1.Treatment of Fracture of Shaft of Humerus by Ender Nailing

Authors presented the experience of Ender nail treatment for the humeral shaft fractures in 24 patients. The results of treatment in all were satisfactory, though there were four cases of delayed union. Bridging callus was formed at 8.2 weeks on an average. The average clinical union time was 14 weeks. No complications developed during nailing procedure. A case of radial nerve palsy after fracture recovered spontaneously. Longer nails which hit the end of medullary canal of distal fragment distracted the fracture gap, and resulted in delayed union in 4 cases. Therefore, it is recommended to use the proper size of nail to avoid the distraction effect of the inserted nail, and to use two nails at minimum for better fixation. However, when intramedullary Ender nailing is properly done, single nailing also can give consistently good anatomic and functional results.

2.Intraabdominal Desmoplastic Small Cell Tumors with Divergent Differentiation: Report of two cases with immunohistochemical and ultrastructural studies.

We studied two intraabdominal desmoplastic small cell tumors. The patients were two men, 37 and 23 years old, with jaundice and palpable abdominal masses. On exploratory laparotomy, each patient revealed a huge mass in the greater omentum with disseminated peritoneal seeding, measuring 32 cm and 11 cm in its greatest dimension, respectively. The tumor involved the diaphragm, rectal shelf, and cul de sac in case 1, and it involved the porta hepatis, retroperitoneum, and serosal surface of the ascending and transverse colon in case 2. Omentectomy of the huge mass and satellite masses was performed in each patient. Both tumors showed nearly the same histopathologic features. The histologic pattern was suggestive of a metastatic small cell carcinoma, but there was no specific, single primary site. The tumors consisted of variably sized, discrete islands of epithelial-like small cells in dense desmoplastic stroma. The tumor cells revealed divergent epithelial, mesenchymal, and neural differentiation by histologic, immunohistochemical, and electron microscopic observations. Only one cycle of chemotherapy including cisplatin and VP-16 was given in case 1 because of a subsequent hepatic problem, who, thereafter, showed massive intraabdominal recurrent tumors 6 months after diagnosis. In case 2, the poor condition of the patient had made chemotherapy and radiotherapy impossible. Case 2 died of disseminated intravascular coagulation following progressive cachexia 7 months after diagnosis.

3.MR findings of craniopharyngioma.

Craniopharyngioma is a benign, slow-growing tumor that constitues 3-9% of all intracranial tumors, and arises from epithelial remnants of the Rathke's pouch. We analized MR (2.0T) findings of ten cases with surgically proved craniopharyngioma retrospectively. CT was avilable in five cases, and Gd-DTPA was used in six cases. Characteristic findings of craniopharyngioma in MRI included multilocularity and variable signal intensities within each loculus that were more prominent in T1WI. Detection rate of calcification in MR was 60%. Six cases with Gd-DTPA enhancement revealed irregular or rim-like enhancement. MRI provides useful informations regarding the location, extent and biochemical characteristics of the oraniopharyngioma as well as its relationship to the neighboring structures which will be valuable in planning surgical resection.

4.A study on the radiopacity of cavity lining materials for posterior composite resin restoration.

PURPOSE: The aim of this study was to determine the relative radiopacities of cavity lining materials (Resin-modified Glass Ionomer cement, Compomer and Flowable resin) for posterior composite resin restoration. MATERIAL AND METHODS: Resin-modified glass ionomer cement (Fuji II LC, Vitrebond(TM)), Compomers (Dyract , Compoglass, F2000, Dyract(R) flow Compoglass Flow) and Flowable resins (Tetric(R) flow, Aeliteflo(TM) Revolution(TM)) were used. Five specimens of 5mm in diameter and 2mm thick were fabricated with each material. Human molars were horizontally sectioned 2mm thick to include both enamel and dentin. The radiopacities of enamel, dentin, cavity lining materials, aluminum step wedge were obtainded from conventional radiograph and NIH image program. RESULTS: All the tested lining materials showed levels of radiopacity the same as or greater than that of dentin. All compomer tested (Dyract(R), Compoglass, F2000, Dyract(R) flow, Compoglass Flow) and Vitrebond(TM), Tetric(R) flow were more radiopaque than enamel. The radiopacities of Fuji II LC and Revolution(TM) were between enamel and dentin and resin-modified glass ionomer cement, Compomer and Tetric(R) flow were greater than those of Revolution(TM), Aeliteflo(TM) or dentin. The level of radiopacity of the tested materials was variable; those with low radiopacity should be avoided in class II restorations, where a clear determination of recurrent caries by the examining clinician could be compromised. CONCLUSON: Clinician should be able to distinguish these cavity lining materials radiographically from recurrent decay, voids, gaps, or other defects that lead to clinical failure. Utilization of materials ranked more radiopaque than enamel would enable clinicians to distinguish the lining material from tooth structure.

6.Development and Effectiveness of a Program for Relieving 'Hwa-Byung' Symptoms.

PURPOSE: This one group experimentation was designed to develop a program for relieving 'Hwa-Byung'(HB) symptoms and examine its effects on HB symptoms, pain threshold emotions like anger, anxiety and depression. METHOD: The program consisted of three components the change of the cognitive thoughts, the formation of a supportive network, and induction of mind-body relaxation. Sixteen middle-aged women with HB were divided into three groups for group dynamics according to the time of the recruitment. Data was collected for nine months at three time points, before, immediately after, and one month later of its application. RESULT: There were statistically significant differences in the severity level of state anger, state anxiety, depression, and HB symptoms according to the time interval. The means of state anger and state anxiety were reduced after the intervention, but it was slightly increased one month later. The means of depression and HB symptoms were continuously reduced after the intervention and one month later. CONCLUSION: This was the first management program for HB women in Korea. A future study must be done with the research design formethodological strength revision of the program.

8.Expression of iC3b Receptor on Candida albicans and Its Role on Adhesion of the Yeast to Human Dermal Microvascular Endothelium.

BACKGROUND: The adherence of microorganisms to host tissue is an important process in the pathogenesis of fungal infection. A protein that shares antigenic and structural homology with the alpha-subunit of leukocyte adhesion glycoprotein CD11b/CD18, also known as iC3b receptor, Mo-1 or Mac-1, has been isolated from the surface of C. albicans. OBJECTIVE: This study was done to observe the changes in the expression of iC3b receptor on C. albicans by glucose or immunosuppressive agents and to elucidate the effect of glucose and anti-iC3b receptor antibodies on adhesion between human dermal microvascular endothelial cells(HDMEC) and C. albicans. METHODS: We utilized immunofluorescence study and immunofluorescent flow cytometry and the binding assay of C. albicans to cultured HDMEC in vitro and blocking assay using monoclonal antibody were performed. RESULTS: Immunofluorescence study and immunofluorescent flow cytometric analysis demonstrated surface expression of iC3b receptor on C. albicans. The expression of surface iC3b receptor on C. albicans in creased in a dose dependent manner with increasing concentrations of glucose, cyclophosphamide and prednisolone. The adherence of C. albicans to HDMEC correlated positively with glucose levels. The adherence of C. albicans to HDMEC decreased significantly by the treatment with anti-iC3b receptor antibodies. CONCLUSION: The results suggest that iC3b receptors should be involved in the adherence of C. albicans to vascular endothelial cells and may be involved in the pathogenesis of hematogenous candidiasis
