2.Each Case of Benign and Malignant Mucocele of the Appendix.

Mucocele of the appendix is uncomrnon and rarely diagnosed preoperatively. The malignant rountpart-i.e. mucinous cystadenocarcinoma--has the same grross appearance and many micro scopic features in cornmon wilhe the benign form. It rnay be associated with ovarian mucinous cystadenoma of strikingly similar microscopic appearence. A serious complication is a rupture of the mucoeele resulting in pseudomyxoma peritonei. Each case of benign and malignant mucocelr of the appendix falsely diagnosecl as an oovarian tumor before laparotxumy are presented wilh a brief rieview of the literatures.

3.Pseudophakic Endophthalmitis.

The authors treated 12 patients with pseudophakic endophthalmitis over a 5 year period between 1990 and 1994. Common clinical manifestations included anterior chamber inflammation, conjunctival injection, decreased visual acuity, ocular pain and hypopyon. Eight of 12 cases(67%) were culture positive when organisms were grown up from aspirates of the aqueous, the vitreous or both. Three of the culture positive cases were grown up from aspirates of the aqueous, the vitreous or both. Three of the culture positive cases were candida endophthalmitis. Treatment consisted of topical, periocular, systemic and intravitreal injection of antimicrobial agents. In addition, because of poor clinical response of medical conservative therapy, six cases had a vitrectomy. Intraocular lenses were removed in 3 candida endophthalmitis at second operation. Final visual acuity has better in culture negative and low virulent bacterial endophthalimitis than high virulent bacterial and candida endophthalmitis. Visual outcome was better in cured patients who received a single intravitreal injection or a single intravitreal injection in combination with virectomy than the patients who did not receive intravitreal injection or received repeated intravitreal injection. Complications were two eviscerations and a retinal detachment.

4.Clinicopathological observation of the patients with isoniazid responsive inflammatory nodules on the legs.

We performed a clinicopathological study of nineteen patients with chronic inflammatory nodose lesions of the legs which responded to the treatment with isolniazid. The results were summarized as follows: 1. Seven patients had a personal or family history of tuberculcsis and all patients showed a high tuberculin sensitivity. But, no one showed the evidence of active pulmonary tuberculosis. 2. The inflammatory nodules and swelling of the legs were resolved within 12 months in all cases. Resolution of the nodules was more rapid than that of leg swelling. 3. The clinical characteristics of the patients with chronic inflanimatory nodules were the same as those of the cases with erythema nodosum or erytiema induratum reported previously in Korea. The basic histopathologic process of inflarr matory nodules seemed to be vasculitis.

7.A Case of cytophagic Histiocytic Panniculitis.

We describe a case of typical cytophagic histiocytic panniculitis occuring in 10-year-old male. He had recurrent subcutaneous nodules, fever, pancytopenia and abnormal liver function tests since 5 months of his age, and recently showed hemorrhagic diathesis. The histologic picture showed lobular histiocytic panniculitis with "bean bag" cytophagic cells.

8.A Case of Multiple Plexiform Schwannomas.

Plexiform schwannoma is a relatively rare, benign peripheral nerve sheath tumor that can be located either in the deep soft tissues or in the dermis or subcutaneous tissue. This tumor may occur singly or as multiple lesions and may be localized to one anatomic site or diffusely distributed. Plexiform schwannoma should be differentiated with plexiform neurofibroma or other plexiform malignant tumors. We describe a case of a 6-year-old patient with multiple cutaneous plexiform schwannomas who had no other stigmata of neurofibromatosis 1 or family history suggesting a genetic disorder. The histopathological study revealed a tumor composed of multiple intradermal or subcutaneous interlacing and interconnecting fascicles and nodules that vary in size and shape. Characteristic Antoni A type cellular tissue showing frequent nuclear palisading and Verocay bodies were observed within well circumscribed elongated nodules.

9.Intramuscular Lipoma of the Frontalis Muscle.

Intramuscular lipomas are benign soft-tissue mesenchymal tumors which rarely occur in the region of the head. These tumors present as slow-growing, generally painless masses and are easily misdiagnosed initially as epidermal inclusion cysts. We describe a 44-year-old woman who presented with an intramuscular lipoma of the frontalis muscle.

10.A Clinical Study of 1,064 Cases of Midtrimester Genetic Amnicentesis.

Midtrimester genetic amniocentesis has been a gold standard for prenatal diagnosis in antenatal care since last 25 years. After the triple serum marker test was introduced as a prenatal screening method for Down syndrome, the frequency of genetic amniocentesis was increased. OBJECTIVE: To determine the complication, risk of amniocentesis and detection rate of chromosomal abnormality. MATERIAL AND METHODS: A retrospective clinical analysis of 1,064 midtrimester genetic amnicentesis in IL Sin Christian Hospital antenatal clinic from Jan 1995 to Dec 1997. Chi square test was used for the statistical analysis and p value < 0.05 was considered significant. RESULTS: Amnicentesis were significantly increased in the age of 35-39 yrs and 40yrs over. And also the incidence of chromosomal abnormality was higher than younger age group. The indications of amnicentesis were screen positive of triple marker test(43%), advanced maternal age(20.8%), abnormal beta-hCG level, past history of chromosome abnormality or malformed baby and abnormal alpha-FP level in order. Total number of chromosomal abnormalities was 30 and the incidence of chromosomal abnormalities was 2.8%(30/1,064). The complications were developed in 13 cases and fetal loss rate was 0.78%(9/1,064). CONCLUSION: The detection rate of chromosomal abnormality in midtrimester amnicentesis for prenatal diagnosis was high and relatively safe procedure but, we should be attention to more careful manipulation.
