection, Staphylococcus aureus was isolated and on two cases with delayed infection, Enterococcus and Staphylococcus coagulase negative were isolated, respectively. All 32 positive culture cases were fa~lse positive and 595 negative culture cases were true negative and one negative culture case was fa~lse negative. The sensitivity of the hip joint culture was 0%, the specificity was 94.9% and the accuracy was 0%. The specificity of hip joint fluid culture in primary total hip arthroplasty was high, hut the sensitivity score was zero. Therefore, the hip joint fluid culture should not be used for a routine check of infection status in primary total hip arthroplasty. We recommend the hip joint fluid culture in revision arthroplasty or hips in which infection is clinically suspected.

5.Study of the Normal Cerebellar Volume Estimated by Magnetic Resonance Imaging MRI in Korean.

Brain size is a useful parameter describing ontogenic character and function. Despite its potential contribution to functional study and diagnosis of disease, it has been hampered by poor assessment tools in vivo and postmortem changes. Recent development of medical imaging techniques such as MRI and CT enable us to understand brain structures in vivo. Many morphometirc studies of the brain has given us new insights in the field of functional neuroanatomy and neuropsychiatric diseases. In spite of these advances, volumetric data of normal Korean brain is not available yet. As an initial approach to Korean standard brain size, we measured the cerebellar volume between 20 and 80 years old Koreans. The MRI films free from lesions in brain was analyzed and measured by NIH image program. The Korean cerebellar volume were ranged between 86.2cm(3) and 141.3 cm(3) and average male cerebellum volume was significantly larger than that of female[male : 118.2+/-11.2cm(3)] > female : 108.0+/-9.6cm(3), P=0.1021E-07<0.05]. And we could not find volume change according to aging process. This is the first report on normal Korean cerebellar volume and the presence of gender difference in the Korean cerebellar volume. These data will be used for the standard useful in studying cerebellar function and cerebellum associated disorders in the future.

6.Cementless Total Knee Arthroplasty in Rheumatoid Arthritis.

We evaluated the clinical and radiologic results of 48 cases (31 patients) who had rheumatoid arthritis of the knee joint and were managed with cementless total knee arthroplasty between November 1988 and December 1994. The mean follow-up period was 3.7 years (range, 2 to 7 years). The knee score of the Hospital for Special Surgery improved from an average of 44 points (range, 5 to 77 points) preoperatively to 83 points (range, 50 to 97 points) at the time of the most recent followup examination. In knee score, 85.4% of cases had excellent or good grade (over 75 points). The range of motion improved from an average of 77 degrees to an average of 103 degrees postoperatively. In tibiofemoral angle, 95.8% of cases were between valgus I degree and 10 degrees. The implants were inserted in an optimal position. Continuous radiolucency was not visible beneath any implant and loosening was not found. In one case, heterotopic ossification was removed to improve the range of motion. There was one case of patellar fracture during operation. Even in relatively poor bone quality in rheumatoid arthritis of the knee, the cementless total knee arthroplasty revealed good clinical and radiologic results.

8.Comparison of Clinical Characteristics and Severity of Glyphosate and Glufosinate Herbicide Poisoning Patients.

PURPOSE: The number of glyphosate and glufosinate intoxication cases are increasing in Korea. This study was undertaken is to compare the clinical manifestations of poisoning by these two herbicides and to document severities and clinical outcomes. METHODS: We retrospectively evaluated cases of glyphosate or glufosinate intoxication among patients that visited our emergency department between January 1, 2013, and December 31, 2017. Incidences of intoxications were analyzed over this five year period, and underlying diseases, transportation, mental state, shock occurrence, inotropics, gastric lavage, charcoal administration, intubation and ventilator therapy, and hemodialysis were investigated. In addition, we included transfer to the intensive care unit, incidences of pneumonia and of other complications, death, and hopeless discharge. RESULTS: There were 119 cases of glyphosate intoxication and 42 of glufosinate intoxication. Levels of consciousness were lower for glufosinate and vasopressor usage was higher due to a high shock rate (p=0.019). In addition, many patients were referred to the ICU for intubation and ventilation. The incidences of pneumonia and of other complications were significantly higher for glufosinate. CONCLUSION: Overall glufosinate intoxication was found to be more severe than glyphosate intoxication as determined by complication and ICU admission rates.

9.Improved Model for Predicting Axillary Response to Neoadjuvant Chemotherapy in Patients with Clinically Node-Positive Breast Cancer.

PURPOSE: Pathological complete response (pCR) of axillary lymph node (LN) is frequently achieved in patients with clinically node-positive breast cancer after neoadjuvant chemotherapy (NAC). Treatment of the axilla after NAC is not well established and the value of sentinel LN biopsy following NAC remains unclear. This study investigated the predictive value of axillary response following NAC and evaluated the predictive value of a model based on axillary response. METHODS: Data prospectively collected on 201 patients with clinically node-positive breast cancer who were treated with NAC and underwent axillary LN dissection (ALND) were retrieved. A model predictive of axillary pCR was developed based on clinicopathologic variables. The overall predictive ability between models was compared by receiver operating characteristic (ROC) curve analysis. RESULTS: Of 201 patients who underwent ALND after NAC, 68 (33.8%) achieved axillary pCR. Multivariate analysis using axillary LN pCR after NAC as the dependent variable showed that higher histologic grade (p=0.031; odds ratio [OR], 2.537; 95% confidence interval [CI], 1.087–5.925) and tumor response rate ≥47.1% (p=0.001; OR, 3.212; 95% CI, 1.584–6.515) were significantly associated with an increased probability of achieving axillary pCR. The area under the ROC curve for estimating axillary pCR was significantly higher in the model that included tumor response rate than in the model that excluded this rate (0.732 vs. 0.649, p=0.022). CONCLUSION: Tumor response rate was the most significant independent predictor of axillary pCR in response to NAC. The model that included tumor response rate was a significantly better predictor of axillary pCR than the model that excluded tumor response rate.

10.Morphological Studies on the Rabbits Ciliary Epithelium by Bunazosin.

Bunazosin is a alpha-1 blocker that is more selective and effective than other alpha-1 receptor blockers. Bunazosin has been shown to lower intraocular pressure but it's mechanism is uncertain. The purpose of this study is to know the morphological changes in the ciliary epithelium by bunazosin. 0.1% Bunazosin was instilled on the eye of the rabbits and the eyes were enucleated on 1, 2, 6 and 24 hours after instillation to find out the tissue response under the electronmicroscopy. As a result, widening of the cell membranes and swelling of the mitochondria were observed in the pigmented and non-pigmented epithelium and these changes were recovered with time and seem to be the morphological changes of bunazosin against aqueous formation in the ciliary body.
