2.A case of pretibial myxedema.

Pretibial myxedema, which consists of localized cutaneous accumulations of acid mucopolysaccharides, oceurs in a few patients with hyperthyroidi';m after its correction by surgery or radioactive iodine therapy. We report a case of pretibial myxedema with exophthalmos in a 36 year old male. He has suffered from hyperthyroidism with bilateral pretibial myxedema for a period of 4 years. He was treated with a subtotal thyroidectomy and excision of both pletibia lesions 1 year ago. The lesions recurred on t,he left pretibial area, as a 11 X 16cm sized, non pitting, non tender, swollen, brownish plaque. The histopathologic findings showed epidermal hyperkeratosis and a considerable arnount of mucin in the mid-dermis. We treat.ed him with intralesional injection of triamcinolone acetonide with marked improvement.

3.Two Cases of Basal Cell Carcinoma Treated with Recombinant Alpha-2 Interferon.

Two patients with basal cell carcinoma (BCC) were treated with recombinant alpha-2 interferon. One patient was a 48-year-old man who had a 8 × 5 an sized ulcer with an elevated border on the face for 10 years, which revealed BCC of the solid type, histopathologically. He received intralesional injections of recombinant alpha-2 interferon (total dose: 51 × 10⁶ IU) for 11 weeks. The biopsy specimen after completion of injections showed a remaining small tumor cell nest in the lesion despite clinical remission. The other patient was a 75-year-old female, who had 1.2 × 1.2 cm-sized, darkly pigmented, nodular BCC of the solid type on the nose for 3 years. She also received intralesional injections of recombinant alpha-2 interferon (total dose: 18 × 10⁶ IU) for 8 weeks. The biopsy specimen after completion of injections did not show any tumor cells.

4.A Case of Generalized Essential Telangiectasia.

Generalized essential telangiectasia is defined as condition in which the veins and cspillaries of the skin undergo primary dilation independently of preceding or coexisting skin lesions of other types. We report herein a case of generalized essential telangiectasia in a 27-year-old female, who has been suffered from generalized erythematous to reddish linear, mottled, or net like telangiectatic macules except face, hands and feet. There were no abnormal laboratory findings and no evidence of systemic disease. Histopathologic examinations showed only dilated telangiectatic vessels in the upper dermis.

5.Three Cases of Malignant Melanoma.

Malignant Melanoma is rare, occupied less than about 0.5% of malignant skin cancer, in Korea. We have experienced three cases for recent 2 years. Case 1 was a 65-year-old woman suffered from 2.0 × 3.0cm sized fungating mass with central ulceration on the left heel for 1 year duration. Left inguinal lymph node was enlarged. She expired 9 months later in spite of treatment with surgical excision, lymph node dissection, and chemotherapy with DTIC. Case 2 was a 37-year-old woman showed 1.0 × 1.5cm sized lobulated, black papules on the right preauricular area for 8 years duration. Satellite lesions developed 1 year ago. There were recurrences on the excisional sites after treatment with wide surgical excision. Case 3 was a 45-year-old woman had 0.5 × 2.Ocm sized, black pigmented patch with satellite lesions on the left sole for 2 years duration. Surgical excision and follow-up for 1 year after treatment showed no recurrence yet.

Tracheoesophageal fistula is a rare disease of abnormal communication between esopha- gus and respiratory system. The common causes are the acquired origins in adult such as trauma, infection of the adjacent organs, malignant tumor, and foreign body. Among the traumatic origins, chemical drug, the procedure of the dilatation on the stenotic area, blunt trauma(fall, collisions), penetrating trauma(bullet, knife), and pressure injury are much more common than others. Recently, trauma and foreign body in the esophagus and bronchus are becoming the main cause of the tracheoesophageal fistula, however the frequency of the development of tracheoesophageal fistula caused by the infectious diseases is getting decreased. Fibrinogen-thrombin glue stimulates the healing process of the wound and the ulcer. We treated a 52-year-old male patient with nan-inalignant tracheoesophageal fistula, who had symtoms of pharyngolaryngeal and chest discomfort concomitant with a paroxysmal cough on swallowing food which were caused by fish bone. The diagnosis of tracheoesophageal fistula was made by the esophagogram, chest CT, and esop aecopy. By using the therapeutic endoscopy with an injection of the fibrinogen-glue, the tracheoesopeal fistula was obliterated completely with dramatic symptomatic improvement. Here we conqluded that this method would be the one of the best methods for the treatment of tracheoesophageal fistula.
