2.A Case of Papillary Eccrine Adenoma.

Papillary eccrine adenoma is a rare sweat gland tumor with a characteristic histopathological appearance and has a benign clinical course. Five cases were reported in Korea since a report by Song et al. in 1988. We report a case of papillary eccrine adenoma in a 38-year-old male who had a firm, dark brown nodule on the medial surface of the left thigh. Microscopically, the tumor was composed of multiple dilated ducts lined by two or more layers of epithelial cells and the luminal cells showed papillary projections into the lumen in some tubules. Immunoperoxidase staining was positive for CEA, S-100, and EMA. We excised the lesion completely and no evidence of recurrence was observed for 6 months.

3.Serum Leptin Levels in Patients with Thyroid Dysfunction.

BACKGROUND: Leptin, the product of ob gene, is an important circulating hormone for the regulation of homeostasis of body weight and enegy expenditure. There was a previous reports that thyroid hormone is one of regulating factors of leptin gene expression in vitro. The aim of this study was designed to evaluate the role of thyroid hormone levels in the regulation of circulating leptin concentrations in human. METHODS: A total 16S subjects were studied; 76 patients with Graves disease, 49 patients with Hashimoto disease and 43 control sujjects. The correlation between thryoid hormone and leptin levels were analyzed and serum leptin levels were compared among the groups which was classified by thyroid functional status. Serum leptin concentratios were measured by radioimmunoassay. RESULTS: There were no significant differences in serum leptin levels between the groups of control, Graves disease and Hashimoto disease. The hypothyroid groups of Graves disease which was induced by excessive antithyroid drug treatment showed significant low levels(5.6 +/-2.8 ng/mL) compared to control(9.6 +/- 5.2 ng/ml) and thyrotoxic groups(10.0 +/- 5,0 ng/mL) CONCLUSION: The hypothyroid patients showed low levels of serum leptin concentrations it may indicate that thyroid horrnone play a role in the appropriate secretion of leptin in human.

4.A Case of Bilateral Lichen Aureus with Good Response to PUVA Therapy.

Lichen aureus is regarded as an uncommon variant of the pigmented purpuric dermatosis and resistant to treatment. Recently several authors reported the PUVA therapy to be effective in the pigmented purpuric dermatosis, but there was no report that the PUVA therapy was tried to the lichen aureus. We tried topical PUVA therapy on a 54-year-old male with bilateral lichen aureus. After 23 times of topical PUVA therapy, the lesion improved markedly. This report may provide a new method of treatment for lichen aureus.

5.Percutaneous Nephrolithotomy: 52 Cases.

A percutaneous nephrostomy tract was used as a conduit to the kidney and upper ureter for extraction of calculi. We have performed percutaneous extraction of renal and upper ureter stones in 52 cases with the use of the percutaneous equipment and ultrasonic lithotrite. Overall success rate was 86.5% and average operating time was 90 minutes and mean hospitalization was 6.9 days. The advantage of this technique are that a skin incision of only 1 cm. is required to remove the stone, hospital days are fewer than with open procedures and postoperative morbidity is minimal. We conclude that percutaneous nephrolithotomy can be the primary choice of treatment in upper urinary tract stones.

6.Hepatic Metastasis from Choriocarcinoma: Angiographic Findings in Two Cases.

We report two cases of hepatic metastases from choriocarcinoma in women of childbearing age in whom imaging studies performed at presentation revealed the presence of liver masses, and who had clinically progressive anemia or intraabdominal hemorrhage. CT demonstrated heterogeneously enhanced liver masses. Characteristic angiographic findings included hypervascular hepatic masses with aneurysmal dilatations of the peripheral hepatic arteries at the arterial phase and persistent vascular lakes at the venous phase.

7.Angiokeratoma of the Vulva.

Angiokeratoma of the vulva is a relatively rare, benign condition that usually presents as single or a few isolated papules mainly on the labia majora. It is histologically identical to angiokeratoma of the scrotum. A 50-year-old woman presented with a one year history of nonpruritic lesions on the vulva. Her skin lesions used to bleed vith subtle trauma. Several dark purplish papules, ranging in diameter from 3 to 5mm, were noticed on both labia majora. A biopsy confirmed the diagnosis of angiokeratoma. A simple electrodesiccation of all lesions was performed under local anesthesia. The patient has remained asymptomatic nd without recurrence for the follow-up period of 10 months without any recurrence.

8.A Case of Chronic Actinic Dermatitis Showing Contact Allergy.

