1.Detection of Chlamydia Trachomatis in Nongonococcal Urethritis by Polymerase Chain Reaction.

Chlamydia trachomatis (C. trachomatis) is the most important pathogen of nongonococcal urethritis. C. trachomatis was detected by cell culture, enzyme-linked immunosorbent assay (ELISA) and polymerase chain reaction (PCR) in 145 patients with nongonococcal urethritis. In 5 of 63 antibiotic-treated patients, C. trachomatis was identified by at least one method. C. trachomutid was identified in 34 (41.5%) of 82 nontreated patients. The most common symptom of the 34 patients was painful urination. C. trachomafir was identified in 6 cases (17.6%) of 34 patients by cell culture. And C. trachorrdtis was identified in 9 cases (26.5%) by ELISA, while in 33 cases(97.1%) by PCR. When PCR was performed with urines and urinary swabs collected from 38 patients with nongonococcal urethritis, 11(29%) cases showed positive with urine and 10(26%) cases with urinary swab. These results suggested that PCR with urine showed the higher positive detection rate of C. trachomatis in the patient with nongonococcal urethritis than other methods.

2.Hyperinsulinemic Hypoglycemia After Roux-en-Y Gastric Bypass.

Bariatric surgery is one of the most efficient ways to achieve weight loss and reduce the complications associated with obesity. Severe hypoglycemia can occur after bariatric surgery, though it is rare. Hypoglycemic episodes have so far been poorly studied and the pathophysiology of hypoglycemia has not yet been demonstrated. Currently, there are two different hypotheses; the first is that hypoglycemic episodes are secondary to hyperinsulinemia and result from beta-cell hyperplasia or hyperfunction. The second is that the primary problem is beta-cell failure, which allows glucose concentrations to reach high values. The few therapeutic options that have been validated are also discussed in this study.

4.A Case of a Glomus Tumor Arising in an Unusual Site.

A glomus tumor, is a benign neoplasm, which is derived from a normal glomus body, and participates in thermal regulation. This tumor is relatively uncommon and usually occurs in the peripheral soft tissue, most frequently in the digits, where the glomus bodies are normally present. We experienced a case of a glomus tumor in a 60-year-old man who had a solitary, dome-shaped, grayish blue-colored papule on his back. This was present in an area where a normal glomus body might not usually occur.

7.A Case of Malignant Fibrous Histiocytoma Developing in a Burn Scar.

Burn scar can give rise to different types of malignant tumors, and malignant fibrous histiocytoma (MFH) is very rare among these different tumors. We herein report on a rare case of MFH that developed on the burn scar. A 58-year-old male patient who had been scalded on the chest and abdomen at the age of 8 years was hospitalized with exudative ulcer in the scar area. He was diagnosed with MFH and then he underwent wide excision and flap coverage. No sign of metastatic recurrence was noted during the 6 month follow-up period.

8.Reversible Cause of Rapidly Progressive Dementia as Presenting Symptom of Primary Hyperparathyroidism.

Primary hyperparathyroidism (PHPT) is imbalance of calcium, phosphate, and bone metabolism attributed to an increased secretion of parathyroid hormone (PTH). Although PHPT is mainly associated with musculoskeletal and kidney dysfunction, variable symptoms can be presented in the elderly patients. A 75-year-old man presented with rapidly progressive dementia (RPD). Through etiological work-up of hypercalcemia and increased PTH, parathyroid adenoma was found. Subtotal parathyroidectomy resulted in recovery of cognitive impairment. Primary hyperparathyroidism should be considered in a differential diagnosis of RPD.

9.The Expression of STAT3 in Skin Tumors.

BACKGROUND: Signal transducers and activators of transcription (STAT) proteins are a family of latent cytoplasmic transcription factors that are expressed in many cell types and they regulate the transcription of a broad spectrum of genes that are critically involved in cytokine signaling, cell proliferation and development. Among them, STAT3 has been most strongly implicated in tumorigenesis. OBJECTIVE: This study was designed to measure the expression of STAT3 in different skin tumors. METHODS: The expression of STAT3 was examined by immunohistochemical staining of 61 specimens of skin tumors; 15 cases of seborrheic keratosis, 15 cases of keratoacanthomas, 18 cases of basal cell carcinomas (BCC) and 13 cases of squamous cell carcinomas (SCC). RESULTS: Immunohistochemical analysis of the 61 skin tumor tissue samples revealed a high expression of STAT3 in the SCCs, which are the more dysplastic and invasive skin tumors. Keratoacanthomas and BCCs showed a mild to moderate expression of STAT3 and a low expression was seen in the specimens of seborrheic keratosis. The degree of the expression of STAT3 in SCCs was much stronger than that in the keratoacanthomas and BCCs, with statistical significance (p<0.05). CONCLUSION: These findings provide the roll of STAT3 in the loss of differentiation and the dysplasia of skin tumors. STAT3 is also thought to have an association with the invasion and metastasis of SCC.

10.A Case of Cutaneous Mucinous Eccrine Carcinoma.

Cutaneous mucinous eccrine carcinoma is a rare malignant tumor that is presumably of an eccrine gland origin. We observed a 53-year-old man with an erythematous nodule on his left infraorbital area. A large mucinous pool and characteristic tumor cells were shown on H&E stain, and further specific immunohistochemical stains confirmed the diagnosis. We report here on a case of mucinous eccrine carcinoma and we review the relevant literature.