Chronic actinic dermatitis is an uncommon, persistent, light induced eruption occurring mostly on exposed skin. It is eczematous or pseudolymphomatous in nature, probably representing delayed type hypersensitivity to endogerious photoallergens. It usually worsens in summer and affects elderly men. A 72-old-male was presented with a 14 month history of pruritic skin lesions. They were composed of waxy thickened scaly plaques of the face and dorsa of the hands, and depigmented patches of the neck, which worsened when exposed to sun light. We performed photo test and photopatch test. MED of UVA was lowered and the patch and photopatch test revealed only contact sensitivity to Balsam of Peru, perfume mix and fragrance mix. A biopsy specimen showed hyperkeratosis and spongiosis of the epidermis and perivascular lymphocytic and many eosinophilic infiltrations of the dermis. The patient was treated with systemic and topical corticosteroids with marked improvement.

9.Healing of the bone around pure titanium implants without primary bone contact.

Primary fixation is one of the most important factor in establishing adequate osseointegration between implant and bone. To evaluate the initial healing response of bone around implants without primary bone contact, this study was designed to create considerable space between implant and bone in 5 mongrel dogs, about l-year old. After 3 holes of 6 . h in diameter were prepared at the femur neck of the dogs, commercially pure titanium thread type implants(STERI-OSS^(R)), 8mm in length and 3.8mm, 5.0mm and 6 .0mm in diameter, were inserted. Implants were supported by only nonresorbable membrane(Teflon^(R)), and the penetration of upper soft tissue into the gap was inhibited by it. The each implant was positioned in the center of the drilled hole. 9 implants with different diameters were inserted in 3 dogs for histologic observation, and 12 were inserted in 2 dogs for mobility test and removal torque test. Fluorescent dyes were injected in order of Doxycycline, Arizarin Red S, and Calcein at intervals of 2 weeks. At 4-,8-, and 12-week after placement, 3 dogs were sacrificed for histologic observation, and at 8- and 12-week after placement, 2 dogs were sacrificed for mobility test using Periotest^(R) (Simens AG, Bensheim, Germany) and torque test using Autograph AGS-1000D series^(R)(Japan). The result were as follows: 1. The wider the gap between bone and implant was, the less bone maturity was, and the later osseointegration was occurred. Trabecular direction of new bone around implant was changed from parallel to perpendicular to the implant, and the gap was filled with new bone, over time. 2. There was a decreasing tendency over time in the mobility of all implants, but the wider gap between bone and implant was, the smaller decrease of the mobility was. 3. There was a increasing tendency over time in the removal torque gauge of all implants, and the wider gap was, the smaller increase of the removal torque gauge was. The results suggest that osseointegration in case of implant without primary bone contact may be obtained by guided bone regeneration technique with prolonged healing period, but the time of second surgery should be considered carefully.

10.Surface characteristics and biocompatibility of bioinert nitrides ion plated titanium implant.

Even though titanium(Ti) and its alloys are the most used dental implant materials, there are some problems that Ti wears easily and interferes normal osteogenesis due to the metal ions. Ti coated with bioactive ceramics such as hydroxyapatite has also such problems as the exfoliation or resorption of the coated layer. Recent studies on implant materials have been proceeding to improve physical properties of the implant substrate and bio Compatibility of the implant surfaces. The purpose of the present study was to examine the physical property and bone tissue compatibility of bioinert nitrides ion plated Ti. Button type specimens(l4mm in diameter, 2.32mm in height) for the abrasion test and cytotoxicity test and thread type implants(3.75mm in diameter, 6mm in length) for the animal experiments were made from Ti(grade 2) and 316LVM stainless steel. Ti specimens were ion plated with TiN, ZrN by the low temperature arc vapor deposition, and the depth profile of the TiN/Ti, ZrN/Ti ion plated surface was examined by Auger Electron Spectroscopy. Three kind of button type specimens of TiN/Ti, ZrN/Ti and Ti were used for abrasion test, and HEPAlClC7 cells and CCD cells were cultivated for 4 days with the specimens for cytotoxicity test. Thread type implants of TiN, ZrN/Ti, Ti, 316LVM were implanted on the femur of 6 adult dogs weighing l0kg-l3kg. Two dogs were sacrified for histological examination after 45 days and 90 days, and four dogs were sacrificed for the removal torque test of the implants after 90 days. The removal torque force was measured by Autograph (Shimadzu Co., AGS-l000D series, Japan). Abrasion resistance of TiN/Ti was the highest, and that of ZrN/Ti and Ti were Followed. The bioinert nitride ion plated Ti had much better abrasion resistance, compared with Ti. In the cytotoxicity test, the number of both cells were increased in all specimens, and there were no significant difference in cytotoxic reaction among all groups (p>0.1). In histological examination, 316LVM showed the soft tissue engagement in interface between the implant and bone, but the other materials after 45 day5 noted immature new bone formation in the medullary portion along the implant surface, and those after 90 days showed implant support by new bone formation in both the cortical and the medullary portion. The removal torque force of TiN/Ti showed significantly higher than that of Ti(p0.05), and that of 316LVM was lowest among all groups(p<0.05). These results suggest that bioinert nitrides ion plated Ti can resolve the existing problems of Ti and bioactive ceramics, and it may be clinically applicable to human.
